Lessons learned from Hurricane Andrew: recommendations for care of the elderly in long-term care facilities.

We report on the experience of a 500-bed, long-term care facility in Miami, Fla, which provides housing and nursing care units for patients--ranging from those who are independently ambulatory to those who are acutely ill and feeble--in preparing for, during, and in the immediate aftermath of Hurricane Andrew, which struck on August 24, 1992. The problems encountered included a massive influx of evacuated elderly to the facility, facility isolation, loss of electrical power, loss of running water, special dietary needs, and limited professional staffing due to personal property losses or loss

Research Priorities in Perioperative Nursing Across the Lifespan: An e-Delphi Consensus Study.

Background/Objective: Perioperative nursing care encompasses a diverse patient population with varying needs, anatomical and physiological characteristics, and differing levels of vulnerability, demanding specialized knowledge and skills. This study aims to identify key research priorities in perioperative nursing across the lifespan. Methods: A two-round e-Delphi study was conducted across three Portuguese hospitals, involving expert perioperative nurses (87 in the first round and 69 in the second). Participants rated the importance of various research topics on a 5-point Likert scale about s

Strengthening resilience in nursing homes: The role of emergency preparedness in mitigating COVID-19 outcomes.

This study investigates the relationship between compliance with Centers for Medicare and Medicaid Services (CMS) emergency preparedness standards and COVID-19 morbidity and mortality rates in U.S. nursing homes. Given the severe impact of COVID-19 on nursing home residents, this research addresses how preparedness influences pandemic outcomes. Using publicly available datasets from CMS and FEMA covering the period 2017 to 2021, a retrospective and longitudinal analysis was conducted. Multiple linear regression analyses were employed to evaluate the association between emergency preparedness d

Nursing Unit Communication During a US Public Health Emergency: Natural Experiment.

BACKGROUND: In the second half of 2014, the first case of Ebola virus disease (EVD) was diagnosed in the United States. During this time period, we were collecting data for the Measuring Network Stability and Fit (NetFIT) longitudinal study, which used social network analysis (SNA) to study relationships between nursing staff communication patterns and patient outcomes. One of the data collection sites was a few blocks away from where the initial EVD diagnosis was made. The EVD public health emergency during the NetFIT data collection time period resulted in the occurrence of a natural experim

Emergency Department Use Among Assisted Living Residents After Hurricane Irma.

OBJECTIVES: Nursing home residents are especially vulnerable to adverse outcomes after a hurricane. Prior research suggests that emergency department (ED) visits increase among community-residing older adults after natural disasters. However, little is known about the impact of hurricanes on the large population of older adults residing in assisted living (AL) settings, particularly the influence of storms on the rates and causes of ED visits. We examined whether rates of ED use for injuries and other medical reasons increased after Hurricane Irma in 2017 among AL residents in Florida. DESIGN

[Local and regional in-hospital trauma care following fireworks depot explosion in Enschede].

UNLABELLED: On Saturday 13 May, 2000 at about 15:30 h, the Dutch city of Enschede was struck by the explosion of a midtown firework depot. Twenty-two people were killed and almost 1,000 people were wounded. A complete district with 1,000 houses was destroyed. In total, 527 victims were treated in one of the regional hospitals, 76 (14%) of whom were admitted and 451 (84%) were treated as outpatients. Of the clinically treated victims, 11 patients were triaged as T1 and needed immediate intervention, 63 patients were triaged as T2 where treatment within six hours was indicated, and two T3 patien

Antimalarial drugs, pregnancy and lactation.

Disease activity has been demonstrated to be one of the major factors contributing to fetal loss in SLE patients, and discontinuation of antimalarial therapy can precipitate a flare of disease. It is therefore important to determine whether it is safe to continue antimalarial therapy throughout pregnancy. We have previously stated that we consider lupus patients and their fetuses to be at risk for disaster if antimalarial therapy is discontinued during pregnancy, and it has been our experience that lupus patients can produce normal offspring even if they are taking daily chloroquine or hydroxy

The Army chemical/biological SMART (SMART-CB) team: the nurse's role.

A chemical or biologic attack probably will be covert, rather than overt. Because presenting signs and symptoms may mimic minor nonspecific illnesses or naturally produced disease syndromes and may not appear for several days, it is likely that nurses in emergency rooms and primary care settings will be among the first to come into contact with victims of a chemical or biologic agent exposure. Early recognition, reporting, decontamination, self-protection, prophylaxis, and treatment are imperative. After the terrorist attack on September 11, 2001, the anthrax incidents, and the heightened publ

Conceptualization of a Health Care Coalition Framework in Georgia Based on the Existing Regional Coordinating Hospital Infrastructure.

