How are we doing with the treatment of essential tremor (ET)?: Persistence of patients with ET on medication: data from 528 patients in three settings.

BACKGROUND: The pharmacological treatment of essential tremor (ET) is not optimal. There are only two first-line medications and troublesome side effects are common. It is not uncommon for patients to simply stop taking medication. Yet, no published data substantiate or quantify this anecdotal impression. OBJECTIVES: To determine, amongst patients with ET who were prescribed medication for tremor, what proportion are still taking medication and what proportion have stopped? METHODS: Five hundred and twenty-eight patients with ET from three distinct study settings (clinical, brain donors, pop

Trends in Medicines Discontinuation and Implications for Access in Brazil (2014-2024).

PURPOSE: The World Health Organization (WHO) began recognizing drug shortages as a global issue in 2014, impacting health systems and patient care. Shortages arise from supply chain disruptions, production issues, and mismatches between supply and demand, aggravated by insufficient mechanisms for forecasting and communication. METHODS: The study analyzed medicine discontinuation notifications in Brazil from 2014 to 2024 from a public secondary database. Data was downloaded, organized, and cleaned using Excel, incorporating ATC classification and verification of medicines listed on the Nationa

Estimated Financing Amount Needed for Essential Medicines in China, 2014.

BACKGROUND: At the present time, the government is considering to establish the independent financing system for essential medicines (EMs). However, it is still in the exploration phase. The objectives of this study were to calculate and estimate financing amount of EMs in China in 2014 and to provide data evidence for establishing financing mechanism of EMs. METHODS: Two approaches were adopted in this study. First, we used a retrospective research to estimate the cost of EMs in China in 2014. We identified all the 520 drugs listed in the latest national EMs list (2012) and calculated the to

Medicine affordability and access in India: lessons from generic-branded price variation under the Jan Aushadhi Scheme.

BACKGROUND: High out-of-pocket expenditure (OOPE) on medicines continues to be a major driver of health-related financial hardship in India. The Jan Aushadhi Scheme (JAS), launched by the Government of India, seeks to improve affordability and access to essential medicines through generic substitution. However, systematic evidence on its pricing dynamics, affordability, and equity implications remains limited. METHODS: We conducted an observational analysis of 696 branded formulations for Eye and Ear, Nose, and Throat (ENT) conditions, comparing their prices with Jan Aushadhi generics listed

Quality evaluation of the Azithromycin tablets commonly marketed in Adama, and Modjo towns, Oromia Regional State, Ethiopia.

BACKGROUND: Azithromycin is a therapeutically" relevant macrolide antibiotic registered on the Essential Medicines List of the World Health Organization. The fact that medicine is selected as an essential drug doesn't mean that it is of good quality. Hence, a continuous quality evaluation of the drug should be mandated to verify that the right medication is available on the market. OBJECTIVE: To evaluate the quality of Azithromycin Tablets commonly marketed in Adama, and Modjo town, Oromia Regional State, Ethiopia. METHODS: All six brands were subjected to in-vitro quality control tests, whi

[Efficacy of treatment for pulmonary tuberculosis with multidrug mycobacterial resistance].

The efficiency of treatment was studied in 149 patients with pulmonary tuberculosis who isolated multidrug resistance of Mycobacteria tuberculosis (MBT). The multidrug resistance of MTB, to at least isoniazid and rifampicin can be associated with both the resistance to other essential (streptomycin, ethambutol) and that to reserve drugs. With this, patients with MBT resistance to a combination of essential and reserve drugs more frequently showed a chronic course of the disease with severe clinical manifestations and more disseminated infiltrative-and-destructive lesions in the lung. Drug trea

Dose optimisation to improve access to effective cancer medicines.

Access to many cancer medicines on WHO's Essential Medicines List (EML) is restricted because of price, especially in low-income and middle-income countries (LMICs). Other cancer medicines that have been shown to improve survival, such as immune checkpoint inhibitors for lung cancer, are not included on the EML because approved doses and schedules exceed affordable prices in LMICs. Multiple strategies are therefore needed to reduce medicine costs or circumvent these problems, such as optimising doses and schedules. Cancer medicines are approved by regulatory agencies, such as the US Food and D

Estimation of cost-based prices for injectable medicines in the WHO Essential Medicines List.

OBJECTIVES: Challenges remain in ensuring universal access to affordable essential medicines. We previously estimated the expected generic prices based on cost of production for medicines in solid oral formulations (ie, capsules or tablets) on the WHO Model List of Essential Medicines (EML). The objectives of this analysis were to estimate cost-based prices for injectable medicines on the EML and to compare these to lowest current prices in England, South Africa, and India. DESIGN: Data on the cost of active pharmaceutical ingredients (APIs) exported from India were extracted from an online d

The availability, prices and affordability of essential medicines in Malawi: A cross-sectional study.

INTRODUCTION: The Malawian government recently introduced cost-covering consultation fees for self-referral patients in tertiary public hospitals. Previously, patients received medicines free of charge in government-owned health facilities, but must pay elsewhere. Before the government implements a payment policy in other areas of health care, it is important to investigate the prices, affordability and availability of essential medicines in Malawi. METHODS: Data on availability and prices of 50 essential medicines were collected in 44 health facilities in two major cities and two districts.

Dose-escalation study of octanoic acid in patients with essential tremor.

BACKGROUND: Recently, 1-octanol has been shown to have efficacy in treating patients with essential tremor (ET). The primary metabolite of 1-octanol is octanoic acid (OA), which is now thought to be the active substance that mediates tremor suppression. Our aim was to describe the maximum tolerated dose (MTD) of oral OA in patients with ET and assess the pharmacokinetics (PK) and pharmacodynamics (PD) profile of OA. METHODS: The MTD was studied using an open-label, single-ascending 3 + 3 dose-escalation design. Predefined single doses ranged from 8 to 128 mg/kg, with grade 2 adverse events (A

WHO shapes priorities for medicines? An analysis of the applicants and decision makers within the historical evolution of the WHO Model Lists of Essential Medicines.

WHO recently announced a process to review and potentially update the procedures for selecting essential medicines. This announcement presents an opportunity to reflect on the evolution of the WHO Model Lists of Essential Medicines (EML), including the composition of the stakeholders that shape priorities. We contextualised our findings within the broader history of the WHO EML to support future reforms to improve access to essential medicines. The current system allows individuals to propose a medicine for the WHO EML. This makes the EML reactive to applicant priorities. Almost all medicines

Access to essential medicines for sexual and reproductive health care: the role of the pharmaceutical industry and international regulation.

The range of medicines and technologies that are essential for sexual and reproductive health care is well established, but access to them is far from universally assured, particularly in less developed countries. This paper shows how the pharmaceutical industry plays a major role in the lack of access to essential medicines for sexual and reproductive health care, by a) investing in products for profit-making reasons despite their negative health impact (e.g. hormone replacement therapy), b) marketing new essential medicines at prices beyond the reach of countries that most need them (e.g. HP
