Ultrasound assessment of patellar enthesopathy in patients with Ehlers-Danlos syndrome with and without concomitant axial spondyloarthropathy.

AIM: Ehlers-Danlos syndromes (EDS) are connective tissue disorders characterized by joint hypermobility, skin hyperextensibility, and tissue fragility. When spondyloarthritis (SpA), an inflammatory arthritis, co-occurs with EDS, overlapping symptoms of joint pain may arise, making diagnosis and management challenging. The aim of this study was to investigate enthesopathic differences in the patellar tendon between patients with EDS and those with co-occurring SpA. MATERIAL AND METHODS: Twenty-three participants with EDS and co-occurring SpA (SpA + EDS) and 14 participants with EDS without SpA

Enthesopathy of the pectoralis major tendon mimicking osteoid osteoma. A case report with an unfortunate series of events.

BACKGROUND: we present the case of an enthesopathy at the proximal humerus which was initially - due to the clinical history and a positive bone scintigraphy - regarded suspicious for metastatic breast cancer in a 50-year-old woman. CASE REPORT: after complementing radiographs and a magnetic resonance (MR) examination exhibiting a focally contrast enhancing juxtacortical osteolysis of the humerus, a metastasis seemed radiologically unlikely, but besides a traction-related periosteal reaction of the pectoralis major tendon an unusual osteoid osteoma could not unequivocally be ruled out. Althou

Treatment of Catastrophic Infection After Surgery for Insertional Achilles Enthesopathy: A Case Report and Review of the Literature.

UNLABELLED: Wound complications after surgical treatment of insertional Achilles enthesopathy are well documented. Skin and tendon necroses pose a significant dilemma with the potential for catastrophic outcomes. Numerous treatment algorithms have been described to treat the resultant skin and tendon defects after catastrophic infection; however, to date, there is no consensus as to the optimal treatment modality. We report our experience in the management of deep infection of 2 patients who had previously undergone surgical treatment for insertional Achilles enthesopathy. A comprehensive revi

Plantar enthesopathy: thickening of the enthesis is correlated with energy dissipation of the plantar fat pad during walking.

BACKGROUND: The enthesis of the plantar fascia is thought to play an important role in stress dissipation. However, the potential link between entheseal thickening characteristic of enthesopathy and the stress-dissipating properties of the intervening plantar fat pad have not been investigated. PURPOSE: This study was conducted to identify whether plantar fat pad mechanics explain variance in the thickness of the fascial enthesis in individuals with and without plantar enthesopathy. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: The study population consisted of 9 patients

Mineral tessellation in mouse enthesis fibrocartilage, Achilles tendon, and Hyp calcifying enthesopathy: A shared 3D mineralization pattern.

The hallmark of enthesis architecture is the 3D compositional and structural gradient encompassing four tissue zones - tendon/ligament, uncalcified fibrocartilage, calcified fibrocartilage and bone. This functional gradient accommodates the large stiffness differential between calcified bone and uncalcified tendon/ligament. Here we analyze in 3D the organization of the mouse Achilles enthesis and mineralizing Achilles tendon in comparison to lamellar bone. We use correlative, multiscale high-resolution volume imaging methods including μCT with submicrometer resolution and FIB-SEM tomography (b

Studies on type II collagen induced arthritis in rats: an experimental model of peripheral and axial ossifying enthesopathy.

It was suggested that type II collagen induced arthritis in rats may be considered an experimental model for rheumatoid arthritis (RA) because of clinical, histological, and immunological similarities. However, some features separate it from RA. We studied 40 inbred female Wistar Furth rats. Two weeks after immunization with native human type II collagen, they had polyarthritis which progressed to ankylosis associated with ossifying enthesopathy and periosteal new bone formation. Inflammatory nodules of the tail appeared after 2 months, with radiological and histopathological aspects of multis

Clinical implications of reconsideration of enthesitis/enthesopathy/enthesial erosion, as tendon attachment-localized avulsions and stress fracture equivalents.

