Cancer of the colon in an egg donor: policy repercussions for donor recruitment.

This paper describes the tragic case of a young woman who died of cancer of the colon after successfully donating eggs to her younger sister. Although there is no direct link between her operation and the subsequent development of bowel carcinoma, this case imparts a feeling of unease when seen in conjunction with other cases reported during the last few years. It is a reminder that little is known of the long-term consequences of some aspects of assisted conception. Women undergoing ovarian stimulation for themselves or a matched recipient have the right to be advised, in an agreed format, th

Mater semper certa est: motherhood is always certain.

The development of oocyte donation has led to a reexamination of the facets of motherhood: the genetic, gestational, and psychosocial contributions. In addition, the practice of oocyte donation has prompted a consideration of the unique psychosocial and legal aspects of this form of family building. In this section of Views and Reviews a summary and discussion of the long-term psychosocial adjustment of oocyte donor-conceived children and their parents is presented. Next, the current permeability around donor anonymity is investigated and questions regarding whether donor anonymity can still e

Preference for anonymity in sperm donation for artificial insemination: an experience from low-resource settings in Nigeria.

OBJECTIVE: Anonymous sperm donation is a common practice in Nigeria with its associated legal and ethical challenges. This study aimed to investigate infertile couples' opinions about issues of sperm donor anonymity and to determine factors that might influence their preferences. METHODS: A cross-sectional, multicentered, questionnaire-based study was conducted among infertile couples attending infertility clinics in three tertiary hospitals in the south-eastern region of Nigeria over a period of 6 months. RESULTS: A total of 450 infertile couples were recruited consecutively from the three

Unique Associations between Insulin-Like Growth Factor Binding Protein-1, Insulin-Like Growth Factor-1 and T Cell Immunoglobulin Mucin 3 in Successful Twin Pregnancies Conceived with Donor Oocytes.

Background and Objectives: To investigate if pregnancies conceived using an oocyte donor necessitate an alteration in immune regulation, we compared concentrations of insulin-like growth factor binding protein (IGFBP)-1, insulin-like growth factor (IGF)-1 and T cell immunoglobulin mucin-3 (Tim-3) in women with ongoing successful twin pregnancies conceived spontaneously, using assisted reproductive technologies that utilized homologous oocytes or with donor oocytes. Differences in levels of these immune modulatory proteins may be magnified and easier to detect in twin as compared to singleton p

For whom the egg thaws: insights from an analysis of 10 years of frozen egg thaw data from two UK clinics, 2008-2017.

PURPOSE: To better understand the characteristics of patients who returned to thaw their frozen eggs to attempt conception and their outcomes. METHODS: A retrospective analysis of clinical records for all own egg thaw patients in two UK fertility clinics across 10 years, 2008-2017. RESULTS: There were 129 patients who returned to thaw their eggs, of which 46 had originally frozen their eggs for social reasons and 83 for a variety of clinical, incidental, and ethical reasons (which we have called "non-social"). Women who had frozen their eggs for social reasons were single at time of freeze,

[Evolution of assisted reproductive technologies].

When natural conception is impossible and the underlying problem cannot be treated, medical intervention can reproduce the steps necessary for fertilization and early embryo development. The first known medical action in the field of human reproduction took place at the end of the 18th century, in the form of artificial insemination with the husband's semen, thus dissociating sexual intercourse from procreation. A further upheaval occurred at the end of the 19th century, with the use of donor sperm, separating the notions of genetic descent and parenthood. In the second half of the 20th centur

Novel insights into non-HLA alloimmunity in kidney transplantation.

Recognition of non-self structures on donor cells represents the main immunological barrier in solid organ transplantation. The human leukocyte antigens (HLA) are considered the most important non-self (allo)antigens in transplantation. Long-term graft attrition is mainly caused by the formation of alloreactive antibodies that are directed against non-self structures (i.e., epitopes) on cell surface proteins. Recently published data provided evidence for a similar importance of non-HLA mismatches between donors and recipients in acute rejection as well as long-term kidney allograft survival. T

Preoperative liver donor evaluation: Imaging and pitfalls.

This article discusses the rationale behind living (related) donor liver transplantation, the role of imaging in the preoperative evaluation of the potential donor, and the currently available imaging modalities for fulfilling this task. Furthermore, the normal hepatic vascular and biliary anatomy, as seen on imaging, is reviewed and the most common anomalies are highlighted. Finally, critical concepts in the diagnostic evaluation of a donor are discussed with special emphasis on how to accurately measure liver volumes.

The Approach of Fertility Physicians to Donor Sperm Treatments: A Worldwide Survey.

OBJECTIVE: Many women presenting for treatment with donor sperm have never attempted conception, and are therefore presumed fertile. With no clear guidelines, treatment can be influenced by factors like patient age and costs. We aimed to explore fertility physicians' attitudes and clinical practices regarding donor sperm treatments through a global survey. METHODS: We conducted an anonymous international web-based survey of fertility clinicians. The survey addressed various aspects of donor sperm treatment in women without previous infertility. RESULTS: A total of 211 clinicians participated

Gamete donation, information sharing and the best interests of the child: an overview of the psychosocial evidence.

