"Ratchet-type" marginal fracture of the radial head with locked anterior dislocation. A case report.

Locked anterior dislocation of the superior radioulnar joint occurred in a 26-year-old man. The configuration of the associated radial head fracture maintained the locked position and predisposed to recurrence after open reduction. Radial head excision was necessary to resolve the problem. Both medial ligament repair and a radial head prosthesis were necessary to stabilize the elbow.

Souter-Strathclyde total elbow arthroplasty.

We have reviewed 66 consecutive Souter-Strathclyde arthroplasties of the elbow implanted in 59 patients between 1982 and 1993. Thirteen patients (15 elbows) (19.6%) died. Sixteen elbows (24.2%) were revised, six for aseptic loosening (9%), four (6%) because of fracture or loosening after a fracture, three (4.5%) for infection and three (4.5%) for dislocation. Four patients refused to attend for review. In 33 elbows with a follow-up of 93 months (60 to 167) complete relief of pain was achieved in 22 (67%) when seen at one year. After ten years or more 36% of the elbows were painfree and 64% had

Capitellocondylar total elbow replacement in late-stage rheumatoid arthritis.

Between 1994 and 2000, 51 capitellocondylar elbow replacements were inserted in 41 patients. All patients had late-stage rheumatoid arthritis. The mean age at operation was 56 years (range, 25-78 years). There were 12 men and 29 women. At follow-up, 6 patients had died of unrelated causes with the implant in situ and without radiographic loosening, and 1 patient was lost to follow-up. The remaining 43 elbows in 34 patients were followed up clinically and radiographically at a mean of 6.9 years (range, 26-119 months). Relief of pain was complete in 91% of the surviving elbows, and in 9%, there

Total elbow arthroplasty as primary treatment for distal humeral fractures in elderly patients.

Distal humeral fractures are difficult to treat. In the elderly population, the problems are compounded by osteoporosis and gross comminution. Open reduction and internal fixation for such fractures is sometimes difficult and may be associated with poor results. Total elbow arthroplasty has been suggested as a last-ditch effort to salvage functional use for such difficult fractures in the elderly. We followed seven patients (seven elbows) with a mean age of 81.7 years at the time of injury. Open reduction and internal fixation was considered a difficult option for these fractures. They were tr

Upper Limb Prosthesis Control: A Hybrid EEG-EMG Scheme for Motion Estimation in Transhumeral Subjects.

This study described the use of Kernel Least Square Tracker based estimation for 3-dimensional shoulder, elbow motion kinematics from surface Electromyogram (EMG) and a two-stage multiclass Support Vector Machine based classification of different wrist, grip and finger motions from Electroencephalogram (EEG). The advantage of employing hybrid EEG-EMG strategy for upper limb motion estimation was demonstrated for a transhumeral subject. The method utilized EMG from upper arm muscles for elbow motion (and shoulder motion in case of higher degree amputation scenario) and used EEG for discerning b

Total elbow arthroplasty after interposition arthroplasty for elbow arthritis.

BACKGROUND: Interposition arthroplasty is an option for the treatment of arthritis of the elbow. Conversion to a total elbow arthroplasty can be considered later, when the patient reaches a suitable age. We investigated the results of conversion of an interpositional elbow arthroplasty to a semiconstrained total elbow arthroplasty in a series of patients. METHODS: The results of twelve consecutive linked semiconstrained total elbow arthroplasties in twelve patients who had had a prior interposition arthroplasty for the treatment of degenerative arthritis of the elbow were evaluated at an aver

Present state-of-the-art in elbow arthroplasty.

Prosthetic joint replacement of the elbow is, with some delay in comparison with the shoulder, the finger joints and especially the hip and knee joint, becoming a routine operation at least in more specialised orthopedic and trauma centers. In the seventies and eighties, more than 80% of the indications were in patients affected by rheumatoid arthritis, in which both sides were typically affected, seriously jeopardising their independence in activities of daily living. In the last decade an increasing number of posttraumatic osteoarthritic cases were included in the indications. Among the nume

Treatment of difficult fractures and nonunions of the humerus and elbow with a modified Küntscher nail.

A series of fourteen difficult fractures and nonunions of the humerus and elbow have been treated over a period of twenty-eight years with a modified Kuntscher Nail. A total of twenty-six operative attempts had been previously made in this group of fourteen patients. One patient had eight failed surgeries prior to treatment. Slots were placed along the spine of the nail for transfixion with screws. In two instances additional modification of the Kuntscher nail was made by attaching a plate to the end of the nail for fixation to the ulna after retrograde insertion into the humerus. One such dev

Complications after radial head arthroplasty: a comparison between short-stemmed bipolar and monopolar long-stemmed osteointegrative rigidly fixed prostheses.

