Ménière's disease is a pathologic condition of the inner ear that is characterized by vertigo, tinnitus and a progressive loss of hearing. When Ménière's disease is unresponsive to medical treatment and when destructive surgery is not advisable, patients, particularly the elderly, often benefit from endolymphatic sac enhancement, a conservative, nondestructive surgical procedure. We evaluated the outcomes of 62 such patients, aged 65 years and older, who underwent a total of 78 endolymphatic sac enhancements. We assessed their response to surgery by means of a questionnaire, which classified p

We correlated the surgical findings and the findings of computerized tomographic scans of the temporal bone in 38 ears from 38 patients with Menière's disease (31 unilateral and 7 bilateral) treated by endolymphatic sac (ES) drainage surgery between 1986 and 1991. At surgery, the position and size of the ES were recorded. The position of the ES was classified as one of three types according to Arenberg's parameters (type I, type II and type III) and the size of the ES was classified as large, intermediate or small. Scans encompassing the lateral semicircular canal were used to measure the mini

OBJECTIVE: Endolymphatic sac decompression (ESD) for the treatment of Ménière's disease (MD) has had limited success and variable results in the literature. We have devised a novel technique that involves blocking the endolymphatic duct with surgical clips. In a separate effort to study the endolymphatic sac (ELS), we have sectioned the lateral part of the main body of the ELS as biopsies from a subset of patients. We aimed to evaluate the effect of the lateral part sectioning of the ELS on short-term surgical outcomes.
MATERIALS AND METHODS: This was a single-physician, retrospective study i

Three of the present authors described a trial in 1981 which showed that cortical mastoidectomy was as effective as an endolymphatic sac shunt in controlling vertigo in patients with Ménière's disease both on a short-term as well as a long-term basis. The hearing was not influenced by the treatment and any change was considered to be the result of time, rather than the effect of the surgery. Upon perusal of the literature only two other studies exist resembling our study, and they both reached the same conclusion, that sac surgery is at best non-specific and at worst of no value. None of the n

In 1981, Danish physician Jens Thomsen conducted the first and only documented sham-controlled surgical trial in the history of otolaryngology. This trial is historically significant as it was the first in the field to use a methodologically sound study design to address a frustratingly complex disorder such as Ménière's disease. Despite this, historical interpretations of this work have varied, and questions about the results have been raised. We review the fascinating historical context of this landmark trial and detail how it was influenced by the rise of the randomized controlled trial. We

To increase the accuracy of identification of the endolymphatic sac during endolymphatic sac drainage surgery, we focused on the intra-operative identification of the operculum as a structural marker of the endolymphatic sac and the vestibular aqueduct at the rugose portion. Then, we examined the relationship between identification of the operculum and surgical outcomes. We used 100 cases with intractable Meniere's disease who received endolymphatic sac drainage with local steroid administration, superior to traditional endolymphatic sac drainage surgery and non-surgical medical treatment. HYPOTHESIS: To identify structural differences between the endolymphatic sac of temporal bones with and without Ménière's disease by applying design-based stereology.
BACKGROUND: The dimension of the human endolymphatic sac has previously been studied by extrapolating 2-dimensional irregularities on the sectional level into 3-dimensional quantities via unfolding algorithms. These computer renderings require certain geometrical assumptions of the object studied. The term design-based used in the present study signifies that the methods and sampling schemes are defined a priori, in such a manne

To clarify the role of the endolymphatic sac (ES) in the regulation of endolymphatic pressure, the effects of isoproterenol, a beta-adrenergic receptor agonist, and acetazolamide, a potent carbonic anhydrase inhibitor, both of which decrease ES direct current potential on cochlear hydrostatic pressure, were examined in guinea pigs. When isoproterenol was applied intravenously, hydrostatic pressures of cochlear endolymph and perilymph were significantly increased with no change in endocochlear potential or the hydrostatic pressure of cerebrospinal fluid. Acetazolamide produced no marked change

CONCLUSION: Endolymphatic sac tumors (ELSTs) are locally invasive, osteolytic, and hypervascular tumors. If the labyrinth has not been invaded, the hearing should be preserved in operation.
OBJECTIVE: To summarize three cases of ELST and discuss whether to keep hearing in the surgical treatment of ELST.
SUBJECTS AND METHODS: Complete otorhinolaryngological examinations with audiologic, vestibular, and neurologic evaluations were performed. CT and MRI scans were carried out before operation and in the course of follow-up. The patients underwent radical removal of the tumor through a transmast

