Adequate endoscopic mucosal resection for early gastric cancer obtained from the dissecting microscopic features of the resected specimens.

BACKGROUND: We have employed endoscopic mucosal resection (EMR), using a cap-fitted panendoscope (EMRC), for early gastric cancer since 1992. The presence of an adequate surgical margin is a requirement because of the radicality of EMR, and dissecting microscopic examination is useful in regard to the diagnosis of spread of the disease. METHODS: To devise an adequate method of EMR that allows no lateral residue, we examined gastric mucosal specimens obtained by EMRC. One hundred and sixty-seven specimens from 97 lesions in 85 patients treated by EMRC were examined in regard to characteristic

Successful endoscopic closure of a colonic perforation one day after endoscopic mucosal resection of a lesion in the transverse colon.

A 73-year-old man underwent endoscopic mucosal resection (EMR) of a 20-mm flat elevated lesion on the transverse colon. The morning after the procedure, he started to have severe right upper quadrant pain after his first meal. A computed tomography scan revealed free air and a stomach filled with food. He was diagnosed to have delayed post-EMR intestinal perforation. He underwent emergent colonoscopy and clipping of the perforated site. He was discharged 8 d after the endoscopic closure without the need for surgical intervention. The meal was not the cause of the colon transversum perforation.

Feasibility study of cap-assisted endoscopic mucosal resection for gastric adenocarcinoma of fundic gland type.

BACKGROUND: Gastric adenocarcinoma of fundic gland type (GA-FG) is a rare subtype that typically arises in the upper stomach, making endoscopic submucosal dissection (ESD) technically challenging. Cap-assisted endoscopic mucosal resection (EMRC) is widely used for early gastric cancer due to its shorter procedure time and improved safety profile. We evaluated its efficacy and safety for GA-FG. METHODS: We retrospectively analyzed patients with GA-FG who underwent EMRC or ESD between April 2013 and September 2024. Outcomes included en bloc, complete, and curative resection rates; procedure tim

Right kidney contusion following endoscopic mucosal resection of hepatic flexure lymphangioma: a rare case report.

Endoscopic mucosal resection (EMR) is widely utilized for the removal of benign colonic lesions; nevertheless, it carries certain inherent risks. We present a rare case of right kidney contusion following EMR for hepatic flexure lymphangioma, representing an uncommon and underreported complication. A 50-year-old woman underwent colonoscopy, which identified a 3 cm cystic mass at the hepatic flexure, subsequently diagnosed as a lymphangioma. EMR was performed with electrocoagulation and mechanical clamping. Postoperatively, she developed progressively worsening upper abdominal pain but did not

Tip-fixing underwater endoscopic mucosal resection without submucosal injection for a laterally spreading colon polyp.

BACKGROUND AND AIMS: Underwater endoscopic mucosal resection (EMR) has become a popular endoscopic resection method for intermediate-to-large colorectal polyps. However, the snare tip can sometimes slip when opening or closing the snare, resulting in increased risk of piecemeal resection. To address this issue, we report our technique of tip-fixing underwater EMR without submucosal injection for a laterally spreading colon polyp. METHODS: Degassed water was infused using a mechanical water pump to completely fill the lumen. By projecting the tip of the snare by 2 mm, a mucosal incision was ma

Clinical implications of a machine learning model predicting colorectal polyp recurrence after endoscopic mucosal resection.

The machine learning model developed by Shi et al for predicting colorectal polyp recurrence after endoscopic mucosal resection represents a significant advancement in the field of clinical gastroenterology. By integrating patient-specific factors, such as age, smoking history, and Helicobacter pylori infection, the eXtreme Gradient Boosting algorithm enables precise personalised colonoscopy follow-up planning and risk assessment. This predictive tool offers substantial benefits by optimising surveillance intervals and directing healthcare resources more efficiently toward high-risk individual

Safety of endoscopic mucosal resection (EMR) of large non-pedunculated colorectal adenomas in the elderly.

BACKGROUND: Endoscopic mucosal resection (EMR) has been proven to be safe and effective for the treatment of colorectal adenomas. However, data are limited on the safety of this technique for large polyps and in elderly patients. Aims of our study were to examine the bleeding and perforation rates in patients with large non-pedunculated adenomas (≥20mm) and to evaluate the influence of size (≥40mm) and age (≥75 years) on the complication rates. METHODS: In this multicenter retrospective study, patients who underwent EMR of non-pedunculated adenomas ≥20mm between January 2012 and March 2016 we

Selective binding of sucralfate to endoscopic mucosal resection-induced gastric ulcer: evaluation of aluminium adherence.

We evaluated the effect of sucralfate in patients with early gastric cancer in endoscopic mucosal resection (EMR)-induced gastric ulcers, and in rats with acetic acid-induced ulcers, by measuring concentrations of aluminium adhering to mucosal lesions. Twenty-two patients who underwent EMR received sucralfate with or without ranitidine and were examined endoscopically after 1 week, 2 weeks and 3 weeks. Gastric juice pH and concentration of aluminium in samples of ulcerated and normal mucosa were measured at various time-points. Good ulcer healing was observed in all patients. Significantly hig

Body composition after treatment of intra-mucosal gastric cancer with endoscopic resection compared to gastrectomy.

BACKGROUND: Patients who undergo gastrectomy may experience changes in their body composition, which may in turn affect survival. However, whether endoscopic resection differs from gastrectomy in its effect on body composition remains unclear. METHODS: This retrospective study included 2267 patients who received either gastrectomy or endoscopic resection for intramucosal gastric cancer. Transverse images at the L3 vertebra level of the patient's staging and follow-up CTs were segmented for the measurement of fat and skeletal muscle areas. In 1134 patients who were propensity score-matched, as

Outcomes of thermal ablation of the defect margin after duodenal endoscopic mucosal resection (with videos).

BACKGROUND AND AIMS: Laterally spreading lesions (LSLs) in the duodenum are conventionally treated by EMR. Recurrence is commonly encountered and can be difficult to treat safely due to the unique anatomic characteristics of the duodenum. Auxiliary techniques designed to prevent recurrence have not been described. METHODS: We sought to evaluate the effectiveness of thermal ablation of the defect margin after EMR (EMR-T) in reducing recurrence at first surveillance endoscopy (SE1, scheduled at 6 months) in a single tertiary referral center. All duodenal LSLs ≥10 mm referred for EMR were eligib
