Endolymphatic hydrops in children.

Apart from clinical observations of Ménière's disease in children, there have only been a few histological descriptions of endolymphatic hydrops (EH) as an incidental finding in children. In this paper we report on the incidence of EH in temporal bones of children. One hundred eighteen temporal bones from 70 infants between newborn and 10 years (average age 12.9 months) were analyzed by light microscopy for EH of the cochlear duct, which was defined as a bulging of Reissner's membrane into the scala vestibuli in more than one turn of the cochlea. All endolymph spaces were carefully examined fo

Experimental endolymphatic hydrops in the rat.

In the rat, the epithelial lining of the endolymphatic duct and sac, and the contents of the sac, showed remarkable differences in comparison with these structures in the guinea pig and man. Obliteration of the endolymphatic duct resulted in endolymphatic hydrops of varying severity in 55% of the rats, after survival times varying from one to five months. The presence of a positive periodic acid-Schiff-stain for coagulum accumulating in the endolymphatic duct after prolonged saccus obliteration suggests active involvement of the endolymphatic duct in the migration of endolymph toward the endol

Vitamin A deficiency and sensory function.

Morphological investigation of tongue, olfactory epithelia, trachea and inner ear in vitamin A deficiency are reported. The results support assumptions concerning the loss of sensory function as been at least a secondary effect of alterations of the neighbourhood of the sensory cells caused by vitamin A deficiency. Taste buds are hindered in function by a dense layer of squamous cells and olfaction is decreased by atrophy of the surrounding respiratory epithelium. Inner ear functionality seems to be affected by vitamin A status via a stabilizing effect on the endolymph-perilymph barrier.

The double-membrane break syndrome in sudden hearing loss.

Some patients with sudden hearing loss actually have two membrane breaks, one at the oval or round window and one further inside the cochlea. One may heal spontaneously and the other not, or both, or neither. It is very likely impossible to detect this early from the threshold audiogram, because the intracochlear breaks allows endolymph and perilymph mixing which spreads over much of the anatomically normal clchlea causing diffuse loss of function. This theory is illustrated by three case histories.

Radiosurgery of vestibular schwannoma: prognostic factors for hearing outcome using 3D-constructive interference in steady state (3D-CISS).

PURPOSE: Stereotactic radiosurgery (SRS) is an effective treatment for vestibular schwannoma (VS). Three-dimensional (3D) constructive interference in steady state (CISS) is the preferred magnetic resonance imaging (MRI) sequence for evaluating signal changes in the inner ear endolymph. Previous studies demonstrated a correlation between pretreatment cochlear signal intensity in 3D-CISS and posttherapeutic hearing outcomes. The purpose of our study was to compare 3D-CISS sequences before and after primary SRS of unilateral VSs to evaluate the effect of radiosurgery on the 3D-CISS signal intens

NKCCs in the fibrocytes of the spiral ligament are silent on the unidirectional K⁺ transport that controls the electrochemical properties in the mammalian cochlea.

Unidirectional K(+) transport across the lateral cochlear wall contributes to the endocochlear potential (EP) of +80 mV in the endolymph, a property essential for hearing. The wall comprises two epithelial layers, the syncytium and the marginal cells. The basolateral surface of the former and the apical membranes of the latter face the perilymph and the endolymph, respectively. Intrastrial space (IS), an extracellular compartment between the two layers, exhibits low [K(+)] and a potential similar to the EP. This IS potential (ISP) dominates the EP and represents a K(+) diffusion potential elic

Gly460Trp alpha-adducin mutation as a possible mechanism leading to endolymphatic hydrops in Ménière's syndrome.

OBJECTIVE: Ménière's disease (MD) is an inner ear disorder characterized by recurrent episodic vertigo, hearing loss that is fluctuating in the first stages, aural fullness, and tinnitus. Raised endolymphatic pressure (hydrops) is commonly accepted as a causal condition. Approximately 90% of cases of MD are sporadic, whereas the remaining 10% of cases are linked to genetic factors. The ionic composition of endolymph may also depend on the activity of Na, K-ATPase. Adducin is a heterodimeric cytoskeleton protein consisting of 3 subunits (alpha, beta, and gamma) coded by 3 different genes (ADD1,

History of Meniere's disease and its clinical presentation.

The term Meniere's disease is used to define either the classic triad of vestibular and cochlear symptoms and aural pressure from known or unknown causes or its clinical variants, vestibular and cochlear Meniere's disease. Some variants evolve after years into typical forms, whereas others do not. Some symptoms (positional vertigo) have been long underestimated in previous reports. The more we study our patients and correlate clinical findings and the natural history with pathologic studies on temporal bones and laboratory research, the more we will understand Meniere's disease and its causes.

