Targeting drug-tolerant persister cells to overcome resistance in HER2-positive gastric cancer.

Gastric cancer (GC) ranks as the fifth most common cancer worldwide, posing a significant global health challenge. Approximately 20 % of GC cases overexpress the human epidermal growth factor receptor 2 (HER2), a feature associated with poorer prognosis. Since the landmark Trastuzumab for Gastric Cancer (ToGA) trial, trastuzumab has become the only FDA-approved first-line therapy for HER2-positive GC. Nevertheless, nearly half of these patients exhibit either primary resistance or early relapse despite trastuzumab therapy, underscoring the urgent need for strategies beyond HER2 blockade. Drug-

Statin use in transthyretin amyloid cardiomyopathy: Assessing tolerability and drug interaction risks with tafamidis.

BACKGROUND: Transthyretin cardiac amyloidosis (ATTR-CM) is a progressive cardiomyopathy treated with tafamidis, a transthyretin stabilizer that improves survival and quality of life. Tafamidis inhibits the breast cancer resistance protein (BCRP), raising concern for drug-drug interactions, particularly with rosuvastatin. OBJECTIVE: To evaluate the potential interaction between tafamidis and statins in patients with ATTR-CM. METHODS: We conducted a retrospective study of 103 patients with confirmed ATTR-CM receiving both tafamidis and statin therapy at Columbia University Irving Medical Cente

Mesenchymal stem cells offer a drug-tolerant and immune-privileged niche to Mycobacterium tuberculosis.

Anti-tuberculosis (TB) drugs, while being highly potent in vitro, require prolonged treatment to control Mycobacterium tuberculosis (Mtb) infections in vivo. We report here that mesenchymal stem cells (MSCs) shelter Mtb to help tolerate anti-TB drugs. MSCs readily take up Mtb and allow unabated mycobacterial growth despite having a functional innate pathway of phagosome maturation. Unlike macrophage-resident ones, MSC-resident Mtb tolerates anti-TB drugs remarkably well, a phenomenon requiring proteins ABCC1, ABCG2 and vacuolar-type H+ATPases. Additionally, the classic pro-inflammatory cytokin

Borrelia burgdorferi, the Causative Agent of Lyme Disease, Forms Drug-Tolerant Persister Cells.

Borrelia burgdorferi is the causative agent of Lyme disease, which affects an estimated 300,000 people annually in the United States. When treated early, the disease usually resolves, but when left untreated, it can result in symptoms such as arthritis and encephalopathy. Treatment of the late-stage disease may require multiple courses of antibiotic therapy. Given that antibiotic resistance has not been observed for B. burgdorferi, the reason for the recalcitrance of late-stage disease to antibiotics is unclear. In other chronic infections, the presence of drug-tolerant persisters has been lin

Practical guide to the management of acute and chronic pain in the presence of drug tolerance for the healthcare practitioner.

BACKGROUND: Drug tolerance has been on the rise in recent years worldwide, and consequently, pain management in our population has become challenging. METHODS: Discussed in this review are commonly abused drugs and considerations for treating acute and chronic pain states in patients with substance disorders. RESULTS: After marijuana, alcohol, and tobacco, the most widely abused substances are oxycodone (Oxycontin), diazepam (Valium), and methylphenidate (Ritalin). Urine testing can detect metabolites of drugs used by patients and is useful for assessing drug abuse, medication diversion, and

Valproate is an effective, well-tolerated drug for treatment of status epilepticus/serial attacks in adults.

OBJECTIVE: Status epilepticus (SE) and serial attacks (SA) represent neurological emergencies, and mortality rate for SE/SA is high, ranging from 3% to 25%, depending on cause and co-morbidity. As SE/SA become more refractory to treatment over time, rapid, appropriate treatment is extremely important. Here, we report a prospective registration of the effect of intravenous (IV) valproate (VPA) on SE/SA in a group of Norwegian patients. PATIENTS AND METHODS: Forty-one adult patients (18 males, 23 females) were included in the study. All had previously been unsuccessfully treated with diazepam.

The influence of immunosuppressive drugs on tolerance induction through bone marrow transplantation with costimulation blockade.

We recently developed a murine protocol for the induction of allogeneic mixed chimerism and tolerance employing nonmyeloablative total body irradiation (TBI), standard-dose bone marrow transplantation (BMT), and costimulation blockade (cobl) with an anti-CD154 monoclonal antibody (mAb) plus CTLA4Ig. We now evaluated whether a short course (1 month) of immunosuppressive drugs, which would be ethically required in the clinical setting of organ transplantation to prevent graft loss in case tolerance is not achieved, interferes with tolerance induced with this regimen. Our results show that calcin

Diflunisal: a review of pharmacokinetic and pharmacodynamic properties, drug interactions, and special tolerability studies in humans.

1 In the fasting state, peak plasma levels of diflunisal were achieved within 2 hours. The drug was not metabolized and almost totally excreted in the urine as unchanged or conjugated drug. The terminal plasma half-life was approximately 8 hours. These results support a twice daily dose regimen. 2 During multiple dose administration the time required to achieve steady-state plasma levels varied with the dose. A dose regimen of 125 mg twice daily required 2-3 d, whereas a regimen of 500 mg twice daily required 7-9 d to reach a steady-state plasma level. 3 Clinically effective doses of diflunisa

The Mycobacterial Efflux Pump EfpA Can Induce High Drug Tolerance to Many Antituberculosis Drugs, Including Moxifloxacin, in Mycobacterium smegmatis.

