[Diagnosis of paroxysmal nocturnal hemoglobinuria with cytofluorometric of molecules bound to the membrane by glycosylphosphatidylinositol groups].

BACKGROUND: Paroxysmal nocturnal hemoglobinuria (NPH) is a clonal disease in which a deficit in the expression of molecules bound to the cell membrane by glycosyl-phosphatidylinositol (MUGFI) groups has been demonstrated. The MUGFI are widely distributed and among them proteins regulating the action of complement may be found. The development of monoclonal antibodies (MoAb) against MUGFI may allow the introduction of a new diagnostic method in this disease and increase the sensitivity, particularly in transfused individuals. METHODS: The erythrocytic and leukocytic phenotype of 14 patients wi

Variability in the Deformability of Red Blood Cells: Application to Treating Premature Newborns with Blood Transfusion.

Blood units are routinely collected from adult donors and stored as packed red blood cells (PRBC). The quality of PRBC, including their deformability, decreases during storage. Since PRBC transfusion has been reported to promote circulatory issues in premature neonates (PNs), they typically receive freshly stored units. To test the hypothesis that freshly stored PRBCs can provide red blood cells (RBCs) with appropriate deformability for PN recipients, we compared the deformability of PRBCs transfused to PNs with that of cord blood RBCs (CRBCs), which are known to have deformability equivalent

Siblings with severe pyruvate kinase deficiency and a complex genotype.

Siblings presented as neonates with severe jaundice and transfusion-dependent hemolytic anemia. Next-generation sequencing revealed both to have three heterozygous mutations in the gene encoding erythrocyte pyruvate kinase (PKLR), plus a heterozygous splice mutation in the beta-spectrin gene (SPTB). In addition, both have a different 5th mutation in a gene encoding other erythrocyte membrane proteins. The asymptomatic parents and all three asymptomatic siblings have different sets of these mutations. © 2016 Wiley Periodicals, Inc.

Host and donor erythrocyte repopulation patterns after allogeneic bone marrow transplantation analysed with antibody-coated fluorescent microspheres.

Analysis of erythrocyte populations, using red blood cell antigen differences between host and donor as marker, was performed with a sensitive fluorescent microsphere assay to monitor marrow engraftment and mixed red cell chimaerism after allogeneic bone marrow transplantation (BMT). An adapted transfusion policy, using marker negative erythrocyte transfusions, was required for this analysis. In all patients the marrow graft was depleted of lymphocytes by counterflow centrifugation. Thirty-seven patients were evaluable for donor repopulation at one or more points in the first 6 months after BM

Toxic effects of drugs on erythrocytes.

The erythrocyte abnormality most often associated with the toxic effects of commonly used drugs is premature destruction. The mechanisms of erythrocyte destruction include: denaturation of unstable hemoglobins, oxidation of sulfhydril groups in hemoglobin and the erythrocyte membrane in the presence of glucose-6-phosphate dehydrogenase deficiency, direct effects on enzymes, cholesterol or phospholipids of the erythrocyte membrane, and various autoimmune reactions. Therapy includes stopping the drug and transfusions when anemia is severe. Splenectomy and steroids are rarely needed. A careful me

Post-mortem analyses in two cases of transfusion-related acute lung injury (TRALI) and literature review.

TRALI (Transfusion-related acute lung injury) is a life-threatening event occurring within six hours post-transfusion, characterized by non-cardiogenic pulmonary edema and severe respiratory distress. It holds particular importance in forensic and medicolegal contexts due to its rapid onset and clinical severity. This study describes two autopsy-confirmed TRALI cases: an 89-year-old man (Case 1) with leukemia who developed acute respiratory symptoms during a home transfusion, and a 63-year-old man (Case 2) who suffered bronchospasm and arrest after receiving erythrocyte units during orthopedic

Red blood cell inventory management: Insights from transfusion laboratory technologists in British Columbia, Canada.

BACKGROUND AND OBJECTIVES: There is concern about sustaining the O negative blood supply, especially in areas with many rural/remote hospitals like British Columbia. Red blood cells are perishable, making inventory management challenging. Demand must be met without wasting this precious resource. Inventory management challenges stem from data scarcity and human factors. Transfusion medicine technologists, who manage inventory daily, are key to understanding the human factors in inventory management. We conducted a qualitative study to understand technologists' inventory management perspectives

Clinical practice for outpatients that are chronically red cell dependent: A survey in the Netherlands.

