Single fiber electromyography in chronic progressive external ophthalmoplegia.

We have reviewed the electromyographic (EMG) studies of 17 patients with chronic progressive external ophthalmoplegia (CPEO). In 13 of 17 patients, conventional concentric needle EMG demonstrated a "myopathic" pattern, usually predominating in the shoulder muscles. Single-fiber EMG showed increased jitter and/or blocking in at least one muscle in 13 of 16 patients. Jitter was increased in the frontalis muscle in 10 of 13 patients and in an arm muscle in 5 of 12. When both muscles were tested, jitter was greater in the frontalis muscle in 5 patients and in the arm muscle in 2. These observation

The potential and limitations of quantitative electromyography in equine medicine.

This review discusses the scope of using (quantitative) electromyography (EMG) in diagnosing myopathies and neuropathies in equine patients. In human medicine, many EMG methods are available for the diagnosis, pathophysiological description and evaluation, monitoring, or rehabilitation of patients, and some of these techniques have also been applied to horses. EMG results are usually combined with other neurophysiological data, ultrasound, histochemistry, biochemistry of muscle biopsies, and clinical signs in order to provide a complete picture of the condition and its clinical course. EMG tec

Diagnostic yield of electromyography in children with myopathic disorders.

BACKGROUND: Interpretation of pediatric electromyography interpretation in myopathic disorders is technically challenging. We assessed our electromyographic experience with respect to sensitivity and specificity in pediatric myopathy. METHODS: We did a retrospective chart review of patients ≤18 years between 2009 and 2013. Two hundred twenty-four electromyographic studies were reviewed with the following referral diagnoses: myopathy, muscle weakness, neuromuscular disorders, myositis, myalgia, myoglobinuria, myasthenia, myotonia, cramps, periodic paralysis, hypotonia, and developmental delay.

Evaluation of neuromuscular junction disorders in the electromyography laboratory.

Neuromuscular junction (NMJ) disorders may be demonstrated using repetitive nerve stimulation (RNS) testing and single-fiber electromyography (SFEMG). RNS testing with low frequency stimulation reduces the safety factor of neuromuscular transmission (NMT) and may elicit decrementing compound muscle action potential (CMAP) responses. Exercise or tetanic nerve stimulation may potentiate acetylcholine release in presynaptic NMT disorders with CMAP facilitation. SFEMG is a selective recording technique assessing MFAPs within the same motor unit. Jitter is increased in NMJ disorders, and is the tem

The clinical value of laryngeal electromyography in laryngeal immobility.

Although laryngeal electromyography (LEMG) is commonly performed, there are no data confirming its efficacy. We evaluated 40 patients with a laryngoscopic diagnosis of unilateral vocal-fold immobility who underwent LEMG of the thyroarytenoid (TA) and cricothyroid (CT) muscle, with the immobile side of each muscle being compared to the normal side. The immobile side compared to the normal side showed more fibrillation potentials and positive sharp waves for the TA (p=0.04), longer MUAP duration for the TA (p=0.04) and CT (p=0.01), more polyphasic potentials for the TA (p=0.002), and more freque

Paediatric electromyography in the modern world: a personal view.

Paediatric electromyography (EMG) is an invaluable diagnostic test for the investigation of neuromuscular disease, but its use is inconsistent between and within different countries. One perception is that the procedure is painful; however, in comparison with common investigations performed routinely in children, EMG is better tolerated. While some developments, such as those within clinical genetics, would appear to mark its demise, paradoxically the more genetic abnormalities that are discovered in conditions such as hereditary neuropathy, the more precise a delineation of the phenotype is r

Electromyography of the anal sphincter: which muscle to examine?

