Long-term follow-up of joint stabilization procedures in the treatment of fixed deformities of feet in leprosy.

This retrospective study of 52 patients, who underwent joint stabilization procedures for static deformities of the feet in leprosy between 1971 and 1985, was undertaken to asses the long-term results of joint stabilization of feet for fixed deformities in leprosy. The main purpose of joint stabilization is to make the feet plantigrade for weight bearing and to make the wearing of footwear possible. Deformities corrected include varus, equinus and equinovarus. Chronic ulceration occurs repeatedly if these deformities are not corrected and leads to inevitable bone destruction and eventual amput

Stump problems in traumatic amputation.

Stump problems in amputations resulting from employment related injuries were investigated in 397 cases in the Chugoku and Shikoku districts of Japan between 1987 and 1991. Ninety-seven patients (24%) had stump problems which interfered the prosthetic fitting. Stump problems of the upper extremity were seen in about 9% (17 amputees), two thirds of which were skin troubles. Stump problems of the lower extremity were seen in about 37% (80 amputees). Certain complaints were associated with specific methods of amputation; abnormal keratosis in Syme's amputation, equinus deformity in Chopart's ampu

Plantar release in the correction of deformities of the foot in childhood.

One hundred and ninety-one feet with residual cavovarus deformities from club foot or poliomyelitis were treated by a plantar release followed by correction with serial cast application. Roentgenographically, there was significant improvement in the adduction of the fore part of the foot and the cavus deformity, but the varus angulation of the hind part of the foot did not improve. In children more than six years old with deformities resulting from club foot, the plantar release was particularly effective in alleviating residual cavus deformity. In cavus deformity resulting from poliomyelitis,

Foot deformities in diastrophic dysplasia. An analysis of 102 patients.

The exceptionally high prevalence of diastrophic dysplasia in Finland has enabled us to analyse the foot deformities of 102 patients at their first orthopaedic evaluation and classify 204 feet into five categories. The most common finding (43%) was a foot with tarsal valgus deformity and metatarsus adductus; 37% showed either equinovarus adductus (29%) or equinus (8%) deformities. At the first examination 13% showed metatarsus adductus deformity alone, and 7% were clinically normal. The expression 'club foot', generally used for the foot deformity in diastrophic dysplasia is a misnomer. There

Congenital idiopathic clubfoot.

Clubfoot or talipes equinovarus is a complex deformity characterized by three distinct manifestations. The foot is in an equinus position; the forefoot and heel are in varus; and the entire foot is supinated. While the exact etiology of this problem remains unknown, many advancements have been made in the treatment of clubfeet. The surgical procedure is progressive and tailored according the severity of the deformity. Pain management, thorough neurovascular assessments, and education of the family are essential to caring for these patients.

Lateral malleolus reconstruction for fibular hemimelia as a prevention of foot deformity recurrence following tibial lengthening-A long-term analysis.

PURPOSE: Management of ankle deformity and instability remains a challenge in patients with severe fibular hemimelia (FH). This study aims to evaluate the long-term effectiveness of lateral malleus reconstruction using the fibular anlage in correcting ankle malalignment and preventing the recurrence of deformity following leg lengthening. METHODS: Ten patients (age: 3.3 years, 0.8-7.7) with a severe FH, equino-valgus foot deformity, ankle instability, and tibial bowing underwent tibia and ankle realignment and lateral malleolus reconstruction. Afterward, all patients underwent tibial lengthen

Incidence and type of hindfoot deformities in patients with low-level spina bifida.

In a consecutive series of 174 children with low-level spina bifida, there was hindfoot deformity in 263 of the 348 feet; 86 were in equinus, 108 were in calcaneus, 41 were in valgus, 20 were in varus, and eight had convex pes valgus. Surgery was performed on 222 (64%) feet. The deformities were symmetric in 114 children. Spasticity causing deformity necessitated surgery in only 44 feet. Calcaneus deformity in the foot is considerably more common in patients with L4 lesions, and in these circumstances, muscle imbalance is clearly a major factor. However, many patients with calcaneus deformity

Management of acquired adult dropfoot.