A model for signal transmission in an ear having hair cells with free-standing stereocilia. III. Micromechanical stage.

Measurements have shown that the sound-induced motion of free-standing stereocilia of hair cells in the alligator lizard cochlea exhibits tonotopically organized frequency selectivity that is correlated with the geometry of the stereociliary tuft. We propose a model in which basilar-membrane motion causes vibration of the receptor organ which drags the stereocilia back and forth through the endolymph. The stereociliary tuft is represented as a rigid rod attached to the cuticular plate by a compliant hinge. Viscous and inertial forces exerted by the endolymph on the rod are computed approximate

Human endolymphatic sac: possible mechanisms of pressure regulation.

The ultrastructure of the normal human endolymphatic sac (ES) has been observed by transmission electron microscopy (TEM). The role of the epithelium, the various regions of the subepithelial space and the general anatomy of the ES in pressure regulation were morphologically studied to generate testable hypotheses of human ES function. Light microscopic (LM) and TEM evidence of pressure regulatory mechanisms by endolymph resorption, mechanical factors, and secretory activity are presented. These mechanisms may be useful in designing experimental studies of the ES, and in interpretation of retr

The mechanics of benign paroxysmal vertigo.

Benign paroxysmal vertigo (BPV) is a disorder of the vestibular labyrinth. The clinical features can be explained by an abnormality in the posterior semicircular canal. Under the influence of gravity, a density differential between the endolymph and the cupula will cause displacement of the cupula when changes in head position occur. The presence or absence of fatiguability is a useful test as it helps define etiology, prognosis, and therapy. At the risk of adding yet another classification of nystagmus to the literature, we submit that division of BPV into two types (fatiguable and nonfatigua

The Epley maneuver for the treatment of benign paroxysmal positional vertigo.

In recent years, different forms of physical therapy have been proposed for the treatment of benign paroxysmal positional vertigo (BPPV). These mainly consist of maneuvers aiming to reposition and disperse free-floating endolymph particles in the posterior semicircular canal. We report our experience with one of these procedures, the Epley maneuver (EM), in treating 30 cases of BPPV. Twenty-six patients (87%) were cured, while four (13%) did not respond to treatment. These results are similar to those reported in the literature for the EM and the Semont maneuver.

[Benign paroxysmal postural vertigo: basis and its therapy in clinical practice].

The theory of canalolithiasis assumes that benign paroxysmal vertigo and nystagmus is due to particles within the semicircular canal which move freely in the endolymph under gravitational forces, thereby deflecting its cupula. According to this theory the particles could be removed from the posterior canal by manoeuvres applying a succession of different head positions. We describe the pathophysiological mechanism of benign paroxysmal vertigo and our experience with one of these manoeuvres, the Semont manoeuvre. The success rate is impressive and the therapy is highly rewarding in a vertigo sy

Hearing improvement in attacks of Meniere's disease treated with pressure chamber.

A method to reduce endolymphatic pressure by exposing patients with Meniere's disease to underpressure has earlier been described. In this work the possible mechanisms are discussed. A material of 36 patients with acute attacks, treated in this way, is now presented. 15 out of these patients experienced a rapid hearing improvement. In 11 cases the remission has lasted for more than 1 year. The method seems to be of great value for those who improved, since at least an early permanent damage to the cochlea can be avoided.

Three-Dimensional Volumetric Measurement of Endolymphatic Hydrops in Meniere's Disease.

Objective: We used volumetric three-dimensional (3D) analysis to quantitatively evaluate the extent of endolymphatic hydrops (EH) in the entire inner ear. We tested for correlations between the planimetric and volumetric measurements, to identify their advantages and disadvantages. Methods: HYDROPS2-Mi2 EH images were acquired for 32 ears (16 patients): 16 ipsilateral ears of MD patients (MD-ears) and 16 contralateral ears. Three-T MR unit with a 32-channel phased-array coil/the contrast agent to fill the perilymphatic space and the HYDROPS2-Mi2 sequence. We calculated the EH% [(endolymph)/(en

Variants encoding a restricted carboxy-terminal domain of SLC12A2 cause hereditary hearing loss in humans.

Hereditary hearing loss is challenging to diagnose because of the heterogeneity of the causative genes. Further, some genes involved in hereditary hearing loss have yet to be identified. Using whole-exome analysis of three families with congenital, severe-to-profound hearing loss, we identified a missense variant of SLC12A2 in five affected members of one family showing a dominant inheritance mode, along with de novo splice-site and missense variants of SLC12A2 in two sporadic cases, as promising candidates associated with hearing loss. Furthermore, we detected another de novo missense variant
