Effectiveness and safety of intravenous labetalol in severe pre-eclampsia and eclampsia at a teaching institution in Chhattisgarh.

INTRODUCTION: Severe hypertension in pregnancy deserves prompt recognition and urgent effective reduction in order to reduce the risk of complications such as eclampsia and HELLP syndrome and to achieve desirable neonatal outcomes. There is a need for effective and safe parenteral antihypertensive treatment. SUBJECTS AND METHODS: We studied the effectiveness and safety of intravenous labetalol use in severe hypertension in pregnancy and post-partum period in a teaching hospital in Chhattisgarh in 101 women. IV labetalol was given as bolus doses till the blood pressures were controlled. Neonat

Factors affecting use of magnesium sulphate for pre-eclampsia or eclampsia: a qualitative evidence synthesis.

BACKGROUND: Hypertensive disorders account for 14% of global maternal deaths. Magnesium sulphate (MgSO4 ) is recommended for prevention and treatment of pre-eclampsia/eclampsia. However, MgSO4 remains underused, particularly in low- and middle-income countries (LMICs). OBJECTIVE: This qualitative evidence synthesis explores perceptions and experiences of healthcare providers, administrators and policy-makers regarding factors affecting use of MgSO4 to prevent or treat pre-eclampsia/eclampsia. SEARCH STRATEGY: We searched MEDLINE, EMBASE, Emcare, CINAHL, Global Health and Global Index Medicus

PROVE-Pre-Eclampsia Obstetric Adverse Events: Establishment of a Biobank and Database for Pre-Eclampsia.

Pre-eclampsia is a leading cause of maternal and perinatal morbidity and mortality. The burden of disease lies mainly in low-middle income countries. The aim of this project is to establish a pre-eclampsia biobank in South Africa to facilitate research in the field of pre-eclampsia with a focus on phenotyping severe disease. The approach of our biobank is to collect biological specimens, detailed clinical data, tests, and biophysical examinations, including magnetic resonance imaging (MRI) of the brain, MRI of the heart, transcranial Doppler, echocardiography, and cognitive function tests.Women

Methodological and technical issues related to the diagnosis, screening, prevention, and treatment of pre-eclampsia and eclampsia.

In contrast with advances made in treating or eliminating many other serious disorders, severe morbidity and mortality associated with pre-eclampsia/eclampsia remain among the leading problems that threaten safe motherhood, particularly in developing countries. This article reviews technical issues related to diagnosis, screening, prevention, and treatment of pre-eclampsia and identifies corresponding needs. The authors stress the lack of standardized definitions of pre-eclampsia and eclampsia and discuss problems in blood-pressure measurements and assessment of urinary protein. They summarize

Eclampsia despite magnesium sulfate prophylaxis.

BACKGROUND: Magnesium sulfate is highly effective for the prophylactic treatment of preeclampsia with severe features, as well as the prevention of recurrent eclamptic seizures. The consequences of developing eclampsia despite treatment with magnesium sulfate prophylaxis have received little attention. OBJECTIVE: Our aim was to describe maternal and perinatal consequences in patients who develop eclampsia despite treatment with magnesium sulfate therapy. STUDY DESIGN: This was a retrospective study of all patients with eclampsia over 13 years at a single institution. Eclampsia was diagnosed

Antiretrovirals causing severe pre-eclampsia.

BACKGROUND: Extremely low birth weight infants (ELBW) (⩽1000g) at our tertiary hospital have a much higher HIV exposure rate than bigger babies and are mainly delivered due to complications of pre-eclampsia. Studies investigating the effect of HIV or antiretroviral therapy on pre-eclampsia, a partially immune-mediated disease have produced contradictory results. OBJECTIVE: To determine if there was an association between pre-eclampsia and HIV and/or antiretrovirals in the mothers of ELBW infants. STUDY DESIGN: A prospective database was maintained for all ELBW infants born from August 2011 t

Fluid management in pre-eclampsia.

