Endoscopic Retrograde Cholangiopancreatography and Endoscopic Ultrasound-Guided Gallbladder Drainage.

"Gallbladder disease is one of the most common gastrointestinal diseases encountered in clinical practice. Surgical removal and percutaneous drainage are both widely available and effective in the management of acute cholecystitis. Several endoscopic approaches exist as an alternative to these interventions. These include transpapillary approaches via endoscopic retrograde cholangiopancreatography (ERCP), transmural drainage and access approaches via endoscopic ultrasound (EUS), and endoscopic surgical approaches using natural orifice transluminal endoscopic surgery (NOTES) techniques. This ar

Clinical endoscopic management and outcome of post-endoscopic sphincterotomy bleeding.

Post-endoscopic sphincterotomy bleeding is a common complication of biliary sphincterotomy, and the incidence varies from 1% to 48%. It can be challenging to localize the bleeder or to administer various interventions through a side-viewing endoscope. This study aimed to evaluate the risk factors of post-endoscopic sphincterotomy bleeding and the outcome of endoscopic intervention therapies. We retrospectively reviewed the records of 513 patients who underwent biliary sphincterotomy in Mackay Memorial Hospital between 2011 and 2016. The blood biochemistry, comorbidities, indication for sphinct

Three-dimensional endoscopic visualization in functional endoscopic sinus surgery.

Three-dimensional (3D) stereoscopic vision in sinus surgery has been achieved with the microscope so far. The introduction of two-dimensional (2D) endoscopes set a milestone in the visualization of the surgical field and paved the way to functional endoscopic sinus surgery (FESS), although the 2D endoscopes cannot provide a stereoscopic visualization. The latest technology of 3D endoscopes allows stereoscopic vision. We provide a clinical investigation of all commercially available 3D endoscopes in FESS to compare their clinical value and efficacy to routinely used conventional 2D HD endoscope

Endoscopic skull base reconstruction after endoscopic endonasal approach.

Recent advances in technology and techniques in neurosurgery and otolaryngology have revolutionised skull base surgery with the introduction of endoscopic endonasal approaches. One of the greatest limitations of endoscopic endonasal skull base surgery is the repair of osteodural defects. Numerous reconstructive techniques have been developed that maximise endoscopic endonasal approach efficacy and minimise complications. The choice of technique for closure depends on different factors and is based on an individual approach in each patient. In particular, size, the localisation of the defect an

Endoscopic resection after endoscopic hemostasis for hemorrhagic gastric cancer.

A 68 years old man was referred to our hospital with symptoms of hematemesis and melena. An emergent gastroscopy showed a gastric ulcerative lesion with an exposed vessel (Forrest IIa) protruding from its base, which was located at the posterior wall of the upper portion of the gastric body. Endoscopic hemostasis was performed with endoclips and antiulcer treatment was done. Although the ulcerative lesion was healed two months after endoscopic hemostasis, the histopathological examination of the biopsy specimens revealed well differentiated adenocarcinoma (0-IIc). The tumor could be resected e

Endoscopic sclerotherapy as compared with endoscopic ligation for bleeding esophageal varices.

BACKGROUND: Endoscopic sclerotherapy is an accepted treatment for bleeding esophageal varices, but it is associated with substantial local and systemic complications. Endoscopic ligation, a new form of endoscopic treatment for bleeding varices, may be safer. We compared the effectiveness and safety of the two techniques. METHODS: In this randomized trial we compared endoscopic sclerotherapy and endoscopic ligation in 129 patients with cirrhosis who had proved bleeding from esophageal varices. Sixty-five patients were treated with sclerotherapy, and 64 with ligation. Initial treatment for acut

Endoscopic removal of gastric lipoma: diagnostic value of endoscopic ultrasonography.