Active efflux of drugs across the membrane is a major survival strategy of bacteria against many drugs. In this work, we characterize an efflux pump, EfpA, from the major facilitator superfamily, that is highly conserved among both slow-growing and fast-growing Mycobacterium species and has been found to be upregulated in many clinical isolates of Mycobacterium tuberculosis. The gene encoding EfpA from Mycobacterium smegmatis was overexpressed under the control of both a constitutive and an inducible promoter. The expression of the efpA gene under the control of both promoters resulted in >32-

Nucleoid-associated protein HU constrains transcription elongation to influence cellular functions, virulence, and drug tolerance in Mycobacterium tuberculosis.

The nucleoid-associated proteins (NAPs) are the primary organizers of chromosome architecture, function, and regulators of gene expression in bacteria. Mycobacterium tuberculosis (Mtb) encodes a limited set of NAPs, among which HU (MtHU) is essential for the organism. While MtHU's DNA-binding properties are well documented, the genome-wide regulatory circuit by which it coordinates transcription remains to be elucidated. Here, we combine CRISPR interference (CRISPRi), chemical inhibition, ChIP-seq, RNA-seq, and RNA polymerase (RNAP) occupancy experiments to dissect MtHU function. Conditional d

Linking lipid profile alterations to antibiotic tolerance and natural product synergy in drug-resistant Mycobacterium tuberculosis clinical isolates.

Despite global control efforts, tuberculosis remains the leading infectious cause of death, with rising incidence, pediatric cases, and drug-resistant strains posing major public health challenges. Mycobacteria, including Mycobacterium tuberculosis, possess a lipid-rich, dual-membrane cell envelope that contributes to their impermeability, drug resistance, and unique pathogenic mechanisms. Some lipids play key roles in modulating host immune responses, enabling survival within macrophages, and promoting granuloma formation. Since it is known that lipid remodeling of the cell envelope is correl

Impaired glucose tolerance in first-episode drug-naïve patients with schizophrenia: relationships with clinical phenotypes and cognitive deficits.

BACKGROUND: Schizophrenia patients have a higher prevalence of type 2 diabetes mellitus with impaired glucose tolerance (IGT) than normals. We examined the relationship between IGT and clinical phenotypes or cognitive deficits in first-episode, drug-naïve (FEDN) Han Chinese patients with schizophrenia. METHOD: A total of 175 in-patients were compared with 31 healthy controls on anthropometric measures and fasting plasma levels of glucose, insulin and lipids. They were also compared using a 75 g oral glucose tolerance test and the homeostasis model assessment of insulin resistance (HOMA-IR). N

Tolerance of nonsteroidal anti-inflammatory drug-sensitive patients to the highly specific cyclooxygenase 2 inhibitors rofecoxib and valdecoxib.

BACKGROUND: Selective inhibitors of cyclooxygenase 2 (COX-2) are generally tolerated by patients sensitive to nonsteroidal anti-inflammatory drugs (NSAIDs) that inhibit COX-1. Valdecoxib is a new sulfonamide-containing COX-2-specific inhibitor indicated for the treatment of acute pain, osteoarthritis, and rheumatoid arthritis. OBJECTIVE: To compare the clinical tolerance to rofecoxib and valdecoxib in patients who previously developed urticaria and angioedema while taking classic NSAIDs. METHODS: Patients with challenge-proven NSAID cutaneous sensitivity were submitted to single-blinded cont

Glucose tolerance and serum insulin levels in an animal model of obesity induced by the antipsychotic drug, sulpiride.

To assess the role of insulin in the development of obesity induced by antipsychotic drugs, a glucose tolerance test was conducted in 40 female rats during the peak of sulpiride-induced weight gain and in 40 vehicle-treated animals. The glucose area under the curve did not differ between the groups (P = 0.24), however, the area under the insulin curve was significantly decreased by sulpiride (55.2 +/- 2.8 versus 115.6 +/- 18.9, P = 0.007). The results suggest that insulin resistance and hyperinsulinaemia are not involved in the excessive weight gain observed in this animal model of drug-induce

A prospective randomized study of adjuvant parenteral nutrition in the treatment of diffuse lymphoma: effect on drug tolerance.

Total parenteral nutrition (TPN) has been suggested as a useful addition to chemotherapy of malignant disease in the hope of decreasing drug toxicity and increasing drug tolerance. In this study, 17 of 36 patients given chemotherapy for advanced diffuse lymphoma were randomly selected to receive TPN. The dose of the chemotherapeutic agents to be administered was decreased according to predetermined toxicity guidelines. A comparison of drug dosage in the group receiving TPN and the group receiving standard nutrition is a measure of drug tolerance in these patients. Drug dosage was evaluable in

Genomic and Phenotypic Characterization of a Drug-Susceptible Acinetobacter baumannii Reveals Increased Virulence-Linked Traits and Stress Tolerance.

Acinetobacter baumannii is an opportunistic pathogen notable for multidrug resistance and environmental persistence. We characterized a clinical isolate, HKAB-1, which exhibits pronounced virulence-associated traits despite being highly susceptible to all tested antibiotics. HKAB-1 exhibited superior growth in MH2B, serum and desiccating conditions, robust biofilm formation, and active motility. Whole-genome sequencing identified two heme utilization clusters, multiple siderophore biosynthesis pathways, and other virulence-associated genes. Gene expression analysis revealed significant upregul
