Ethically justified, clinically comprehensive guidelines for percutaneous endoscopic gastrostomy tube placement.

Guidelines for the placement of percutaneous endoscopic gastrostomy (PEG) tubes are not available. We developed a decision-making algorithm by integrating the medical and ethical dimensions of the decision. According to our algorithm, physicians should not offer PEG tubes to patients with anorexia-cachexia syndromes. For patients with permanent vegetative states, physicians should offer and recommend against the procedure. For patients who have dysphagia without other deficits in quality of life, physicians should offer and recommend the procedure. For the the remaining patients who have dysph

Clinical, social and ethical aspects of HIV-1 infections in an Arab Gulf State.

Clinical, social and ethical aspects of HIV-1 infection as occurring in the Arab Gulf State of Qatar are presented. Up until November 1989, 50 patients were reported with HIV infection. In more than 75% of cases, the disease was acquired via transfusion of imported blood; 52% have developed AIDS and 65% of these have died. In response to the problem, the Ministry of Health has established a National AIDS Committee whose major function has been to educate both the medical profession and lay public about the disease and on ways to prevent its spread. Furthermore, the Committee has also taken on

Commentary on Patient Participation in Clinical Ethics Support Deliberation: Perspectives from Parents and Clinicians in a Children's Hospital.

AbstractWhether and how patients should participate in clinical ethics discussions is an important contemporary debate. Murdoch recently reported clinicians' and ethics service members' views from England and the United States on patient participation in ethics consultation. In response to the call for further understanding, we report parents' and clinicians' experiences of active parental and child involvement in ethics case reviews (ECRs), including the then-novel use of videoconference ECRs during the pandemic in a UK tertiary children's hospital. Between January 2018 and May 2023, parents

The Unbefriended Patient, Their Professional Guardians, and Clinical Liaison Psychiatry: The Challenging Ethics of Changing Goals of Care.

AbstractUnbefriended patients are those with decisional impairments who lack family or friends to serve as healthcare surrogates. When such patients cannot make decisions, the court typically appoints a professional guardian to make choices aligned with the patient's values and preferences. However, this case report illustrates ethical challenges that can arise when professional guardians disregard the patient's authentic wishes. In this case study, the 38-year-old unbefriended African American male patient expressed fears about traumatic resuscitation efforts and ultimately desired de-escalat

Consent, Refusal, and Waivers in Patient-Centered Dysphagia Care: Using Law, Ethics, and Evidence to Guide Clinical Practice.

PURPOSE: When patients refuse medical or rehabilitation procedures, waivers of liability have been used to bar future lawsuits. The purpose of this tutorial is to review the myriad issues surrounding consent, refusal, and waivers. The larger goal is to invigorate clinical practice by providing clinicians with knowledge of ethics and law. This tutorial is for educational purposes only and does not constitute legal advice. METHOD: The authors use a hypothetical case of a "noncompliant" individual under the care of an interdisciplinary neurorehabilitation team to illuminate the ethical and legal

Current issues in medically assisted reproduction and genetics in Europe: research, clinical practice, ethics, legal issues and policy.

STUDY QUESTION: How has the interface between genetics and assisted reproduction technology (ART) evolved since 2005? SUMMARY ANSWER: The interface between ART and genetics has become more entwined as we increase our understanding about the genetics of infertility and we are able to perform more comprehensive genetic testing. WHAT IS KNOWN ALREADY: In March 2005, a group of experts from the European Society of Human Genetics and European Society of Human Reproduction and Embryology met to discuss the interface between genetics and ART and published an extended background paper, recommendatio

Ethics of medical care and clinical research: a qualitative study of principal investigators in biomedical HIV prevention research.

In clinical research there is a tension between the role of a doctor, who must serve the best interests of the patient, and the role of the researcher, who must produce knowledge that may not have any immediate benefits for the research participant. This tension is exacerbated in HIV research in low and middle income countries, which frequently uncovers comorbidities other than the condition under study. Some bioethicists argue that as the goals of medicine and those of research are distinct, it is a mistake for researchers to assume therapeutic responsibilities while engaging in research. Oth

Ethics and informed consent in clinical trial integrity: An international, multi-stakeholder consensus statement by Cairo Consensus Group.

Recent studies have revealed issues with ethical practice and informed consent in randomized clinical trials (RCTs). We aimed to develop an international, multi-stakeholder consensus statement on human research ethics committee approval and informed participant consent for RCTs. The consensus process encompassed a systematic scoping review, modified two-round Delphi survey, and final consensus meeting of a multi-stakeholder group, including trialists, ethics committee members, methodologists, patient representatives, journal editors, and other experts on RCTs. Delphi consensus deployed the Ave

Balancing ethics and clinical need: The Japanese experience with preimplantation genetic testing for monogenic disease regulation and implementation.

Preimplantation genetic testing (PGT) has emerged as an important extension of assisted reproductive technologies. Alongside these technological advances, PGT has also generated substantial ethical and social debate, particularly regarding the sanctity of human life. Ethical perspectives are deeply context-dependent and vary significantly across different backgrounds. The unique historical and social context of Japan necessitated careful ethical discussions as PGT was introduced. The Japan Society of Obstetrics and Gynecology (JSOG) provided official guidance under which PGT was implemented. W

Live-case demonstrations in pediatric urology: Ethics, patient safety, and clinical outcomes from an 8-year institutional experience.