OBJECTIVE: Large endolymphatic duct and sac syndrome (LEDS) is known as the most common kind of inner ear malformations, which is radiologically detectable. Nevertheless, nowadays many questions are not fully cleared and LEDS is relatively unknown among general radiologists. The aim of this study was to evaluate the incidence of LEDS in the own patient population and to present our experiences regarding imaging findings, clinical presentation and follow up.
MATERIALS AND METHODS: Based on a complete recording of all patients, sent from ENT department to radiology, we identified all radiologic

The endolymphatic sac is a part of the homeostasis-regulating system of the membranous labyrinth of the inner ear. Disturbances in the function of the endolymphatic sac are believed to be involved in the genesis of different inner ear disorders, such as endolymphatic hydrops and Ménière's disease. To make studies of the ion- and fluid-regulating mechanisms of the sac possible, a method to culture the tissue in vitro was developed. Epithelial cells and fibroblasts were morphologically characterised in the cell cultures with light and electron microscopy as well as immunohistochemically using an

The anti-diuretic hormone vasopressin (AVP) regulates water excretion from the kidney by increasing the water permeability of the collecting duct. AVP binds to V2-receptors and induces the translocation of aquaporin-2 water channels (AQP-2) into the apical plasma membrane of principal cells. By this mechanism AVP controls water reabsorption in the kidney. The effects of AVP on the endolymphatic sac (ES) of the inner ear, which is thought to mediate reabsorption of endolymph, were investigated. Both the V2-receptor and the AQP-2 water channel were found to be expressed in the ES epithelium. In

The role of endolymphatic sac (ES) dysfunction in the etiopathogenesis of Meniere's disease has remained controversial since the early 1900s. The first reports of the ultrastructural (transmission electron microscopy, TEM) pathology of the human ES in Meniere's disease have been published only in the last decade. These studies have been based on biopsies of the extraosseous (intradural) ES and in no cases has the TEM appearance of the intraosseous ES been described. Likewise the control material used has been from biopsies of extraosseous ES taken from patients with acoustic schwannomas. To da

Hyalin bodies are amorphous, eosinophilic masses protruding from the subepithelial connective tissue into the lumen of the intraosseous endolymphatic sac (ES). We studied hyalin bodies at the electron microscopic level. Celloidin- embedded temporal bone sections known to have hyalin bodies were re-embedded into plastic and cut into thin sections appropriate for electron microscopy. The results revealed that the hyalin bodies are composed predominantly of thick bundles of collagenous fibers arranged in various directions. Fibroblasts and disintegrated macrophages were occasionally observed amon

Hyalin bodies are amorphous, eosinophilic masses that protrude from the subepithelial connective tissue into the lumen of the endolymphatic sac. In this study, hyalin bodies were analyzed in two groups of temporal bones: normal bones and bones with cochlear otosclerosis. The results revealed that bones with cochlear otosclerosis had significantly more and larger hyalin bodies that did normal bones. In addition, the hyalin bodies in cochlear otosclerosis were denser and associated with more edema and loose connective tissue in the surrounding areas. Foamy macrophages, concentric calcific struct

The endolymphatic sacs of ten vervet monkeys were surgically ablated unilaterally, and the animals were assigned predetermined survival times ranging from 6 months to 5 years. The left and right ears were prepared in anatomical continuity for light microscopic study. Four experimental ears had openings in their membranous labyrinths of unknown etiology and were not considered acceptable for correlative studies. The remaining five experimental ears showed mild cochlear endolymphatic hydrops, which was localized to the apical regions or was more severe in the apical regions than elsewhere. Four

Scanning electron microscopy has been used to examine and compare one normal endolymphatic sac with one endolymphatic sac from a patient with Meniere's disease. The surgical procedure for obtaining these specimens and their preparation for scanning electron microscopy are described. The luminal surface of the rugose portion of both specimens was lined with two populations of epithelial cells: one with a dome-shaped apical surface, the other with a flattened polygonal surface. The surface of dome-shaped cells in both specimens was covered with microvilli. Neither specimen had observable loss of

OBJECTIVE: Endolymphatic hydrops is the primary pathological feature of Menière's disease (MD). An imbalance between endolymphatic fluid production and absorption due to endolymphatic sac (ES) dysfunction may be the major cause of endolymphatic hydrops. This study aimed to identify and characterize global protein expression of the ES in MD patients using microscale proteomics for the first time.
METHODS: Liquid chromatography-tandem mass spectrometry was used to analyze the ES samples from six MD patients and six acoustic neuroma (AN) patients (as control) for microscale proteomics and differ