The watershed events of September 11, 2001; the anthrax attacks; Hurricane Katrina; and H1N1 necessitated that the United States define alternative mechanisms for disaster response. Specifically, there was a need to shift from a capacity building approach to a capabilities based approach that would place more emphasis on the health care community rather than just first responders. Georgia responded to this initiative by creating a Regional Coordinating Hospital (RCH) infrastructure that was responsible for coordinating regional responses within their individual geographic footprint. However, i

New challenges for public health care: biological and chemical weapons awareness, surveillance, and response.

Recent events in the United States have demonstrated a critical need for recognizing nurses and emergency health care providers as important elements of the nation's first line of defense and response against terrorist attacks involving biological, chemical, or radiological weapons. The anthrax letter attacks of September/October 2001 demonstrate the importance of vigilance and attention to detail while interviewing and attending patients and when entering, reviewing, and cataloging patient records. Nursing professionals, emergency care responders, and physicians can perform a crucial role in

[SURGICAL "NEVER EVENTS" IN THE ANTERIOR CERVICAL APPROACH].

This article deals with incidents which involved damages that could have been prevented. This includes patients who have been suffocated and have suffered irreversible damage, unfortunately, as far as we know, due to the powerlessness of the medical staff who did not act suitably. The disasters have happened as a result of upper respiratory airway obstruction by a blood clot (retropharyngeal hematoma) that developed as a complication after an anterior cervical surgery (spine/carotid artery) or after a sharp cervical injury. The main question that derives from the following cases is whether it

[A new structure for mental health and welfare in the Soso area to promote the recovery of people in Fukushima from the 3.11 earthquake and nuclear power plant accident].

Immediately after the 3.11 Earthquake and Fukushima Dai-Ichi Nuclear Power Plant Accident, 5 hospitals with psychiatric beds within a 30-km radius of the nuclear power plant were ordered to move their inpatients to other hospitals outside the 30-km zone. As a result, more than 800 inpatients in total were transferred to other hospitals within or outside Fukushima Prefecture, and the 5 hospitals were closed. In addition, 3 psychiatric clinics within the 30-km radius stopped operating temporarily. In March of 2011, several volunteer members of the Department of Neuropsychiatry and the Mental Hea

Designing a disaster.

A mass casualty simulation was developed and implemented for senior-level nursing students in a large baccalaureate program. This simulation was developed to introduce students to rapid triage in an interactive and immersive experience. The purpose of the simulation was to provide students with a realistic, hands-on experience in a safe environment. Unlike other similar exercises, all students participated in the health care provider role rather than as observers or victims. Didactic content regarding triage and a short video preceded the surprise simulated "bus crash." The element of surprise

COVID-19 and First Responder Social Workers: An Unexpected Mental Health Storm.

COVID-19 has disproportionally affected people living in poverty; new immigrants; and those living in healthcare settings (hospitals, clinics, and nursing homes), shelters, detention centers, and prisons. The pandemic has also significantly impacted social workers who face hidden mental health consequences. The resultant feelings and behaviors are similar to disaster-induced trauma, including excessive rumination, hypervigilance, exhaustion, and excessive crying as a result of the adrenaline-driven fight-or-flight response. This article aims to provide information for social workers to protect

The role of a regional trauma system in the management of a mass disaster: an analysis of the Keystone, Colorado, chairlift accident.

On December 14, 1985, the Teller chairlift at the Keystone, Colorado, ski area collapsed, throwing 60 of the 372 people aboard to the ground from heights up to 50 feet. Initial triage and management of the victims was carried out by the local ski patrol, the on-duty physician at the area's Snake River Health Services Clinic, and by volunteer physicians and nurses present at the scene. Thirty-three people required immediate evacuation to hospitals, most of them being transported 75 miles by helicopter air ambulance to level I and II trauma centers in the Denver metropolitan area. TRAINING PROGRAM FOR NURSING STAFF REGARDING VIRAL HEMORRHAGIC FEVERS IN A MILITARY HOSPITAL.

Viral hemorrhagic fevers (VHFs) refer to a group of illnesses caused by several distinct families of viruses. In general, the term "viral hemorrhagic fever" is used to describe a severe multisystem syndrome (multisystem in that multiple organ systems in the bpdy are affected). Characteristically, the overall vascular system is damaged, and the body's ability to regulate itself is impaired. These symptoms are often accompanied by hemorrhage (bleeding); however, the bleeding is it rarely life-threatening. While some types of hemorrhagic fever viruses can cause relatively mild illnesses, many of