Recognizing the mechanical origin of enthesitis/enthesopathy and the avulsion-nature of what had previously been considered erosions, it seems inappropriate to attribute it to stresses related to a person's normal activities. Conversely, sudden or unconditioned repetitive stresses appears the more likely culprit. Studies of enthesial reaction have lacked standardization as to findings present among individuals who appear to be healthy. Clinical evaluation by palpation and manipulation may be as effective as application of radiologic techniques. Recognition of the mechanical nature of the disea

Lipopolysaccharide treatment suppresses spontaneously developing ankylosing enthesopathy in B10.BR male mice: the potential role of interleukin-10.

BACKGROUND: Ankylosing enthesopathy (ANKENT) is an animal model of human ankylosing spondylitis. ANKENT is an inflammatory disease affecting the ankle and tarsal joints of the hind limbs in susceptible mouse strains. In the disease, the participation of intestinal microbiota components was suggested. Therefore, we attempted to increase the incidence of ANKENT by systemic administration of lipopolysaccharide (LPS), which is a component of bacterial cellular walls and stimulates inflammatory processes. METHODS: ANKENT occurrence, serum cytokine profiles, spleen cellular composition and in vitro

Are the Pathologic Features of Enthesopathy, Tendinopathy, and Labral and Articular Disc Disease Related to Mucoid Degeneration? A Systematic Review.

BACKGROUND: Tendinopathy, enthesopathy, labral degeneration, and pathologic conditions of the articular disc (knee meniscus and ulnocarpal) are sometimes described in terms of inflammation or damage, while the histopathologic findings are often consistent with mucoid degeneration. A systematic review of the histopathology of these structures at diverse locations might reconceptualize these diseases as expected aspects of human aging. The potential benefits of this evolution might include healthier patient and clinician mindsets as well as a reduced likelihood of overdiagnosis and overtreatment

Analysis and comparison of autologous platelet-rich plasma preparation systems used in the treatment of enthesopathies: A preliminary study.

BACKGROUND: Autologous platelet-rich plasma (PRP) injection is an alternative but widely accepted method for the treatment of degenerative changes in tendon attachments known as enthesopathies. The PRP is considered a safe source for high concentrations of the growth factors involved in the healing process. Despite initial promising outcomes, many recent studies report conflicting results for this treatment. This may be due to differences in the concentrations of platelets and growth factors in PRPs obtained using different methods. OBJECTIVES: The aim of this study was to compare PRP prepara

The absence of human beta 2-microglobulin increases the occurrence of ankylosing enthesopathy in HLA-B27 transgenic mice.

HLA-B27 transgenic mice develop a spontaneous ankylosing enthesopathy (ANKENT). We have investigated the occurrence of ANKENT in transgenic mice carrying transgenes for human beta 2-microglobulin (M TGM), HLA-B27-heavy chain (27 TGM), or both (27M TGM). An unexpected finding was the increase in ANKENT occurrence among the HLA-B27 transgenic mice lacking the human beta 2-microglobulin transgene (27 TGM): 33% of such mice were found to develop ANKENT, whereas 19% of 27M mice were diseased. In addition, the expression of HLA-B27 molecules in individual 27 TGM was highly variable, ranging from no

A Case Report of Deep Gluteal Syndrome Secondary to Piriformis Enthesopathy Successfully Treated With Ultrasound-Guided Dextrose Prolotherapy.

Deep gluteal syndrome (DGS) is a challenging cause of buttock pain and sciatic-like symptoms, often mimicking lumbar radiculopathy and remaining difficult to diagnose when conventional imaging is inconclusive. Structural abnormalities within the deep gluteal space, particularly at muscle-tendon insertions, may contribute to mechanically mediated sciatic nerve irritation but are frequently overlooked. We report a case of DGS in a 45-year-old man, caused by piriformis enthesopathy with a cortically based bony spur at the greater trochanter, identified using computed tomography (CT) and functiona
