High-risk behavior and potential transmission of drug-resistant HIV among injection drug users.

Evidence of increasing prevalence of drug resistance among recent HIV seroconverters indicates a growing public health concern and warrants an examination of the problem from a prevention perspective. Among 638 HIV-infected injection drug users (IDUs) completing 2731 visits between December 1996 and February 2000 in an ongoing cohort study in Baltimore, Maryland, factors associated with unprotected sex and needle sharing were determined. Participants were classified as being at higher or lower risk of HIV and of drug-resistant HIV transmission based on viral load, antiretroviral therapy use, a

Recurrent Bioprosthetic Valve Serratia marcescens Endocarditis in Intravenous Drug Users.

BACKGROUND We report 2 cases of recurrent right-sided endocarditis in 2 young patients known to be intravenous (i.v.) drug users. We highlight the importance of early diagnosis and management, especially in recurrent infection, which has a higher mortality rate and poor prognostic outcome despite antibiotic treatment. CASE REPORT A 30-year-old woman with a medical history of active i.v. drug use and tricuspid valve replacement owing to Serratia marcescens endocarditis 2 months prior to presentation was admitted to the Intensive Care Unit for septic shock. The patient did not respond to i.v. fl

Compulsory treatment of drug users in Asia: designed to torture?

PURPOSE: - Injecting drug use is a global concern, with an estimated 16 million people who inject drugs (PWIDs) in over 148 countries. A number of Asian countries detain PWIDs for compulsory treatment. The paper aims to discuss this issue. DESIGN/METHODOLOGY/APPROACH: - The authors reviewed the literature on compulsory drug treatment in seven Asian countries. FINDINGS: - The authors identified 1,269 closed settings which held over 600,000 drug users in eight countries. The average detainee was aged from 20 to 30 years and was predominantly male. HIV risk behaviour continued in detention in s

Clinical profile of injection drug users presenting to the ED.

BACKGROUND: Injection drug users (IDUs) commonly use the emergency department (ED) as their primary health care access point. OBJECTIVE: We sought to characterize infectious disease clinical presentations and laboratory results of IDUs admitted to the hospital from the ED and contrast them with those of non-IDUs. METHODS: We enrolled all admitted adult patients with infectious disease-related diagnoses at a county level 1 trauma center from June 2010 to January 2011 and used a structured chart abstraction tool to record patient characteristics and clinical outcomes. We compared clinical pres

CCR5 haplotypes influence HCV serostatus in Caucasian intravenous drug users.

BACKGROUND: Up to 90% HIV-1 positive intravenous drug users (IDUs) are co-infected with HCV. Although best recognized for its function as a major co-receptor for cell entry of HIV, CC chemokine receptor 5 (CCR5) has also been implicated in the pathogenesis of HCV infection. Here, we investigated whether CCR5 haplotypes influence HIV-1 and HCV seropositivity among 373 Caucasian IDUs from Estonia. METHODS: Of these IDUs, 56% and 44% were HIV and HCV seropositive, respectively, and 47% were coinfected. 500 blood donors seronegative for HIV and HCV were also evaluated. CCR5 haplotypes (HHA to HHG

Hepatitis C virus-specific immune responses in noninjecting drug users.

Noninjection drug use, although recognized as an emerging risk factor for acquisition of other blood-born pathogens, is still unconfirmed as a route of hepatitis C virus (HCV) transmission. Our goal was to measure HCV exposure and prevalence in noninjection drug users (NIDUs). Fifty-seven NIDUs were screened by extensive questionnaire to exclude prior injection drug use and evaluated for HCV-specific serologic and cellular immune responses. HCV-specific T-cell responses were measured using interferon-gamma (IFN-γ) enzyme-linked immunospot (ELISpot) assay with overlapping HCV peptides covering

[Psychosocial determinants of drug users to use a new syringe].

BACKGROUND: A limited number of injection drug users are hard to reach through needle-exchange programs. They obtain their injection material from drug-using peers. This dependence on others can make them more at risk of contracting HIV through sharing non-sterile syringes. The aim of this study is to identify determinants of the intention to systematically resort to use of a new syringe by injection drug users rarely or never involved in needle-exchange programs in Quebec. METHODS: A cross-sectional survey was conducted in Québec city by means of a questionnaire measuring variables from the

Preventing relapse in incarcerated drug users in Yunnan Province, China.

