Comparison of Dexmedetomidine versus Propofol for Sedation after Uvulopalatopharyngoplasty.

BACKGROUND: Adequate sedation is important in the post-anesthesia care unit (PACU) following uvulopalatopharyngoplasty (UPPP) to ensure patient comfort and decrease the duration of mechanical ventilation (MV), PACU stay, and bleeding. This study aimed to compare dexmedetomidine and propofol as sedatives after UPPP in the PACU. MATERIAL AND METHODS: We randomized 124 mechanically ventilated adults following UPPP who were managed in the PACU of the General Hospital of the Shenyang Military Region between January 2014 and June 2014, to receive either dexmedetomidine or propofol. The patients in

Protective effects of dexmedetomidine on intestinal ischemia-reperfusion injury.

Dexmedetomidine (DEX) has been hypothesized to possess anti-oxidative properties that may mitigate the damage caused by ischemia-reperfusion (IR) injury. The aim of the present study was to examine the effects of DEX on intestinal contractile activity, inflammation and apoptosis following intestinal IR injury. Intestinal IR injury was induced in rats by complete occlusion of the superior mesenteric artery for 60 min, followed by a 60-min reperfusion period. Rats received an intraperitoneal injection of 25 µg/kg DEX at 30 min prior to the mesenteric IR injury. Following reperfusion, segments of

Dexmedetomidine improves early postoperative cognitive dysfunction in aged mice.

Postoperative cognitive dysfunction (POCD) is a frequent complication following major surgery in the elderly. However, the exact pathogenic mechanisms are still unknown. Dexmedetomidine, a selective alpha 2 adrenal receptor agonist, was revealed anesthesia and brain protective role. The present study aimed to examine whether dexmedetomdine protects against POCD induced by major surgical trauma under general anesthesia in aged mice. In the present study, cognitive function was assessed by Y-maze. Proinflammatory cytokines interleukin-1β (IL-1β) and tumor necrosis factor (TNF-α), apoptosis-relat

[Gasserian ganglion block for trigeminal neuralgia under dexmedetomidine sedation].

We report a case of successful procedural sedation using dexmedetomidine (DEX) in a 68-year-old woman undergoing left gasserian ganglion block for intractable trigeminal neuralgia. DEX was chosen to provide an effective sedation and clear communication about the injection of drugs or thermocoagulation. After 15 minutes of DEX administration at 0.8 microg x kg(-1) x hr(-1), nerve block needle insertion was bearable. The patient could feel a weakening of the sensation of her left maxilla and mandible after receiving a 0.4 ml mepivacaine test dose. High-frequency thermocoagulation elicited tolera

Dexmedetomidine pharmacokinetics in pediatric intensive care--a pooled analysis.

BACKGROUND: Published dexmedetomidine pharmacokinetic studies in children are limited by participant numbers and restricted pathology. Pooling the available studies allows investigation of covariate effects. METHODS: Data from four studies investigating dexmedetomidine pharmacokinetics after i.v. administration (n = 95) were combined to undertake a population pharmacokinetic analysis of dexmedetomidine time-concentration profiles (730 observations) using nonlinear mixed effects modeling (NONMEM). Estimates were standardized to a 70-kg adult using allometric size models. RESULTS: Children had

Central apnoea after balanced general anaesthesia that included dexmedetomidine.

Dexmedetomidine is an alpha(2)-adrenoreceptor agonist that, in spite of its potent sedative, amnesic, and analgesic properties, has minimal respiratory depressant effect. Even at doses adequate for general anaesthesia, it does not cause central apnoea. Thus, it has been claimed that "combining alpha(2)-agonists with opiate narcotics or non-steroidal anti-inflammatory drugs can enhance the analgesic efficacy without increasing the respiratory depressant effect of the latter" and "the combination of alpha(2)-adrenoceptor agonists with opioids does not lead to further ventilatory depression". We

Intranasal esketamine plus dexmedetomidine versus dexmedetomidine alone for emergence delirium in pediatric patients: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Intranasal dexmedetomidine is commonly used preoperatively in pediatric anesthesia to reduce agitation and emergence delirium. Esketamine, with sedative and analgesic ef-fects and minimal respiratory depression at clinical doses, is also widely used in chil-dren. However, current evidence remains limited regarding the efficacy and safety of combining intranasal esketamine with dexmedetomidine versus dexmedetomidine alone in improving cooperation during anesthesia induction and reducing postoperative complications. METHODS: This meta-analysis adhered to PRISMA guidelines and was re

Effects of midazolam and dexmedetomidine on liver and intestinal mitochondrial respiration.