PURPOSE: Live case demonstrations have become a common occurrence at surgical meetings around the world. These demonstrations are meant to serve as an educational medium for teaching techniques, promote discussion, improve interventions and outcomes. Despite the valuable educational benefits, many authors still question the ethics of this approach. We present our 8-year experience in live surgery, discuss the ethical issues, and provide recommendations. MATERIALS AND METHODS: We reviewed records of patients who underwent live robotic surgery during broadcasting events. Procedures performed we

[The clinical ethics committee (CEC) in the area of conflict between hospital certification, moral pragmatics and scientific approach].

During the last decade numerous consultative bodies for bioethical and medical ethical issues have been established. In this study we will introduce the clinical ethics committee (CEC), which can be mainly brought into action for three purposes: discussing moral problems in a hospital's everyday work, developing guidelines for the clinic, and giving further education to the hospital's staff. Starting with the denominational hospitals at the end of the 1990s, CECs have been established in the meantime at a large number of German clinics, often in an interrelation with hospital certification. Patient Advocates for Clinical Research (PACER): A Step Toward Ethical, Relevant, and Truly Participatory Clinical Research in India.

Background Clinical research presents a promising path for improving healthcare in contemporary India. Yet, researchers identify gaps in trust, awareness, as well as misconceptions about being a '"guinea pig." We proposed building the capacity of training patient advocacy groups (PAGs) in patient-centered clinical research and through them creating aware patients as research partners. Methodology Patient Advocates for Clinical Research (PACER) is a tiered program to share information and education about clinical research with PAGs. Tier one is a self-paced online learning course, followed by w

Ethical Dilemmas in Predictive Testing for Monozygotic Twins With Hereditary ALS: A Clinical Case Report.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder typically presenting in midlife and leading to death within 2-5 years of onset. Pathogenic SOD1 variants, inherited in an autosomal dominant manner, account for approximately 20% of hereditary ALS cases. Predictive genetic testing in SOD1-related ALS presents unique ethical challenges, particularly in monozygotic twins who share identical genetic information but hold differing views about testing. We describe an 18-year-old monozygotic twin who requested predictive testing for the familial SOD1 pathogenic variant i

Medical assistance in dying and palliative care in Canada: clinical and ethical considerations in an evolving landscape.

Medical Assistance in Dying and palliative care represent distinct yet intersecting approaches to addressing suffering at the end of life. Their intersection has generated ethical and clinical debates regarding medical responsibility and patient autonomy at the end-of-life. This review examines this intersection by synthesizing empirical research, clinical literature and health policy. Requests for hastened death are often linked with psychological distress, social vulnerability, and unmet psychosocial needs rather than physical pain alone. Evidence shows that palliative care enhances quality

Ethical and social implications of implementing polygenic embryo screening into clinical care: A scoping review.

PURPOSE: Polygenic embryo screening (PES), or preimplantation genetic testing for polygenic conditions/traits, is an emerging application of polygenic risk scores within in vitro fertilization. Marketed commercially to prospective parents with limited professional guidelines, PES raises ethical and social concerns. To date, no scoping review has synthesized the ethical landscape of PES. Our study addresses this gap by mapping ethical and sociocultural issues across academic literature. METHODS: Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, in Septemb

Retinal Embolization after Carotid Artery Stenting in a Monocular Patient: Clinical and Ethical Implications.

Severe carotid artery narrowing is a well-recognized cause of ischemic stroke, and carotid artery stenting (CAS) is frequently used to restore blood flow and prevent recurrence. Although embolic protection devices are designed to reduce the risk of distal embolization, ocular complications can still occur. When treating patients with vision in only one eye, any compromise in ocular circulation can have devastating consequences. A 63-year-old male presented with an acute ischemic stroke caused by severe narrowing of the Right internal carotid artery. He underwent CAS. Shortly after the procedur

Off-label use of antiretroviral drugs: ethical and legal implications for clinical decision-making.

The use of authorized medicinal products under conditions other than those approved, commonly referred to as off-label use, is an increasingly frequent practice in health care. It is particularly relevant in the therapeutic management of human immunodeficiency virus infection, where treatment innovation often outpaces regulatory updates. In the absence of specific protocols to guide the appropriateness of off-label prescribing, this narrative review analyses, from an ethical and legal perspective, the conditions under which off-label use may be considered an appropriate therapeutic option. "The significance of clinical foetal autopsy for reproductive health care: an ethical analysis in the German context".

The clinical autopsy of foetuses, stillborn babies, or neonates and the examination of the placenta help to inform the mother's decision-making and her medical care for subsequent pregnancies. Indeed, these post-mortem examinations can identify unexpected congenital malformations or the cause of repeated miscarriage or stillbirth. However, the use of clinical pathology for diagnostic purposes in the context of family planning for bereaved parents with an unfulfilled desire to have a child, and IVF couples has received little attention to date. This article applies Beauchamp and Childress' bioe

Infective Endocarditis in an Intravenous Drug User: Clinical Challenges, Ethical Dilemmas, and Multidisciplinary Management.

Infective endocarditis (IE), an infection of the endocardial surface of the heart, is a severe condition frequently seen in intravenous drug users (IVDU), primarily driven by Staphylococcus aureus. This case study examines the intricate clinical, ethical, and multidisciplinary challenges in managing a 48-year-old female with a history of IVDU, who presented with IE and multisystem complications. Initial evaluation revealed systemic septicemia with disseminated intravascular coagulation, septic emboli, and significant tricuspid valve involvement.
