PURPOSE: We wanted to characterize the immediate effect of endarterectomy on flow of the arteries composing the extracranial carotid artery system.
METHODS: Transit time ultrasound probes were used to measure flow through the carotid bifurcation in 48 patients undergoing endarterectomy. Maximum single-diameter stenosis affecting the internal carotid artery (ICA) was determined by angiography. To evaluate haemodynamic effects of carotid endarterectomy on cerebral circulation, transcranial Doppler sonography was performed on 69 patients operated for unilateral carotid stenosis greater than 70%. Pre- and post-operative Vm and PI on bilateral middle-cerebral arteries, Vm PI and flow direction on ipsilateral anterior cerebral artery and Vm on basilar artery were evaluated. All patients presented more or less evident preoperative haemodynamic alterations of cerebral circulation and after surgery showed a normal distribution of cerebral blood supply.

Sixty-six patients (72 carotid arteries) were examined after carotid endarterectomy (CEA) using Doppler colour flow imaging (DCFI). Examinations were performed 4-18 days (mean: 7 days) after surgery (32 patients, 34 arteries) or between 2 and 100 months (mean: 39 months) after CEA (34 patients, 38 arteries). Late postoperative digital subtraction angiographic (DSA) findings after a mean follow-up of 6 years were examined after 61 internal carotid artery (ICA) endarterectomies performed for symptomatic (mainly TIAs) ICA stenosis on 52 patients. The risks of carotid angiography--technical, allergic, cardiac, renal, or emotional--may be lessened or absented by the use of real-time B-mode ultrasound to evaluate carotid artery disease. A patient with amaurosis fugax, a positive fluorescein angiogram, ulceration on ultrasound, and an unremarkable computed tomography scan was operated on without angiography. Cellular proliferation in response to endothelial injury has been examined extensively in experimental animals. This report examines and reviews the frequency, potential causes and management of blood pressure aberrations in 100 consecutive carotid thromboendarterectomies. BACKGROUND: Management and outcome of patients with operable chronic thromboembolic pulmonary hypertension (CTEPH) who underwent pulmonary endarterectomy (PEA) at a large German referral center were investigated.
METHODS: In Germany, 394 PEAs were performed in 2014 and 2015 with an in-hospital mortality rate of 5.8%.