Electromyographic examination of the deeper external anal sphincter (EAS) muscle is far more uncomfortable than of the subcutaneous muscle, so we tested the need for its examination. We compared the findings in 85 paired examinations of ipsilateral subcutaneous and deeper EAS muscles in 67 patients with a cauda equina lesion. When the other muscle was normal, the deeper or subcutaneous muscle was pathological in 18% or 13% of pairs, respectively. In addition, the subcutaneous EAS muscle was usually more severely affected than the deeper EAS muscle. Thus, examination of the deeper EAS muscle di

Electromyography of respiratory muscles in amyotrophic lateral sclerosis.

We reviewed the records of 52 amyotrophic lateral sclerosis (ALS) patients examined between 1995 and 2000 who had needle electromyography (EMG) of their respiratory muscles, including the diaphragm, at or near the time of their diagnosis. With respiratory function testing, patients with abnormal diaphragmatic EMG at diagnosis (Group 1, n=23) had significantly lower forced vital capacity (FVC), lower daytime arterial PO(2) and higher PCO(2) measurements (p<0.05) than patients with normal diaphragmatic EMG (Group 2, n=29). Twenty-eight percent of the patients without symptoms or signs of respira

[Single fiber electromyography in the diagnosis of myasthenia gravis].

INTRODUCTION: The electrophysiological exam complements the clinical diagnosis in patients with suspected myasthenia gravis. The most common used electrophysiological test are the repetitive nerve stimulation test (RNST) and the single fiber electromyography (SFEMG); this last one is more sensitive than the first one. OBJECTIVE: To describe the usefulness of SFEMG in a group of myasthenic patients. PATIENTS AND METHODS: We studied 52 patients with a clinical diagnosis of myasthenia gravis (female: 35, male: 17; age range: 10-48 years). They were classified according to Osserman (type I, IIa,

Electromyography of the infrahyoid muscles: pathological and normal findings.

UNLABELLED: In recent years, the infrahyoid muscles (IHM) have been used by plastic reconstructive surgeons as a neurovascular muscle flap in the neck and mouth region. METHODS: A preoperative electromyographic examination (EMG) of the IHM was performed in 10 patients, of whom 9 suffered from tongue cancer, in order to detect neurogenic lesions caused by possible metastases or lymph nodes. The results were compared to those of 10 healthy controls. RESULTS: The EMG at rest showed no pathological spontaneous activity in any patient. During light voluntary innervation, the motor unit potentials

Can electromyography predict the prognosis of transverse myelitis?

The role of clinical and magnetic resonance imaging (MRI) features on the prognosis of acute transverse myelitis has been studied, but the role of electromyography (EMG) changes, although reported, has not been investigated. Seventeen patients with acute transverse myelitis were subjected to clinical evaluation, MRI scanning and concentric needle EMG. The outcome was defined on the basis of a 3-month Barthel Index (BI) score as good or poor. The EMG changes in these groups were compared. All of the patients had complete paraplegia (power grade 0), except 1 who had grade III power. Mild upper l

Validity of long-term electromyography in the quantification of tremor.

We previously developed a method of tremor quantification using long-term electromyography registration of antagonistic forearm muscles that is reliable, sensitive, and specific for pathologic tremors. The present study demonstrates that tremor occurrence as measured by this method correlates well with clinical parameters of tremor in idiopathic Parkinson's disease (PD) and essential tremor (ET) (subscores of the Unified Parkinson's Disease Rating Scale for PD, tremor rating according to Bain et al. for ET). We conclude that the method is a valid and objective means of tremor quantification in

Major patterns of laryngeal electromyography and their clinical application.

Laryngeal electromyography (LEMG) is clinically valuable in the evaluation of laryngeal dysfunction and vocal fold immobility. To facilitate clinical application of this electrophysiologic test, a detailed description of modified LEMG techniques is presented. The techniques were applied for simultaneous bilateral recordings of the thyroarytenoid, cricothyroid, and posterior cricoarytenoid muscles. The basic patterns of LEMG are classified into three different types: normal, neuropathy, and myopathy. In an attempt to characterize these patterns, we have reported eight LEMG-documented cases: uni