INTRODUCTION AND AIMS: The mainstay of drug control in China is compulsory incarceration and detoxification, but relapse rates following release are very high. The aim of the study was to explore factors which would help prevent relapse in injecting drug users following release from detoxification centres. DESIGN AND METHODS: Semi-structured interviews were carried out at three compulsory detoxification centres in Derhong prefecture, Yunnan Province, which has the highest proportion of injecting drug users in China. RESULTS: Interviews were completed with 235 men and 125 women aged between 1

Nonstructured treatment interruptions among injection drug users in Baltimore, MD.

BACKGROUND: We characterized patterns of highly active antiretroviral therapy (HAART) use and predictors of nonstructured treatment interruptions (NTIs) among injection drug users (IDUs) in Baltimore, MD. METHODS: Three hundred thirty-five IDUs who initiated HAART from 1996 to 2006 were studied. NTIs were defined as any subsequent 6-month interval where HAART was not reported. Predictors of the first NTI and subsequent restart of HAART were examined using Cox regression. RESULTS: Two hundred sixty (78%) reported > or =1 NTI. Of 215 with > or =1 follow-up visit after the NTI, 44 (20%) never r

[Prevention and treatment of hepatitis C in illicit drug users].

Drug use is a complex behavior with multidimensional determinants, including social, psychological, cultural, economic, and biological factors. Blood borne viral infections including hepatitis C virus are transmitted when an uninfected intravenous drug user (IVDU) uses injection equipment, especially syringes, that have previously been used by an infected person. The transmission can also result from sharing other injection equipment such as 'cookers' and 'cottons'. Recent studies have shown that the prevalence and incidence of drug abuse have declined substantially since the introduction of n

Alcohol and HIV sexual risk behaviors among injection drug users.

We analyzed data from 6341 injection drug users (IDUs) entering detoxification or methadone maintenance treatment in New York City between 1990 and 2004 to test the hypothesis that alcohol use and intoxication is associated with increased HIV sexual risk behaviors. Two types of associations were assessed: (1) a global association (i.e., the relationship between HIV sexual risk behaviors during the 6 months prior to the interview and at-risk drinking in that period, defined as more than 14 drinks per week for males or 7 drinks per week for females), and (2) an event-specific association (i.e.,

Incarceration experiences in a cohort of active injection drug users.

BACKGROUND: Incarceration has been associated with a number of health-related harms among injection drug users (IDU). However, little is known about the prevalence and correlates of incarceration among community-based samples of IDU. METHODS: We examined the prevalence and correlates of recent incarceration among IDU in the Scientific Evaluation of Supervised Injecting (SEOSI) cohort examined between 1 July 2004 and 30 June 2006 using generalised estimating equations (GEE). RESULTS: A total of 902 individuals were included in the analysis, of whom 255 (28.72%) were female and 536 (59.42%) re

Identifying heterogeneity among injection drug users: a cluster analysis approach.

OBJECTIVES: We used cluster analysis to subdivide a population of injection drug users and identify previously unknown behavioral heterogeneity within that population. METHODS: We applied cluster analysis techniques to data collected in a cross-sectional survey of injection drug users in Winnipeg, Manitoba. The clustering variables we used were based on receptive syringe sharing, ethnicity, and types of drugs injected. RESULTS: Seven clusters were identified for both male and female injection drug users. Some relationships previously revealed in our study setting, such as the known relations

Health status of illicit stimulant drug users in rural Ohio.

The SF-8 health survey was used to assess the physical and mental health status of a community sample of not-in-treatment, illicit stimulant drug-using adults (n = 249) living in rural Ohio. Physical health status scores indicative of poor health were present in 30.5% of the sample. Poor physical health was associated with older age (OR = 1.06; 95% C.I. = 1.02-1.11), chronic disease (OR = 2.24, 95% C.I. = 1.14-4.40), and frequent opioid use (OR = 3.14, 95% C.I. = 1.16-8.50). Poor mental health status scores were present in 63.9% of the sample. Men were less likely (OR = 0.25, 95% C.I. = 0.11-0

Residential transience and HIV risk behaviors among injection drug users.

Housing instability has been linked to HIV risk behaviors. Many studies have focused on the implications of one's housing structure or lack thereof. This study focuses on residential transience as an additional dimension of housing instability. Specifically, we assessed the associations between transience and four HIV risk behaviors. Transience was defined as moving twice or more in the past six months. Multivariate analyses of a sample of current injectors (n = 807) indicated that transience had an independent effect on HIV risk behaviors. Transient individuals were more likely to share needl