BACKGROUND: Mitochondrial dysfunction frequently occurs in critically ill patients and is thought to contribute to metabolic shutdown and hepatic or gastrointestinal failure. In addition to hypoxemia and inflammation, sedatives are considered potential contributors to further impairment of mitochondrial function. OBJECTIVE: This study investigated the effect of the sedatives midazolam and dexmedetomidine on liver and colon mitochondrial function. Especially colon mitochondrial affection by both drugs was evaluated here for the first time. DESIGN: Liver and colon tissue homogenates from healt

Dexmedetomidine for the treatment of sepsis-associated encephalopathy: Mechanism and prospects.

Sepsis-associated encephalopathy (SAE) is a severe central nervous system complication that is secondary to sepsis and is characterized by a poor prognosis, high mortality, and multiple systemic manifestations. Although the specific etiology remains incompletely understood, SAE typically presents with varying degrees of neurological dysfunction. The complex mechanisms underlying SAE significantly influence patient outcomes. Recent studies have emphasized the roles of bloodbrain barrier (BBB) disruption, microglial activation, mitochondrial dysfunction, apoptosis, inflammatory responses, and ox

Subcutaneous Dexmedetomidine for Refractory Symptoms in a Hospice Inpatient Unit.

CONTEXT: Pain, agitation, and other refractory symptoms occur commonly in palliative patients. Dexmedetomidine has anxiolytic, analgesic, and sedating effects; however, its use remains uncommon in palliative care. In New Zealand, the Hospice Waikato Inpatient Unit has increasingly used dexmedetomidine since 2019 when symptoms are refractory and deep sedation is not desirable due to patient and prognostic factors. OBJECTIVES: To describe use of subcutaneous dexmedetomidine infusions in the Hospice Waikato Inpatient Unit, noting patient demographics, treatment characteristics, safety factors, a

Effect of Perioperative Dexmedetomidine in Cardiac Surgery: A Narrative Review.

Cardiac procedures carry a higher perioperative risk than other operations for major adverse cardiovascular events and kidney injury, especially since patients with multiple comorbidities have been accepted as candidates for undergoing surgical treatment. Dexmedetomidine (DEX) is an alpha-2 agonist and has been widely used as an adjuvant anesthetic in clinical anesthesia for many different types of operations, including cardiac surgery. While it can be associated with bradycardia as well as hypotension in hypovolemic patients, DEX has been shown to reduce surgical complications like atrial fib

Effect of Dexmedetomidine on Fluid Resuscitation in Burn-Injured Patients.

Fluid creep occurs when resuscitation after extensive burn injury reaches volumes higher than predicted. Since this has been described in patients with high opioid requirements, continuous analgesics and sedatives, including dexmedetomidine, have been avoided during resuscitation. This study sought to describe the impact of dexmedetomidine on fluid resuscitation requirements. This retrospective cohort study included adult patients with burns greater than 20% total body surface area (TBSA) resuscitated between January 2017 and June 2022 at a regional burn center. Patients deceased within 48 hou

Dexmedetomidine for agitation in dementia: Current data and future direction.

BACKGROUND: The incidence and prevalence of dementia, and thus dementia-related behavioral and psychological symptoms, are increasing significantly. Currently, there are limited safe and efficacious options for treating these symptoms. Dexmedetomidine has been used for agitation related to delirium and showed significant benefit in prior studies. This raises the question whether dexmedetomidine could also provide a safe and effective treatment for BPSD, including agitation related to dementia. METHODS: Our team searched PubMed, Cochrane Database, and Ovid with the terms dexmedetomidine and de