A 47-yr-old male with gastric lipoma is presented. X-ray and endoscopic examinations revealed a submucosal tumor on the posterior wall of the gastric antrum. Endoscopic ultrasonography demonstrated the hyperechoic mass in the submucosa without continuity to the muscularis propria, by which the lesion was diagnosed as lipoma. The lesion was successfully removed by endoscopic polypectomy without complications. The cross-sections of the removed specimens were quite consistent with the ultrasonographic findings. Endoscopic ultrasonography is valuable in assessing the exact extent of the tumor and

Endoscopic retrograde cholangiopancreatography and endoscopic papillotomy in recurrent pyogenic cholangitis.

In recurrent pyogenic cholangitis (RPC), there is primary bacterial cholangitis resulting in the formation of strictures and stones in the intrahepatic as well as the extrahepatic bile ducts. Endoscopic retrograde cholangiopancreatography (ERCP) is a very useful investigation in the study of RPC. The location of stones and strictures and the morphology of the bile ducts are well delineated. Moreover, cholangitis liver abscesses and biliary-enteric fistulas, which are frequently encountered in RPC, are demonstrated. ERCP can also be used to differentiate RPC from ascariasis, clonorchiasis, hepa

Endoscopic Adaptive Transformer for Enhanced Polyp Segmentation in Endoscopic Imaging.

Polyp segmentation in endoscopic imaging is essential for the early detection of colorectal cancer, as polyps are precursor lesions in the colon and rectum, yet the task is complicated by the morphological variability and indistinct boundaries of polyps, which often blend into surrounding tissues. Conventional approaches struggle with these complexities, as fixed scale and window sizes are unable to adapt to the diverse and irregular structures of polyps. To address this challenge, we introduce the Endoscopic Adaptive Transformer, EAT, a novel framework specifically engineered for polyp segmen

Endoscopic remodeling of esophageal stenosis refractory to traditional endoscopic treatment.

Esophageal atresia (EA), with or without tracheoesophageal fistula, is the most common congenital anomaly of the esophagus. Surgical correction is the primary treatment, however, up to 80% of patients experience anastomotic stenosis, with esophageal balloon dilation (EBD) being the first-line treatment. Unfortunately, some patients develop refractory anastomotic stenosis (RAS), defined by the presence of strictures after three EBD sessions. The endoscopic management of RAS remains controversial. Although EA itself is rare, the incidence of postoperative stricture is significant, contributing t

[Endoscopic Treatment for Esophageal Achalasia:Per-oral Endoscopic Myotomy( POEM)].

Per-oral endoscopic myotomy( POEM) is an endoscopic treatment of achalasia. Since the world's first case was performed in Japan in 2008, over 15 years have passed. More than 3,200 cases have been performed at Showa University alone, and over 6,000 cases have been performed throughout Japan. The success rate in Japan exceeds 97%. Internationally, the efficacy of POEM has been demonstrated to be equal to or better than surgical treatment in randomized controlled trials( RCTs) with Heller-Dor surgery [The New England Journal of Medicine (NEJM) 2019]. Additionally, the superiority of POEM over bal

Co-treatment with endoscopic laryngopharyngeal surgery and endoscopic submucosal dissection.

OBJECTIVE: Endoscopic laryngopharyngeal surgery (ELPS) is an effective and minimally invasive treatment for pharyngeal cancers. However, the disadvantages of ELPS are the interference of instruments in the operative field and the difficulty in approaching certain areas. To overcome these drawbacks, we began to perform combination treatment of ELPS and endoscopic submucosal dissection (ESD). The aim of the present study was to compare the efficacies of treatment with ELPS alone and ELPS combined with ESD. METHODS: A total of 103 lesions in 73 patients who underwent pharyngeal ELPS for superfic

Endoscopic ultrasonographic assessment of gastric polyps and endoscopic mucosal resection.

Gastric polyps refer to a heterogeneous group of epithelial- and subepithelial-based lesions that vary in histology and malignant potential. Although many gastric polyps have characteristic endoscopic appearances, their diagnosis, classification, and potential for malignant transformation are based on histologic evaluation. Therefore, all types of gastric polyps should be biopsied during the incident endoscopy. Mucosal-based polyps include fundic gland polyps, hyperplastic polyps, adenomatous polyps, and hamartomatous polyps. Inflammatory fibroid polyps may endoscopically appear similar to muc
