Glottis Recognition Software Development Using Artificial Intelligence.

The use of video laryngoscopes has enhanced the visualization of the vocal cords, thereby improving the accessibility of tracheal intubation. Employing artificial intelligence (AI) to recognize images obtained through video laryngoscopy, particularly when marking the epiglottis and vocal cords, may elucidate anatomical structures and enhance anatomical comprehension of anatomy. This study investigates the ability of an AI model to accurately identify the glottis in video laryngoscope images captured from a manikin. Tracheal intubation was conducted on a manikin using a bronchoscope with record

chronic kidney disease.

UNLABELLED: In a traditional epiglottis biopsy surgery, the operator performs the direct laryngoscopy to visualise an operating field. However, in patients with tumour-altered epiglottis, this procedure can be unsuccessful, because of difficult laryngeal exposure (DLE). This term refers to an insufficient visualisation of larynx, according to anatomical and pathological lesions. A CASE REPORT: The aim of paper was to present a video laryngoscopy performed for surgical purpose in patient with tumour-altered epiglottis. Woman 52 year old was admitted to hospital for planned surgical epiglottis

The effect of balloon dilation at the vallecular using videofluoroscopic swallowing study on patient who has a Dysphagia.

Authors have previously experienced the effect of balloon dilation at the vallecular by utilizing the video-fluoroscopic swallowing study (VFSS) and the urethral catheter to physically stretch and spread in the direction of the posterior inferior towards the patients who have claimed for dysphagia symptoms due to epiglottic dysfunction. A 72-year-old male patient has been diagnosed with rectal cancer and have been treated with an ileocolostomy after the intubation. After the removal of tracheal intubation, the patient complained of dysphagia. Foods and drinks could not be transmigrated into th

Radiographic parameters in adult epiglottitis.

A retrospective study was undertaken to define precise radiographic criteria for the diagnosis of epiglottitis in the adult. We reviewed the standard lateral neck films of six patients over the age of 18 with epiglottitis and five with a normal epiglottis. Radiographic anatomy measured included the angle of the valleculae, retropharyngeal soft tissue width at C2, retrotracheal soft tissue width at C6, width of the epiglottis, width of the aryepiglottic folds, and the hypopharyngeal to tracheal air column ratio. The measurement differences were significant between the groups only for the width

A Newly Discovered Tendon Between the Genioglossus Muscle and Epiglottic Cartilage Identified by Histological Observation of the Pre-Epiglottic Space.

Epiglottic retroversion is difficult to explain anatomically. One reason is inadequate structural identification of the ligaments in the submucosal tissue anterior to the epiglottis (pre-epiglottic space, PES). Although studies have shown that tongue root movement plays a role in epiglottic retroversion, few morphological reports have investigated the attachment of the lingual muscles to the epiglottis. This study reconstructed the fiber structure of the PES by comprehensively analyzing fiber alignment in the PES focusing on the hyoepiglottic ligament, which runs between the lingual muscles an

Assessment of upper airway anatomy in awake, sedated and anaesthetised patients using magnetic resonance imaging.

Magnetic Resonance Imaging was used to quantify the effects of 1. sedation and 2. general anaesthesia with a laryngeal mask airway (LMA) in place on the minimum antero-posterior (A-P) diameters of the naso-, oro- and hypopharynx and on the angle of the epiglottis relative to the adjacent posterior pharyngeal wall. Median sagittal T1-weighted images of the pharynx were obtained in 46 patients (16 awake, 14 sedated, 16 under general anaesthesia). In sedated patients, the A-P diameters of the pharynx were less than in awake patients, in particular at the levels of the epiglottis and soft palate.

[Tuberculous forms of epiglottitis].

This paper discusses three observations of atypical forms of tuberculosis epiglottitis. In two patients, clinical manifestations were similar to abscess formation of acute epiglottitis and in one patient productive inflammation of the epiglottis and its ulceration resembled epiglottis carcinoma. In all the patients these were advanced pathologies that developed together with pulmonary tuberculosis. This indicates that ENT doctors must be very careful when treating patients with atypical epiglottis that shows resistance to anti-inflammatory therapy.