This paper overviews key empirical findings from social science research regarding the impact of gamete donation on child wellbeing. In particular, the paper addresses current regulatory debates concerning information sharing and the best interests of the child by considering psychosocial aspects of telling--or not telling--children about their donor conception and the identity of their donor. The paper identifies three core sets of empirical, ethical and policy concerns underpinning these debates relating to (i) the psychosocial impact of gamete donation per se on child wellbeing, (ii) the ps

DNA Genotyping of Nonmolar Donor Egg Pregnancies With Abnormal Villous Morphology: Allele Zygosity Patterns Prevent Misinterpretation as Complete Hydatidiform Mole.

DNA genotyping is the gold standard diagnostic test to distinguish hydatidiform moles from nonmolar but morphologically abnormal products of conception (POC). The test is based on comparison of alleles at 15 short tandem repeat loci in the chorionic villi of the POC to those in the maternal decidual tissue. If alleles in the POC are not present in the decidua, then the most concerning interpretation is that the POC has a paternal uniparental genome diagnostic of a complete hydatidiform mole (CHM). However, a nonmolar pregnancy from a donated egg would also appear the same because the maternal

Donor insemination programmes with personal donors: issues of secrecy.

This study involved 46 recipients and donors in personal donor programmes interviewed anonymously by postal questionnaire and interview: 38% (30/80) of possible recipients responded. The total number of people told about the donor involvement ranged between two and 78, with no significant gender difference. Relationships had changed for half of the participants in the programmes with 75% reporting that they had developed a closer relationship and 25% reporting a deterioration. Contact between couples and donors was seen as being in the original role of family friend or relative rather than as

Perineal perforator-based island flaps: the next frontier in perineal reconstruction.

BACKGROUND: Perineal reconstruction is a challenging prospect. Conventional flap reconstruction often involves the sacrifice of a source artery and muscle, resulting in significant donor morbidity. Perforator flaps sought to overcome this but required tedious dissection. In this article, the authors introduce a new concept in perineal reconstruction using perforator-based island flaps. METHODS: The perineal perforator-based island flap is raised based on perforators that most commonly arise from the perineal artery. The flap is designed in the inguinal and gluteal folds in order to achieve ae

Can fertility service providers justify discrimination against lesbians?

In Britain, for the last 25 years, lesbians have been seeking access to fertility services to achieve pregnancy. The problem for this particular group of clients is that often they are refused access to fertility services because of their sexuality. The reasons for treating lesbians less favourably than heterosexual clients are twofold: (1) donor insemination is defined as a treatment for infertility; and (2) service providers have a legal requirement to take into account the child's need for a father, according to Section 13(5) of the Human Fertilisation and Embryology Act 1990. The question

Adolescents with open-identity sperm donors: reports from 12-17 year olds.

BACKGROUND: Donor insemination programs can include 'open-identity' sperm donors, who are willing to release their identities to adult offspring. We report findings from adolescent offspring who have open-identity donors. METHODS: Using mail-back questionnaires, youths from 29 households (41.4% headed by lesbian couples, 37.9% by single women, 20.7% by heterosexual couples) reported their experience growing up knowing how they were conceived and their interest in the donor's identity. RESULTS: Most youths (75.9%) reported always knowing, and were somewhat to very comfortable with their conce

The effect of priming with a love concept on blood donation promise.

This field study on blood donor behavior tests the effectiveness of semantic priming on donor intention and commitment. Using face-to-face interactions, participants were primed with the concept of love and solicited to promise blood to the French National Blood Bank. Results showed a significant effect on willingness to donate blood and on donor commitment. The relatively simple and easily implemented technique used in this study could be of interest in improving performance of recruitment and retention campaigns.

Tools for the Measurement of Psychological Aspects of Organ Donation among the Families of Brain-dead People.

BACKGROUND: According to the basic ethical principle of non-maleficence, organ procurement systems need to be accountable to donor families. As organ donation can be potentially traumatic, donor families are at risk of developing psychological damage. Appropriate measurement tools are needed to diagnose such disorders and develop appropriate treatment measures. OBJECTIVE: To examine the appropriateness of measurement tools and approaches used in previous studies for assessing donor families' psychological well-being. METHODS: A structured online search was conducted in electronic databases n

Absolute male factor infertility. A useful model for evaluating the efficacy of cryopreserved semen.

Cryopreserved semen generally results in diminished pregnancy rates when used for donor insemination. However, coexisting female subfecundity may significantly confound the pregnancy outcome. Therefore, in order to determine the efficacy of cryopreserved semen, 43 consecutive women with absolute male factor infertility (single women or wives of azoospermic men) were inseminated with cryopreserved spermatozoa using cervical caps. Life table analysis of the results revealed a cumulative probability of conception of 84% with a monthly fecundability rate of 19% after eight insemination cycles. The

Application of the 2017 KDIGO Guideline for the Evaluation and Care of Living Kidney Donors to Clinical Practice.

The Kidney Disease: Improving Global Outcomes (KDIGO) 2017 "Clinical Practice Guideline on the Evaluation and Care of Living Kidney Donors" was developed to assist medical professionals who evaluate living kidney donor candidates and provide care before, during, and after donation. This guideline Work Group concluded that a comprehensive approach to donor candidate risk assessment should replace eligibility decisions on the basis of assessments of single risk factors in isolation. To address all issues important to living donors in a pragmatic and comprehensive guideline, many of the guideline
