OBJECTIVE: Neuroimaging studies reveal frontal lobe (FL) contributions to memory encoding. Accordingly, memory impairments are documented in frontal lobe epilepsy (FLE). Still, little is known about the structural or functional correlates of such impairments. Particularly, material specificity of functional changes in cerebral activity during memory encoding in FLE is unclear.
METHODS: We compared 24 FLE patients (15 right-sided) undergoing presurgical evaluation with 30 healthy controls on a memory fMRI-paradigm of learning scenes, faces, and words followed by an out-of-scanner recognition t

Social behaviour of healthy humans and its neural correlates have been extensively studied in social neuroscience and neuroeconomics. Whereas it is well established that several types of epilepsies, such as frontal lobe epilepsy, lead to social cognitive impairments, experimental evidence on how these translate into behavioural symptoms is scarce. Furthermore, it is unclear whether social cognitive or behavioural disturbances have an impact on therapy adherence, which is critical for effective disease management, but generally low in these patients. In order to investigate the relationship bet

Determining the laterality of the seizure onset zone is challenging in frontal lobe epilepsy (FLE) due to the rapid propagation of epileptic discharges to the contralateral hemisphere. There is hemispheric lateralization of autonomic control, and heart rate is modulated by interactions between the sympathetic and parasympathetic nervous systems. Based on this notion, the laterality of seizure foci in FLE might be determined using heart rate variability (HRV) parameters. We explored preictal markers for differentiating the laterality of seizure foci in FLE using HRV parameters. Twelve patients

Personality disorders can influence and, along with cognitive deficits, compromise the quality of life of patients with epilepsy. This study evaluated personality traits and disorders in patients with frontal (FLE) or temporal lobe epilepsy (TLE) using the Millon Clinical Multiaxial Inventory-III with the aim to determine prevalent personality profiles. The results demonstrate the presence of particularly pronounced personality traits and disorders with prevalence of histrionic and obsessive-compulsive personality profiles, respectively, in FLE and TLE. These profiles may be related to differe

Oral automatism (OA) and manual automatism (MA) are common signs during initiation of temporal seizures. However, the precise symptomatogenic zones for OA and MA remains largely unclear. In this study, we presented a case of intractable frontal lobe epilepsy (FLE) in which the patient received intracranial EEG monitoring with subdural electrodes. During electrical stimulation of a grid contact located over the right pars opercularis cortex, OA and contralateral MA were observed unexpectedly without afterdischarges (ADs) or with brief ADs detected in the adjacent contacts. This case suggested t

BACKGROUND: Emotional disturbances have been reported in patients with epilepsy. Although conflicting results emanate from relevant studies, depressive symptoms are seen more often in temporal lobe epilepsy (TLE) whereas, hypomanic/manic symptoms usually accompany frontal lobe epilepsy (FLE); the above psychiatric symptoms are especially seen in refractory epilepsy. However, neocortical TLE and medial TLE are considered as distinct epileptic syndromes, and there is limited literature on comparison of affective traits in medial TLE (MTLE) and FLE.
AIM: In the present study, we sought to invest

OBJECTIVES: To measure the accuracy of anamnestic features collected during clinical history for the diagnosis of nocturnal frontal lobe epilepsy (NFLE).
METHODS: A case-control diagnostic study. Participants included a case group of people with ascertained target disease (NFLE group) and a control group of people with sleep disorders potentially confounding for NFLS (NOT-NFLE group), defined by means of a consensus procedure among experts (panel diagnosis as reference standard). Two major clinical patterns defining the semeiology of the epileptic event (i.e. dystonic, DP, and/or hyperkinetic

PURPOSE: Nocturnal frontal lobe epilepsy (NFLE) may be associated with sleep fragmentation and reduced sleep efficiency. Daytime sleepiness and disturbed sleep quality have been reported in some patients. We investigated the occurrence of daytime sleepiness-related symptoms and subjective sleep quality in patients with NFLE in comparison with matched controls.
METHODS: Patients with NFLE at a single epilepsy center and matched controls randomly selected from the general population self-administered questionnaires on daytime sleepiness-related symptoms and subjective sleep quality [Epworth sle

BACKGROUND: Pediatric frontal lobe epilepsy surgery can achieve seizure freedom, but cognitive outcomes are heterogeneous and poorly predicted by global measures. This pilot study aimed to implement a domain-specific cognitive assessment protocol and perform an exploratory analysis to identify clinical variables that may influence postoperative outcomes, thereby generating hypotheses for future research.
METHODS: This retrospective pilot study included 17 pediatric patients with drug-resistant epilepsy (DRE) who underwent frontal lobe resection. All patients completed the Computerized Cogniti

Herein, we report the first case of mild malformation of cortical development with oligodendroglial hyperplasia and epilepsy (MOGHE) in Bulgaria. It is a newly recognised clinico-pathological entity with medically intractable focal epilepsy in paediatric patients. The patient of interest is a 9-year-old boy who has been suffering from refractory epilepsy since the age of three. Positron emission tomography revealed a consistent hypometabolism with maximum in the orbitofrontal and fronto-opercular cortex, as well as in the adjacent anterior insula and the anterior temporal regions. BACKGROUND: Frontal lobe epilepsy (FLE) is the most common epilepsy syndrome in the pediatric population; however, brain magnetic resonance imaging (MRI) of the children with FLE is frequently normal. We use both cortical thickness and brain volume measurements to report on cortical changes in children with FLE. Our aim was to determine cortical thickness and brain volume changes on 3 Tesla MRI of children with FLE and normal brain magnetic resonance imaging.
METHODS: Twenty-seven children with FLE and 27 healthy controls received brain magnetic resonance imaging. Cortical thickness and regio

Children with frontal lobe epilepsy (FLE) are at elevated risk for deficits in working memory (WM) and attention that contribute to academic and psychosocial difficulties. Computerized cognitive training (CCT) programs such as Cogmed show promise in pediatric populations, but their effects in FLE remain largely unexplored. This single-case experimental study evaluated the impact of CCT on WM and attention in three children (ages 9-11) with FLE. A non-concurrent multiple-baseline design across participants was used, with staggered intervention onset to control for practice and maturational effe

Frontal lobe epilepsy (FLE) is the second most common surgically treated focal epilepsy. Our study aims to compare the clinical characteristics, seizure outcomes, and their predictors between pediatric (<18 years) and adult patients undergoing FLE surgery. Clinical data from consecutive pediatric and adult patients with FLE were retrospectively analyzed and compared at a tertiary hospital. Univariate and multivariate analyses were used to identify the predictors of seizure recurrence. A total of 234 patients with FLE (67 children) were included. Of them, 61.1 % (143/234) achieved Engel I outco

OBJECTIVE: Mild malformation with oligodendroglial hyperplasia (MOGHE) is a recently described clinicopathologic entity, associated with drug-resistant epilepsy and extensive epileptogenic networks. Knowledge is accumulating about particular electroclinical phenotypes, correlations with imaging, and potential prognostic significance for surgical outcomes. The study adds relevant information by documenting the presence of a hyperkinetic frontal lobe seizure phenotype in adolescents and an epileptic encephalopathy phenotype in young children.
METHODS: Five cases were subjected to a structured p
