Topical review on the abuse and misuse potential of tramadol and tilidine in Germany.

BACKGROUND: Tramadol and tilidine (in combination with naloxone) are used as weak opioid analgesics in Germany. Tramadol is not scheduled in the German Narcotic Drugs Act. Tilidine is scheduled, whereas Tilidine in fixed combinations with naloxone is exempt from some of the provisions of the Narcotic Drugs Act. Recent reports on misuse of both substances led to an evaluation of their potential for misuse, abuse, and dependency by the expert advisory committee established by the German Federal Government, resident at the Federal Institute for Drugs and Medical Devices. METHODS: A subcommittee

Are We on the Same Page? Defining Consensus for Pediatric Open Fracture Management Among Level 1 Pediatric Trauma Centers.

BACKGROUND: Open fractures require urgent management, including timely prophylactic antibiotics, to minimize infection and morbidity. Most pediatric treatment protocols are extrapolated from adult data, resulting in practice variation at pediatric centers regarding antibiotic timing and selection. In this study, we aimed to identify consensus regarding open fracture management protocols at Level-1 Pediatric Trauma Centers across the United States. METHODS: A survey was administered to 17 institutions participating in "Children's Orthopaedic Trauma and Infection Consortium for Evidence-Based S

Practical and clinical significance of the pharmacist-led antimicrobial stewardship.

Antimicrobial resistance (AMR) undermines clinical care. Thus, ensuring appropriate antimicrobial therapy is crucial in global and national AMR countermeasures. This narrative review aimed to summarize pharmacist-led antimicrobial stewardship interventions in Japan and discuss their practical and clinical significance across inpatient, outpatient, and community settings. In inpatient care, antimicrobial stewardship programs (ASPs), usually embedded within antimicrobial stewardship teams and supported by national policies and reimbursement incentives, have consistently reduced broad-spectrum an

Appropriateness of surgical antibiotic prophylaxis in Spanish hospitals: a national multicentre cross-sectional study.

BACKGROUND: While surgical antibiotic prophylaxis (SAP) is a cornerstone of preventing surgical site infections, adherence to clinical guidelines determines its effectiveness. In Spain, nationwide data on SAP appropriateness are scarce. This study evaluated prescribing practices across Spanish hospitals to assess alignment with national standards and identify factors associated with inappropriate antibiotic use. METHODS: We conducted a multicentre, cross-sectional, point-prevalence survey (PPS) in hospitals nationwide in Spain. Adult and paediatric patients undergoing elective or urgent surge

In-Person Versus Telemedicine for Medication Adherence in Hepatitis C.

BACKGROUND: Direct-acting antivirals (DAAs) cure more than 95% of hepatitis C virus (HCV) infections, shifting the principal barrier to elimination from drug efficacy to patient engagement and adherence. Telemedicine is generally non-inferior to in-person care for HCV management, but most evidence treats care modalities as mutually exclusive and does not address how their integration in routine practice affects treatment completion. The objective or this work was to evaluate the association between care-modality patterns: in-person only, telemedicine only, or hybrid, and DAA medication adheren

Community Pharmacy Involvement in Cardiovascular Patient Follow-Up: A Conceptual Framework for Integrated Ambulatory Pharmaceutical Care.

Background/Objectives: Cardiovascular disease (CVD) remains the leading cause of morbidity and mortality worldwide, requiring coordinated long-term management of cardiovascular risk factors, complex pharmacotherapy, and continuity of care across healthcare settings. Community pharmacists are well-placed to support ambulatory cardiovascular care; however, their contribution often remains insufficiently integrated into cardiovascular care pathways, limiting continuity, information exchange, and coordinated medication management across community, primary, and specialist care. This study aimed to

Herbal therapies in gastrointestinal and hepatic disorders: An evidence-based clinical review.

The gastrointestinal tract (GIT) and the liver constitute the major organs of the human body. Indeed, the very survival of the human body depends on their proper functioning. Because the GIT is a huge and complex organ system, the maintenance of proper GIT and liver health is an arduous task. GIT disturbances such as diarrhea, stomach ache, flatulence, constipation, nausea, and vomiting are very common, and they contribute to a significant burden on the healthcare system. Pharmacies are full of over-the-counter pharmacological drugs to alleviate its common conditions. However, these drugs do n

Evaluation of Loma Linda University Health clinicians' beliefs, knowledge and experiences regarding pain management.

BACKGROUND: Effective pain management and safe opioid prescribing remain major challenges in healthcare delivery. Prior research demonstrates that clinicians often manage chronic pain with little or no formal training in pain management, contributing to low confidence and apprehension when prescribing opioids. Surveys also show that insufficient knowledge and inadequate preparation are among the most common barriers to adopting evidence-based opioid-prescribing practices. This study examined Clinicians' beliefs, knowledge, and experiences regarding pain management and opioid prescribing at a l

Preoperative Cryoneurolysis in Total Knee Arthroplasty: Current Concepts in Pain Control and Opioid Stewardship.

Total knee arthroplasty (TKA) is increasingly common, yet postoperative pain remains a major driver of delayed recovery and opioid exposure. Cryoneurolysis is a preoperative, non-opioid adjunct that temporarily ablates peripheral sensory nerves to reduce pain signaling. This narrative review summarizes the mechanism, clinical evidence, and practical implementation of cryoneurolysis for perioperative pain control and opioid reduction after TKA. Published clinical studies evaluating preoperative cryoneurolysis in patients undergoing TKA or with end-stage knee osteoarthritis in perioperative path

A new evaluation methodology study - Integrating 'ancient literature - clinical research - expert consensus' firstly proposes eight elements for taking Chinese medicine decoctions.

BACKGROUND: The scientific and rational use of Chinese medicine decoctions can improve their compliance, safety and efficacy. However, there is a lack of national-level best practice guidelines on the management of Chinese medicine decoctions, particularly in the rational use of them. We aim to establish a comprehensive and standardised reference standard comprising key elements of Chinese medicine decoctions. METHODS: This study was conducted using the method of "Ancient Literature - Clinical Research - Expert Consensus". (1) Ancient literature analysis: Systematically analysed 1,019 records