INTRODUCTION: To date, only a few studies have compared radial head prostheses (RHP) and their different anchoring principles. The aim of this study was to characterize concomitant injuries, necessary adjuvant procedures, complications, and radiological findings after implantation of two different types of RHP. METHODS: Sixty-six patients with radial head fractures were treated with MoPyC (Tornier/France, 50 mm stem, monopolar) or SBI rHead prostheses (Small Bone Innovations/USA, 22-mm stem, bipolar) and followed up over 42 months (16-64 months). Primary objective was the detection of differe

Radial head arthroplasty: state of the art.

Radial head arthroplasty has joined the armamentarium of options for the treatment of complex radial head fractures, elbow instability, and arthritic conditions. A variety of implants has been introduced in the past decade; these differ in metallic composition, design, and method of fixation. Good short- and intermediate-term outcomes have been reported with the use of loose-fitting prostheses. Press-fit devices restore stability and improve pain and motion but are associated with a greater likelihood of implant loosening, leading to revision surgery. Postoperative elbow stiffness, pain, ulnar

Biomechanical evaluation of the effect of varying humeral implant length in total elbow replacement: a finite element analysis.

BACKGROUND: Total elbow arthroplasty (TEA) is increasingly used for expanding indications but complications like aseptic loosening and periprosthetic fractures persist. Our objective is to examine the biomechanical behavior of the total elbow implant in response to varying implant lengths by investigating the stresses and the stress shielding effect in the bone-implant assembly using finite element (FE) modeling. METHODS: A fourth-generation synthetic humerus sawbone and its corresponding digital model were used in this study. Total elbow implants are laser scanned to obtain the 3D implant mo

Pyrocarbon radial head arthroplasty offers satisfactory clinical and radiological outcomes with low revision rate: A systematic review.

INTRODUCTION: Pyrocarbon promises to be an optimal material choice for radial head arthroplasty (RHA) due to an elastic modulus comparable to the radial diaphysis and thus providing higher biocompatibility. Primary objective was to determine the complications and revision rates related to the usage of these prostheses. The secondary objective was to assess the clinical and radiological outcomes of Pyrocarbon RHAs. HYPOTHESIS: Pyrocarbon RHAs have good clinical and radiological outcomes with low complications and revisions. METHODS: Ovid MEDLINE and Embase databases were used to search for st

[Treatment of complex elbow injuries with the hinged fixation device DJD II. Technique and results].

Fractures and luxations with persistent instability of the elbow are severe injuries often followed by arthrofibrosis. The need for prolonged immobilization results in shortage of the ligaments and shrinking of the capsule. Treatment with dynamic hinged external fixation offers an option of protected early mobilization providing better results and avoiding stiffness and instability. In a retrospective study 22 patients with complex injuries of the elbow were evaluated with respect to the range of motion and subjective parameters (DASH score, DASH disabilities of the arm, shoulder and hand).

French innovations in upper limb surgery that has become gold standards.

BACKGROUND: France has significantly contributed to the development of upper limb surgery over the past century. Although several French techniques and implants are now used worldwide, their historical origin, their technical evolution and the level of evidence supporting their status as "gold standards" have rarely been gathered. This narrative review aims to analyze from a short historical point of view with long-term published evidence of French innovations in shoulder, elbow and hand surgery that have achieved international reference status. METHODS: This narrative review will cover four

[Unreconstructible radial head fracture: resection, implant of Swanson or prosthesis? Retrospective comparative study].

Treatment of unreconstructible comminuted fractures of the radial head remains a therapeutic challenge. Thirty two patients sustained unreconstructible radial head fracture between 1969 and 1999 and have been treated by resection (16 patients), by Swanson implant (8 patients), or by Judet prosthesis (8 patients). The three groups of patients were reviewed clinically and radiologically by two surgeons not involved in their treatment. Functional outcomes of the elbow (Morrey scoring-SOO scoring system, instability, cubitus valgus) and involvement of the wrist (pain, grasp, RUD instability) were

Combined free flap, Ilizarov lengthening and prosthetic fitting in the reconstruction of a proximal forearm amputation--a case report.

A composite free latissimus dorsi musculocutaneous flap combined with Ilizarov lengthening was used in the resurfacing of the elbow and the reconstruction of the forearm following a very proximal below elbow amputation with soft tissue degloving to the level of the mid arm. The free flap provided gliding soft tissue to the elbow joint, and the deep fascia and muscle components of the flap were contoured round the bony stump so as to allow bone transport within the soft tissue envelope. The lengthening achieved was 14 cm with no bone resorption at the end of the stump. The reconstructed forearm

Implant sonication for the diagnosis of prosthetic elbow infection.

BACKGROUND: Periprosthetic infection is a potentially devastating complication of elbow arthroplasty, associated with formation of microbial biofilm on the implant surface. The definitive microbiologic diagnosis of periprosthetic infection after elbow arthroplasty may be difficult to establish. Our study aim was to compare the diagnostic accuracy of conventional periprosthetic tissue culture and culture of fluid derived from vortexing and bath sonication of the explanted hardware (a biofilm-sampling strategy).