Intravenous fluid given to women with pre-eclampsia may be a necessary form of treatment; however, intravenous fluid therapy can also cause iatrogenic pulmonary oedema. The indications for the use of intravenous fluids, the titration of the amount of fluid given and the use of invasive monitoring have not been subject to adequate examination in randomised studies. Clinical experience, combined with available evidence and a reasoned approach are the basis for a suggested management algorithm.

[Pre-eclampsia and facial paralysis].

We describe here the case of a patient affected with pre-eclampsia and who had idiopathic facial palsy and HELLP syndrome during the puerperium. It developed favourably with a treatment associated with an antihypertensive medicine, corticoid therapy and ophtalmological care. There is an association between pre-eclampsia and idiopathic facial palsy during pregnancy. This can be explained by identical physiopathological mechanisms. Facial palsy generally appears in the course of the third trimester of pregnancy or during the early puerperium. The prognosis is usually good and does not seem to be

Neurologic complications of pre-eclampsia.

Pre-eclampsia is mainly responsible for the world's large maternal mortality rates, mostly due to acute cerebral complications. This review provides insight into the pathogenesis of the neurologic complications of hypertensive disease in pregnancy. In addition, practical relevance for clinical care is highlighted. Pertaining to pregnancy, the blood pressure level at which cerebral autoregulation operates and possible deregulation occurs is unknown, but is likely to be variable. From clinical observation, eclampsia may occur despite a mild clinical picture and before the development of hyperten

Serum markers in pre-eclampsia.

Preeclampsia occurs approximately in 10% of pregnancies and remains a leading cause of maternal and neonatal mortality and morbidity worldwide, particularly in developing countries. The condition is usually diagnosed in late pregnancy by the presence of hypertension with proteinuria and/ or edema. Prevention of any disease process requires knowledge of its etiology and pathogenesis, as well as the availability of methods for prediction of those at high risk for this disorder. Numerous clinical, biophysical, and biochemical tests have been proposed for prediction or early detection of preeclamp

Prolongation of pregnancy following eclampsia.

Eclampsia is a serious complication of preeclampsia with high maternal and perinatal mortality and morbidity rates. The recommended management is induction of labour when convulsions are controlled and the women's condition is stabilised. Here, we report about stabilisation after eclampsia followed by prolongation of a preterm pregnancy for 11 days within early gestational weeks.

Calcium supplementation in pre-eclampsia.

OBJECTIVE: The objective of this prospective, open, randomised trial was to determine the role of calcium supplementation in preventing pre-eclampsia. METHODS: One hundred uncomplicated normotensive primigravidae were enrolled in the study before 20 weeks of pregnancy. Fifty each were randomised to receive either two gram elemental calcium daily from 20 weeks of gestation to delivery (study group) or no calcium supplementation (control group). Prior to 20 weeks of gestation each underwent a complete clinical and laboratory evaluation. Serum and urine calcium was measured first at 20 weeks of

Fluid management in pre-eclampsia.

We review the evidence base for fluid management in pre-eclampsia. Current understanding of the relevant pathophysiology and the possible impact of styles of fluid management on maternal and fetal outcome are presented. There is little evidence upon which to base the management of fluid balance in pre-eclampsia. Reports are conflicting and no large prospective outcome studies of fluid management have been performed. Volume expansion does not appear to reduce the incidence of fetal distress. Pulmonary oedema and oliguria receive particular attention. There is no evidence of long-term renal dama

Medical prevention of pre-eclampsia.

There is no clear evidence that any of the antihypertensive drugs available can defer or prevent the occurrence of proteinuric pre-eclampsia or associated problems such as fetal growth retardation or perinatal death. When antihypertensive treatment is indicated, there seems to be no reason to prefer any of the tested beta-blockers, or to prefer labetalol to a pure beta-blocker, or indeed, to prefer beta-blockers to methyldopa. The increased maternal, fetal and infant mortality and morbidity associated with hypertension in pregnancy justify careful evaluation of the risks of the more severe for
