Targeting ROCK2 for Atopic Dermatitis Relief.

Atopic dermatitis (AD) remains a significant therapeutic challenge. This Viewpoint summarizes the discovery of compound 10d, a natural product-inspired selective ROCK2 inhibitor, whose design was guided by the identification of a novel hydrophobic surface unique to ROCK2. The compound demonstrates compelling efficacy in a mouse model of MC903-induced AD, positioning selective ROCK2 inhibition as a promising strategy for this condition.

Hypogammaglobulinemia during infancy and atopic dermatitis.

Background: Hypogammaglobulinemia during infancy (HI) is often transient, resolves with time, and typically does not associate with a specific immunologic deficiency. If hypogammaglobulinemia persists, then it may be secondary to an immunologic disorder and be associated with significant clinically diagnosed infections. When phenotyping HI, any implications of associated atopic dermatitis (AD) can be clinically helpful. Objective: The purpose of this study was to explore the clinical and immunologic differences between patients who have HI and AD versus those with AD alone or with HI alone. Me

Contact Allergy Screening for Atopic Dermatitis.

Atopic dermatitis (AD) and allergic contact dermatitis (ACD) are common inflammatory skin diseases in both children and adults that present similarly and often coexist. Patch testing is the gold standard for establishing the diagnosis of ACD and can often help distinct between the 2 conditions. Patch testing is more challenging in patients with underlying AD due to potential for angry back reactions. In this review, we discuss the current evidence and guidelines regarding the screening for contact allergies in patients with AD. We also discuss the most frequent relevant allergens in adults and

Ocular surface disease in atopic dermatitis.

Atopic keratoconjunctivitis (AKC) is a severe ocular surface disease (OSD) that significantly affects patients' quality of life and can lead to blindness. AKC is associated with atopic dermatitis (AD), a common chronic inflammatory skin disease affecting both children and adults. This thesis investigates the prevalence, clinical features and underlying mechanisms of OSD and AKC in AD patients, aiming to improve the understanding and management of these conditions. Two distinct questionnaire studies were conducted to assess the association of OSD symptoms in AD patients within the general popul

Comprehensive Ophthalmological Evaluation in Atopic Dermatitis.

INTRODUCTION: Atopic dermatitis (AD), a chronic type 2 inflammatory skin disease, is frequently associated with ocular surface diseases (OSD) which may appear or worsen under anti-type 2-targeted treatments. However, the exact prevalence of OSD and the ophthalmology referral criteria remain ill-defined in AD patients before initiating such biotherapies. We aimed to characterize the prevalence, the nature and the factors related to OSD development in AD that may justify an ophthalmological management. METHODS: A total of 98 consecutive AD inpatients without biological treatment were retrospect

Tacrolimus Ointment in Periorbital Atopic Dermatitis.

Periorbital atopic dermatitis (AD) is a common sign in ophthalmological practice and usually has a persistent and relapsing course. Treatment with topical corticosteroids has various side effects associated with their usage. Tacrolimus topical ointment has unique immunomodulatory properties that decrease skin inflammation and pruritus in AD. In this case series, we present a prospective case series of five patients (three males and two females) who received topical application of tacrolimus ointment 0.1-0.03% in the periorbital area twice daily for one to four weeks. The pre- and post-treatmen

Atopic dermatitis across the life course.

Atopic dermatitis, the most common chronic inflammatory skin disease, can occur at any age, and patterns of disease activity vary over time. Both prevalence and incidence are highest in infancy and early childhood, followed by a second peak in older adulthood. Birth cohort studies from European countries following children through adolescence have identified subgroups of patients with early-onset persistent disease, early-onset resolving disease, and later-onset disease. Parental history of atopy and genetic factors are among the most consistent predictors of more persistent disease. Studies h

Atopic Dermatitis: Striving for Reliable Biomarkers.

Atopic dermatitis (AD) is a highly heterogeneous inflammatory disease regarding both its pathophysiology and clinical manifestations. However, it is treated according to the "one-size-fits-all" approach, which may restrict response to treatment. Thus, there is an unmet need for the stratification of patients with AD into distinct endotypes and clinical phenotypes based on biomarkers that will contribute to the development of precision medicine in AD. The development of reliable biomarkers that may distinguish which patients with AD are most likely to benefit from specific targeted therapies is

Food allergy testing in atopic dermatitis.