Dropfoot is a catchall term for ankle equinus, equinovarus, and equinovalgus. The deformity can be flexible or rigid and may be associated with other pathology. In the adult, dropfoot may be congenital or acquired. Acquired dropfoot results from weakness of the ankle dorsiflexors, overpull of the plantarflexors, contracture of the soft tissues, bony deformity, or any combination of these factors. Appropriate treatment includes observation, orthotic devices, bracing, tendon transfers, arthrodesis, and neurolysis. The purpose of this paper is to review the pathophysiology and treatment of acquir

Endoscopic gastrocnemius recession for treating equinus in pediatric patients.

UNLABELLED: Gastrocnemius recessions have been performed as open or endoscopic procedures. Most of the literature describes the outcomes of these procedures in children with specific neurologic limitations. We report an alternative approach to endoscopic gastrocnemius recessions in neurologically healthy pediatric and adolescent patients whose gastrocnemius equinus could not be corrected nonoperatively. We prospectively followed 23 patients (16 boys, seven girls) who underwent 40 procedures for equinus deformity (n = 22) or osteoarthritis (n = 1). All patients had been directly referred for su

Correcting pediatric flatfoot with subtalar arthroereisis and gastrocnemius recession: a retrospective study.

BACKGROUND: Flatfoot deformities are common in children and are treated using many conservative and surgical approaches. Subtalar extra-articular arthroereisis, in particular, limits talar motion, spares the subtalar joint, and prevents excessive subtalar joint pronation. Addressing the underlying equinus deformity with gastrocnemius recession is an important factor in optimizing outcomes in patients with flatfoot deformity. METHODS: This study included 20 children, 4 to 17 years old. The patients presented 34 cases of functional flexible flatfoot, and each was treated with gastrocnemius rece

Effects of biofeedback treatment on gait in children with cerebral palsy.

PURPOSE: We evaluated the effectiveness of biofeedback treatment on gait function in children with cerebral palsy. METHOD: Thirty-six children with spastic cerebral palsy and dynamic equinus deformity were included in the study. The biofeedback group consisted of 21 children who each received EMG biofeedback training plus conventional exercise programme. The control group consisted of 15 children who each received conventional exercise programme only. Active range of motion of the ankle joints, muscle tone of plantar flexors, and gait function of the children were evaluated and compared. RES

[The treatment of spastic equinovarus in the adult by transplantation of peroneus brevis and lengthening of the tendo-calcaneus (author's transl)].

A new technique is proposed for the treatment of spastic equinovarus foot. After lengthening the tendo calcaneus, the peroneus brevis tendon is divided at the junction with its muscle belly and the distal end of the tendon is transferred through the interosseous membrane to be sutured to the tendo calcaneus. This technique aims to correct the varus as well as the equinus deformity. It was performed in seven cases with good results. The technique is compared with other methods designed to treat this condition. The method is worthwhile in cases without bony deformity.

Management of Severe Cavovarus and Equinocavovarus Deformities Using Hexapod External Fixation.

Traditional reconstructive surgery for severe equinocavovarus or cavovarus deformities has disadvantages, especially in patients with complex multiaxial deformities or scar tissue, including neurologic and vascular injury after acute correction, soft tissue complications, deep infection, over or undercorrection, and shortening of the foot. Moreover, newer software-guided hexapod multiplanar systems have allowed precise 3 dimensional corrections. Distraction osteogenesis and joint angular correction are the main methods of correction associated with additional limited soft tissue procedures. Th

The Baumann procedure to correct equinus gait in children with diplegic cerebral palsy: long-term results.

Although equinus gait is the most common abnormality in children with spastic cerebral palsy (CP) there is no consistency in recommendations for treatment, and evidence for best practice is lacking. The Baumann procedure allows selective fractional lengthening of the gastrocnemii and soleus muscles but the long-term outcome is not known. We followed a group of 18 children (21 limbs) with diplegic CP for ten years using three-dimensional instrumented gait analysis. The kinematic parameters of the ankle joint improved significantly following this procedure and were maintained until the end of fo

Use of circular external fixation to maintain foot position during free tissue transfer to the foot and ankle.

Limb salvage techniques of traumatized extremities using free-tissue transfer and microsurgical techniques have become standard reconstructive methods. To our knowledge there is no published data on the incidence or likelihood of equinus following free tissue transfer about the ankle, although in our experience we have perceived an unacceptable incidence of equinus following free tissue transfers about the ankle and therefore initiated prophylactic ring fixation across the ankle. Fourteen patients were placed in circular external fixation spanning the ankle at the time of free tissue transfer
