Electroconvulsive therapy in neuroacanthocytosis or McLeod syndrome.

We present the case of a 24-year-old man diagnosed with a neuroacanthocytosis, obsessive-compulsive disorder, mild-moderate severity depressive disorder, and obsessive personality disorder. He received multiple psychotropic drugs, including neuroleptics, antidepressants, and benzodiazepines with small therapeutic response.Severe and progressive clinical impairment appeared including cognitive deterioration, chorea, tics, lingual dystonic protrusion, Parkinsonism, automutilations, and other behavioral disorders consistent with neuroacanthosytosis. Pharmacological trials were not efficacious, an

Safety of electroconvulsive therapy in patients with asthma.

BACKGROUND: Patients with depression and other psychiatric disorders being considered for electroconvulsive therapy (ECT) may also have asthma. Since ECT requires the administration of general anaesthesia, it is assumed that extra care should be taken with asthmatic patients before and during ECT. We sought to investigate the safety of ECT in asthmatic patients. METHODS: A retrospective review was conducted of the medical records of all of the patients with currently active and managed asthma who underwent ECT for severe depressive syndromes at Mayo Clinic, Rochester, Minnesota, between 1 Jan

Indications for the use of propofol in electroconvulsive therapy.

BACKGROUND: Propofol is an anesthetic agent commonly used for ambulatory surgery because is associated with rapid recovery and a benign side effect profile. In electroconvulsive therapy (ECT), its use is limited because of its seizure-shortening effects, and is recommended only for patients with preexisting cardiac conditions requiring attenuated hemodynamic response during treatment. We have identified two additional significant indications for propofol: to induce shorter seizures in patients with prolonged seizures and to minimize post-ictal nausea and vomiting. METHODS: We reviewed the rec

Electroconvulsive therapy in the era of modern psychopharmacology.

Fifty years of advances in the pharmacotherapy of major depression (MDE), mania, schizophrenia, and other severe and persistent psychiatric disorders have neither made ECT obsolete nor unnecessary. However, advances in pharmacotherapy have radically changed the practice of ECT. ECT is rarely a first-line treatment of mental disorder, unless the clinical situation is desperate. Otherwise, ECT is most often offered to persons who have failed to respond to pharmacotherapy, thus defining a relatively treatment-refractory population for ECT. The physician who refers patients for ECT, as well as the

Effect of electroconvulsive therapy on hematological parameters.

Although a complete blood count is part of the evaluation before the use of electroconvulsive therapy (ECT), there are no known hematological contraindications for the procedure. A preliminary study was done on 31 randomly selected psychiatric patients (chronic schizophrenia, n=10; acute depression, n=8; acute mania, n=6; acute psychosis, n=6; delusional disorder, n=1) receiving ECT to study its hematological effects. Blood samples were drawn just before and 0, 1 and 2 h after ECT. Hemoglobin (Hb%), total and differential leukocyte count (TLC and DLC), red blood cell (RBC) count, mean corpuscu

[Mechanism of action of electroconvulsive therapy in depression].

The knowledge of the therapeutic efficacy of the electroconvulsive therapy (ECT) dates back to the fourties. Since then important investigation efforts have been carried out, to analyze the mechanism of action by which the ECT achieves its therapeutic effect. The present work reviews the different hypothesis that have been related to the mechanism of action or the ECT, as well as the large neurophysiological findings, believed to be involved in its therapeutic effect.

Electroconvulsive therapy in a patient with a cerebellar meningioma.

We report a 66-year-old woman with a cerebellar meningioma who successfully received electroconvulsive therapy (ECT) for mania. No special modifications in technique were required. A growing literature supports the relative safety of ECT in patients with small, stable brain tumors.

Combined valproate or carbamazepine and electroconvulsive therapy.

This pharmacoepidemiological study was undertaken to determine if the combination of electroconvulsive therapy (ECT) and the anticonvulsants valproate (VPA) or carbamazepine (CBZ) is safe and efficacious. The charts of seven patients receiving ECT and VPA or CBZ (ECT-anticonvulsant group) concurrently between May 8, 1989, and May 9, 1993, were reviewed to determine the indication for each treatment, the number and type of ECT treatments, the seizure duration, adverse events, and the efficacy of the combination. The ECT-anticonvulsant group was compared to patients not treated with anticonvulsa

Combined chlorpromazine and electroconvulsive therapy in mania.

We report the efficacy of combined chlorpromazine and electroconvulsive therapy (ECT) in the treatment of mania. Two groups of 15 manic patients received eight ECT sessions either actual or simulated, in a double-blind, controlled study. All patients also received 600 mg of chlorpromazine daily until the sixth session. Results indicate that the group receiving the combination of chlorpromazine and ECT did significantly better than the other group.

Treatment delays in the course of electroconvulsive therapy.

Electroconvulsive therapy (ECT) is often delayed because the patient develops cognitive disturbances. The authors reviewed the charts of 45 depressed patients who received ECT and found that 25 patients developed cognitive dysfunctions severe enough to cause a delay in treatment. The development of organic symptoms causing delays in treatment was positively correlated with increased age and the presence of preexisting cognitive dysfunction, and the treatment delays led to longer periods of hospitalization. The authors emphasize the need for early identification of the causes of cognitive dysfu

[Nursing care to patient on electroconvulsive therapy].

Eletric convulsive shock therapy has become a controversial topic in psychiatric services. Patients continue to have electroconvulsive therapy. The team nursing has seen the treatment as just another task. In this paper the authors present how the nurse can help the patient who is submitted to electroconvulsive therapy.

Present practice of electroconvulsive therapy in Scandinavia.

We have set up guidelines for the practice of electroconvulsive therapy, based on Scandinavian clinical experience and research. Because the therapeutic effect is a result of the cerebral seizure, and the organic side effects partly consequences of the electrical stimulation, the aim should be to induce maximal seizure activity using minimal electrical energy. Essential features of optimal therapy are (1) absence of therapy with benzodiazepines and other anticonvulsant drugs, (2) superficial narcosis, (3) abundant oxygen supply, (4) threshold stimulation with brief-pulse stimuli, and (5) unila

Anaesthesia for electroconvulsive therapy: a psychiatric viewpoint.

Brief general anaesthesia has added to the safety and comfort of electroconvulsive therapy (ECT). While usually a simple procedure, there are several areas in which ECT differs from other procedures requiring brief anaesthesia. These differences call for even more collaboration between psychiatrist and anaesthetist than is customary. This review details the areas where the anaesthetic management may affect the outcome of ECT for both an individual treatment and a treatment series. These include the patient evaluation, the patient preparation, the sequence and doses of anaesthetic agents, and t

The efficacy of electroconvulsive therapy and antidepressants in depression.

Electroconvulsive therapy (ECT), antidepressants, and neither treatment were compared by reviewing 609 hospitalizations for depression from 1959 to 1969. The groups receiving ECT had a significantly (p less than 0.001) greater percentage of patient who had marked improvement or a complete response (49%) than either adequate or inadequate antidepressant therapy groups (27%) or the group which received neither ECT nor antidepressants (25%). If antidepressant failures who require ECT are included in the evaluation, the percentage total improvement with ECT (90%) is significantly (p less than 0.00