BACKGROUND AND OBJECTIVES: Limited data are available to guide physicians on how to determine the red blood cell (RBC) transfusion regimen in chronically transfusion-dependent patients. The lack of clarity on thresholds and targets to be used for transfusion could easily result in either under or over transfusion in these patients. The aim of our survey is to investigate (1) transfusion thresholds; (2) number of RBC units given per transfusion episode; (3) interval between transfusions and (4) patient factors, like decreased cardiac function modulating the former. MATERIALS AND METHODS: We se

Effects of increasing blood hemoglobin levels on systemic hemodynamics of acutely anemic cirrhotic patients.

BACKGROUND/AIMS: In experimental portal hypertension, blood hemoglobin levels have been shown to influence the hyperdynamic circulatory state. The aim of this study was to assess the hemodynamic effects of increasing hemoglobin concentration in human portal hypertension. METHODS: Sixteen cirrhotic patients recovering from a variceal bleeding episode were randomly assigned to receive two units of packed red cells or 500 ml of a protein solution. Systemic and portal hemodynamics, and rheological and hormonal parameters were measured at baseline and after expansion. RESULTS: Both groups were si

A rose is a rose is a rose, or not.

In this issue of Blood, Roubinian et al provide important evidence to confirm, and refute, a long-standing maxim in clinical medicine that a 1-unit transfusion of red blood cells (RBCs) should yield a posttransfusion hemoglobin increment of 1 g/dL. Although true, in general, this rule was not always accurate, and deviations could be misleading. They evaluated many single-unit transfusion outcomes in stable adult patients by mining electronic health records (EHRs) and linked blood donor data. This approach was not only pragmatic, but it also points the way to future studies.

Ensuring Transfusion Safety: Screening Blood Donors for Human Parvovirus B19.

Ensuring the safety of blood and blood products is a vital aspect of healthcare. The potential for transmission of pathogens through blood and blood products makes transfusion safety a significant concern. Despite advancements in testing methodologies, donated blood products still pose a risk for infection transmission. Human parvovirus B19 (B19V) is a small, single-stranded, non-enveloped DNA virus transmissible parenterally by blood transfusion. B19V causes a wide range of clinical manifestations, which is generally harmless in healthy individuals. B19V infection may cause severe complicatio

Can Red Blood Cell and Platelet Transfusions Have a Pathogenic Role in Bronchopulmonary Dysplasia?

OBJECTIVE: To evaluate whether transfusions in infants born preterm contribute to the pathogenesis of bronchopulmonary dysplasia (BPD). STUDY DESIGN: We conducted a multihospital, retrospective study seeking associations between red blood cell or platelet transfusions and BPD. We tabulated all transfusions administered from January 2018 through December 2022 to infants born ≤29 weeks or <1000 g until 36 weeks postmenstrual age and compared those with BPD grade. We performed a sensitivity analysis to assess the possibility of a causal relationship. We then determined whether each transfusion w

Is the erythropoietin-hematocrit feedback control operative in chronic renal failure?

Transfusion studies in 23 chronic uremic patients with low hematocrit (HCT) values showed that (1) the pretransfusional immunodetectable (id) serum erythropoietin (EP) levels were within the normal range for healthy people; (2) the posttransfusional idEP levels remained unchanged; (3) 3 patients with analgesic nephropathy and high pretransfusional bioassayed (bio) EP levels had markedly suppressed posttransfusional bioEP, 6 patients with other kidney diseases had undetectable bioEP levels; (4) the raised pretransfusional reticulocyte (RETI) counts were markedly reduced by transfusions in all p

[Intrauterine blood transfusions in the treatment of severe serologic conflict].

Efficacy of the intraperitoneal and intravenous blood transfusions guided ultrasonographically was evaluated in severe cases of fetal hemolytic disease due to Rh conflict. It was shown, that survival rate of fetuses without generalized edema is two-fold higher in the group treated with intraperitoneal blood transfusions in comparison with the control group. It was found, that the proportion of erythrocytes containing HbA in umbilical blood of newborn is related to the number of intraperitoneal transfusions. One has to underline the failure of such a treatment in features with generalized edema