Descent of the human larynx: An unrecognized factor in airway distress in babies with cleft palate?

The intranarial epiglottis, a feature of all newborn mammals, allows suckling and breathing to continue almost simultaneously by separating an oral food stream from a nasal airstream. In contrast to other mammals, the human larynx descends in the neck between birth and six months, extending the distance between the caudal aspect of the soft palate and the cephalic tip of the epiglottis. The mechanism of airway protection changes from a pattern in which an upright epiglottis is grasped by an intact palatopharyngeal sphincter to one in which the epiglottis folds down over the laryngeal aditus an

Radiologic localization of the laryngeal mask airway in children.

In the absence of data on the anatomic localization of the cuff of the laryngeal mask airway (LMA) in children, radiologic images were obtained from 50 infants and children (aged 1 month to 15 yr) undergoing diagnostic radiologic procedures during halothane and N2O:O2 anesthesia. In 46 patients, the cuff of the LMA was in the pharynx and covered the laryngeal opening. The upper (proximal) section was adjacent to the base of the tongue at the level of C1 or C2 vertebrae pushing the tongue forward and its lower (distal) end was in the inferior recesses of the hypopharynx at the levels of C4 to T

Otolaryngologic features of Laurence-Moon-Bardet-Biedl syndrome.

The most common otolaryngologic features associated with LMBBS include SNHL, speech and language disorders, and oral and dental abnormalities. Early otolaryngologic, audiologic, speech pathology, and dental evaluation of these individuals is recommended. This is the first reported case of bifid epiglottis, a rare congenital laryngeal anomaly, found in association with LMBBS. Most patients with bifid epiglottis have additional congenital anomalies, most commonly polysyndactyly. Polysyndactyly is a feature of both LMBBS and bifid epiglottis and may be an early hallmark for the presence of other

Change in glottic view during intubation using a KoMAC videolaryngoscope: A retrospective analysis.

Intubation with videolaryngoscopy has become popular in various clinical settings. However, despite the use of a videolaryngoscope, difficult intubation still exists and intubation failure has been reported. This retrospective study assessed the efficacy of the 2 maneuvers in improving the glottic view during videolaryngoscopic intubation. The medical records of patients who underwent videolaryngoscopic intubation and whose glottal images were stored in electronic medical charts were reviewed. The videolaryngoscopic images were divided into 3 categories according to the applied optimization te

The effect of CPAP on the upper airway and ventilatory flow in patients with obstructive sleep apnea.

BACKGROUND: Continuous positive airway pressure (CPAP) is the mainstay of treatment for obstructive sleep apnea (OSA). However, data about its effect on the upper airway, especially the epiglottis, are scarce. The aim of this study was to investigate the changes in upper airway dimensions and inspiratory flow in response to incremental pressure levels. METHODS: This is a secondary analysis of a prospective clinical trial in which patients with moderate to severe OSA underwent drug-induced sleep endoscopy with simultaneous recordings of flow and mask pressure. CPAP was titrated in small increm

Epiglottoplasty technique in endoscopic partial laryngectomy.

The main advantage of endoscopic laser surgery for laryngeal cancer is to allow tumour resection, while limiting functional sequelae, thereby improving the postoperative course. In this type of surgery, the epiglottis is often partially resected, leaving a raw zone without any reconstruction. The surgical technique described here involves endoscopic reconstruction of the epiglottis after partial resection. The sectioned edge of the epiglottis is sutured to the base of the tongue to create a neoepiglottis and to reconstruct the vallecula, thus resembling preoperative anatomy, allowing improveme

Epiglottic laryngoplasty after extended hemilaryngectomy for glottic cancer.

This paper reports the experience in the application of modified epiglottic laryngoplasty in 20 extended hemilaryngectomies for glottic carcinoma. The Kambic-Sedlacek-Tucker (K-S-T) technique using the epiglottis in glottic reconstruction can lead to early decannulation with an excellent airway, but a wide glottis formed after operation will increase the chance of aspiration and poor voice. Sphincteric function preservation of the glottis is a key point to success in partial laryngectomy. In order to prevent aspiration and to improve the quality of voice, the reconstructive technique as modifi
