Nocturnal enuresis in children is associated with differences in autonomic control.

STUDY OBJECTIVES: To assess the relationship between urine osmolality, cardiovascular parameters, and nocturnal enuresis in a population of children undergoing polysomnographic assessment. METHODS: This prospective observational study included consecutive children aged 5-17 years presenting for overnight polysomnography. Children were evaluated using continuous ambulatory blood pressure monitoring to assess heart rate and blood pressure. Urine samples were collected throughout the night to determine urine sodium excretion and osmolality. Comparisons of results were made between children with

Importance of neuropsychiatric evaluation in children with primary monosymptomatic enuresis.

BACKGROUND: Nocturnal enuresis (NE) is an involuntary voiding during sleep. It is a very common disorder in school-age children. Comorbid psychopathologies are common in patients affected by enuresis. According to the ICCS, the rate of behavioral and emotional disorders in children with enuresis is doubled compared with healthy control (HC) children. OBJECTIVE: The aim of the present study was to investigate the prevalence of neuropsychiatric comorbidities in children affected by NE. STUDY DESIGN: Two hundred children with a diagnosis of enuresis were recruited from the Neuropsychiatric Unit

Enuresis and Punishment: The Adverse Effects on Child Development and on Treatment.

OBJECTIVE: To investigate the role of punishment in enuretic children and how the punishments can influence therapy response. METHODS: We enrolled 218 enuretic children. The children and their families were asked to participate in the study at the end of the clinical evaluation. RESULTS: The analysis of the questionnaires shows that at least one punishment because of nocturnal enuresis (NE) had been applied to 27 out of 218 (12.4%) children. Punishment methods were reprimanding in 19 out of 27 (70.4%), depriving of sleep in 11 out of 27 (40.7%), mildly beating in 3 out of 27 (11.1%), leaving

Eosinophilic Cystitis: A Rare Cause of Nocturnal Enuresis in Children.

INTRODUCTION: Eosinophilic cystitis (EC) is a rare and poorly understood inflammatory condition, characterized by eosinophilic infiltration of all layers of the bladder wall, which mimics bladder tumors. EC may present with symptoms such as increased urination frequency, dysuria, gross/microscopic hematuria, suprapubic pain and urinary retention. CASE PRESENTATION: We present a 17-year-old male patient, who was continent night and day in his childhood, and was admitted to our clinic for complaints of hematuria and nocturnal enuresis for the past six months. His history and physical examinatio

Nocturnal enuresis and nocturia, differences and similarities - lessons to learn?

This review highlights the current views on and differences and similarities between nocturnal enuresis (NE) in children and nocturia in adults, which might be a guidance to elucidate the missing links in our knowledge. In both conditions, a genetic factor is suspected. Reduced bladder capacity and nocturnal polyuria are the main underlying lower urinary tract-related conditions. There is a link with sleep disorders, although it is not clear whether this is a cause or consequence. Physical and mental health are comprised in both conditions, however, in different ways. In NE, constipation and a

Winter is associated with failure in the alarm treatment of nocturnal enuresis.

OBJECTIVE: To assess whether the winter season in Japan is associated with failure in the alarm treatment of nocturnal enuresis (NE). PATIENTS AND METHODS: Consecutive patients with NE referred to our center between June 2009 and May 2010 were treated with the enuresis alarm (EA). The EA was used for 16 weeks with each child. Patients were divided into a success group and a treatment failure group. Pretreatment variables were collected, including age, sex, night-time urine volume, severity of enuresis, presence of nocturnal polyuria, presence of daytime incontinence, and treatment initiation

Spina bifida occulta: not to be overlooked in children with nocturnal enuresis.

OBJECTIVE: Previous reports have suggested that the incidence of spina bifida occulta in patients with nocturnal enuresis is higher than in the general population. However, the effect of spina bifida occulta on the response to nocturnal enuresis treatment is controversial. The purpose of this study was to investigate the relationship between spina bifida occulta and response to treatment of nocturnal enuresis. METHODS: Between 2006 and 2009, the records of 160 children with nocturnal enuresis were reviewed. Children with other organic urological disease or symptoms suggestive of spinal dysrap

Simple behavioural and physical interventions for nocturnal enuresis in children.

BACKGROUND: Nocturnal enuresis (bedwetting) is a socially disruptive and stressful condition which affects around 15-20% of five year olds, and up to 2% of young adults. Although there is a high rate of spontaneous remission, the social, emotional and psychological costs can be great. Simple behavioural methods of treating bedwetting include reward systems such as star charts given for dry nights, lifting or waking the children at night to urinate, retention control training to enlarge bladder capacity (bladder training) and fluid restriction. OBJECTIVES: To assess the effects of simple behav

Desmopressin tablets in the treatment of severe nocturnal enuresis in adolescents.