Food allergy and food-related worsening of dermatitis can occur in patients with atopic dermatitis (AD). We reviewed the relationship of AD with food allergen hypersensitivity and the risks and benefits of food allergen testing and avoidance in patients with AD. Skin prick testing and specific immunoglobulin E to aeroallergens may identify patients with immediate hypersensitivity. Atopy patch tests may detect non-immunoglobulin E-mediated reactions but are not standardized or routinely used. Younger children with more severe AD in whom the optimal management failed may have food-triggered AD.

Current pharmaceutical developments in atopic dermatitis.

Atopic dermatitis (AD) is an inflammatory, pruritic, chronic or chronically relapsing skin disease that typically begins in early childhood and is occurring frequently in families with other atopic diseases (bronchial asthma and/or allergic rhino-conjunctivitis). Thanks to immunological and neurobiological research, the era of new treatments is coming as well as it occurred with psoriasis 15 years ago. Many treatments targeting cytokines (IL-4, IL-13, IL-31, TSLP) or neurotransmitters (substance P, opioids) or their respective receptors as well as phosphodiesterase-4 or the Jak/Stat pathways a

[Atopic dermatitis and general medical comorbidities].

Atopic dermatitis (AD) is one of the most frequent chronic inflammatory skin diseases, characterized by pruritic eczematous lesions. Due to the fact that AD is accompanied by signs of systemic inflammation and that an increased number of novel systemic treatment options are currently emerging, research into general medical comorbidities in moderately to severely affected AD patients has received great impetus in recent years. These studies have confirmed an increased risk of atopic diseases such as allergic asthma in AD patients. Furthermore, an association between AD and dermatological diseas

Long-Term Treatment of Atopic Dermatitis.

Many patients with mild to moderate atopic dermatitis (AD) are managed by identifying and avoiding allergens and irritants, ensuring skin moisturization, and graded use of topical corticosteroids and/or calcineurin inhibitors. There is little consensus on the next step. Most systemic therapies are "off label" in the United States and include phototherapy, cyclosporine, mycophenolic acid precursors, azathioprine, and methotrexate. The decision to use these therapies should be based on efficacy and safety readouts from well designed, long-term trials. This article reviews the long-term randomize

Systemic therapy of childhood atopic dermatitis.

Atopic dermatitis (AD) is a common childhood inflammatory disease that, in a small percentage of cases, can become severe enough to require potent systemic treatment. Many trials have been conducted with systemic agents for the treatment of severe pediatric AD; we review the evidence here. Although corticosteroids are widely used in practice, they are not generally recommended as a systemic treatment option for AD in children. Most patients experience a relatively rapid and robust response to cyclosporine. Treating children with cyclosporine long term is troubling; however, azathioprine, mycop

Risk factors for developing atopic dermatitis.

The aim of this thesis was to investigate possible risk factors affecting the development of AD. AD is a frequent disease among children and has a substantial impact on the lives of both the child and its family. A better understanding of the disease would enable better treatment, prevention and information to the families involved. Previous risk factor studies have been hampered by an unsuitable study design and/or difficulties in standardization when diagnosing AD, which limit their conclusions. In paper I, we conducted a traditional cross-sectional analysis testing 40 possible risk factors

Addressing psychosocial aspects of atopic dermatitis.

Moderate to severe atopic dermatitis (AD) negatively affects patients and their families. Pruritus, scratching, and sleep problems are common complaints linked to disturbed quality of life. Treatment is complex, and nonadherence rates are high. This article reviews the effect of AD on patients and their families and intervention strategies that have some success in improving quality of life. A treatment model for addressing the psychosocial effect of moderate to severe AD within a multidisciplinary setting is suggested herein.

Severe atopic dermatitis treated with everolimus.

BACKGROUND: Patients with severe atopic dermatitis (AD) often require treatment with oral immunosuppressive drugs. Everolimus is a rapamycin-derived macrolide with immunosuppressive and antiproliferative effects. Everolimus demonstrated efficacy not only in the prophylaxis of organ rejection in kidney transplant patients, but also in decreasing disease activity in psoriasis patients. OBJECTIVE: To evaluate whether everolimus is an effective treatment in patients with severe AD. METHODS: Two patients with severe AD were treated with everolimus in combination with low-dose cyclosporin A (CsA)

Keratinocytes in atopic dermatitis: inflammatory signals.

Atopic dermatitis (AD) is a chronic inflammatory skin disorder that usually predates the development of allergic airway disease. In most cases, this is thought to be an allergen-driven disease with prominent roles played by antigen presenting cells and effector Th2 cells. But keratinocytes, by virtue of their location, provide an important window to the environment and are also thought to contribute to the development of AD. In this review, we discuss several biologic attributes of keratinocytes that are relevant for AD: 1) intrinsic defects in barrier function, 2) production of inflammatory m