BACKGROUND: In recent years the treatment of primary nocturnal enuresis (PNE) with desmopressin (DDAVP) has been promising. The route of administration until now had been intranasal, but because the tablets were introduced for the treatment of diabetes insipidus they have also become available for the treatment of PNE. OBJECTIVES: To find the optimal dosage of desmopressin tablets and to compare desmopressin's efficacy with placebo in a group of adolescents with severe monosymptomatic enuresis. The long-term safety of desmopressin was also studied in the same group of patients. METHODS: The

Significance of spina bifida occulta in children with diurnal enuresis.

We reviewed retrospectively 456 patients with diurnal enuresis to determine the relationship between spina bifida occulta and attainment of continence. Of these patients 127 had undergone x-rays of the spine and 48 children (48%) had spina bifida occulta. The clinical course of these patients was compared to 79 enuretic children with normal spine films. All patients were initially treated with a timed voiding program and 28 with persistent enuresis were given anticholinergic medication. Findings on urodynamic studies were similar for both groups. Uninhibited bladder contractions were found in

Intravesical Botox: An Effective Therapy for Refractory Monosymptomatic Nocturnal Enuresis.

AIM: Monosymptomatic nocturnal enuresis (MNE) significantly impacts many children, with substantial patient groups refractory to conventional first-line treatments. Intravesical Botox is used for various refractory functional voiding disorders in children, but its efficacy in MNE has not been widely established. This study aimed to determine the efficacy of intravesical Botox in children with refractory monosymptomatic nocturnal enuresis. METHODS: A retrospective analysis was conducted of 22 children with MNE (median age 11 years; 19 males) treated with intravesical Botox injection over a 6 y

Risperidone-related reversal of primary enuresis: an unusual case report.

Occasional reports have documented treatment-emergent enuresis with medication regimens that include risperidone. In contrast to these reports, we present a 12-year-old girl with mild mental retardation and primary nocturnal enuresis who received risperidone (0.5 mg/day) for the isolated symptom of inappropriate smiling. Surprisingly, in addition to reduction in inappropriate smiling, risperidone also substantially decreased the frequency of enuresis. These benefits with risperidone were confirmed in an on-off-on treatment sequence. To our knowledge, this is the first case in literature of pri

Association between primary nocturnal enuresis and habitual snoring in children.

OBJECTIVES: Nocturnal enuresis has been associated with obstructive sleep-disordered breathing mostly in hospital-referred adults and children. To investigate whether primary nocturnal enuresis is significantly associated with habitual snoring in the general pediatric population irrespective of subjects' age and sex, we studied children attending six randomly selected schools in a city in central Greece. METHODS: A questionnaire regarding the symptoms of sleep-disordered breathing and the presence of nocturnal enuresis (bedwetting for at least one night per week) was answered by the children'

Monosymptomatic nocturnal enuresis is associated with abnormal nocturnal bladder emptying.

PURPOSE: We investigate the nature of enuresis episodes in monosymptomatic nocturnal enuresis using a fluid provocation model. MATERIALS AND METHODS: The study included 18 children 7 to 13 years old with monosymptomatic nocturnal enuresis. Based on basal home recordings patients were subgrouped into those with a normal nocturnal urine output and those with nocturnal polyuria (mean nocturnal urine production on wet nights exceeding 130% of functional bladder capacity, normal functional bladder capacity for age provided). Children were admitted to the hospital for 4 consecutive nights. After an

Bladder capacity and renal concentrating ability in enuresis: pathogenic implications.

PURPOSE: We compared bladder volume and renal concentrating capacity in dry children and 2 distinct groups of children with enuresis to hypothesize about the pathogenesis of various types of enuresis. MATERIALS AND METHODS: A total of 55 dry children and 100 with enuresis underwent an overnight thirst provocation test to assess renal concentrating capacity and completed a 2-day voiding chart to assess functional bladder capacity. The enuretic children were subdivided into 27 desmopressin responders and 73 desmopressin nonresponders before study inclusion. RESULTS: The desmopressin responder

[Enuresis nocturna--neu studies of genetic, pathophysiologic and psychiatric correlations].

Nocturnal enuresis is characterized by a highly complex interaction of somatic and psychiatric factors. A primary monosymptomatic, a symptomatic (with diurnal rnicturition problems) and a secondary form (following a dry interval) can be differentiated. Despite deep sleep with difficult arousal, the sleep architecture itself is normal and enuretic episodes occur in every sleep stage without urodynamic changes. Changes in the circadian AVP-rhythm with nocturnal polyuria have been demonstrated repeatedly. Formal genetically, many families are compatible with an autosomal dominant mode of inherita

Desmopressin acetate and nocturnal enuresis: how much do we know?

OBJECTIVES: Desmopressin acetate (DDAVP) is promoted to treat nocturnal enuresis but indications for its use are unclear. We reviewed all randomized controlled trials to determine (1) short- and long-term efficacy, (2) responders, (3) dose-response curve, (4) side effects, and (5) comparative efficacy with other treatments. METHODS: A Medline search of the English language literature from January 1966 to August 1992, supplemented by contact with the drug companies, yielded 18 articles which were true randomized controlled trials (11 cross-over and 7 parallel studies). RESULTS: The 18 randomi
