Neurologic sequelae following transcatheter embolization to control massive perineal hemorrhage.

A case of exsanguinating perineal hemorrhage resulting from rupture of a pseudoaneurysm of the right internal pudendal artery is presented. Transcatheter embolization of Gelfoam and wire coils proved to be life saving, but subsequent neurologic sequelae developed, including loss of sensation over the right side of the scrotum, penis, and medial thigh. Urinary incontinence developed as well. Fortunately, the paresthesias were temporary.

Results with abdominal vascular trauma in the modern era.

This is a report of a 10-year experience (1989-1998) with 300 consecutive patients found to have an injury to a named abdominal vessel at the time of an exploratory laparotomy for trauma. An abdominal gunshot wound was the mechanism of injury in 78 per cent of patients, and injury to more than one named abdominal vessel was present in 42 per cent. The abdominal aorta, inferior vena cava, and external iliac artery and vein were the most commonly injured vessels. When management for the five most commonly injured arteries was grouped, exsanguination before attempts at repair occurred in 11 to 15

Emergency preservation and resuscitation for cardiac arrest from trauma.

Patients who suffer a cardiac arrest from trauma rarely survive. Surgical control of hemorrhage cannot be obtained in time to prevent irreversible organ damage. Emergency preservation and resuscitation (EPR) was developed to utilize hypothermia to buy time to achieve hemostasis and allow delayed resuscitation. Large animal studies have demonstrated that cooling to tympanic membrane temperature 10 °C during exsanguination cardiac arrest can allow up to 2 h of circulatory arrest and repair of simulated injuries with normal neurologic recovery. The Emergency Preservation and Resuscitation for Car

The literature increasingly supports expectant (conservative) management of renal trauma--a systematic review.

BACKGROUND: The perfect degree of operative intervention in renal trauma is unknown. However, expectant management for most blunt renal trauma is the standard of care, and nonoperative management is increasingly accepted for stab wounds. The best treatment of gunshot wounds and vascular injuries is still unclear; however, recent data indicates that a trial of nonoperative therapy may be warranted in those not exsanguinating from the kidney. Conservative management has many benefits, the greatest of which is decreasing the rate of iatrogenic nephrectomy. We have reviewed the world's literature

Common iliac artery pseudoaneurysm following inguinal hernia repair--a case report and literature review.

The authors report a patient presented with a ruptured infected iliac artery pseudoaneurysm 2 weeks after ipsilateral inguinal hernia repair. Pseudoaneurysms that occur because of infection develop rapidly and mandate ligation of the affected artery and extraanatomic bypass. Noninfected pseudoaneurysms are usually discovered incidentally and may be managed with either endovascular or standard surgical techniques. A percutaneously placed aortic occlusion balloon may prevent exsanguination, when used as an adjunct to surgical repair in cases of ruptured pseudoaneurysm.

Emericella nidulans as an agent of guttural pouch mycosis in a horse.

A case is reported in a 9-year-old Anglo-Arab horse with guttural pouch mycosis caused by Emericella nidulans. Acute death occurred by exsanguination following erosion of the external carotid artery. Histopathological examination of the mycotic plaque demonstrated septate hyphae, conidial heads, hülle cells and mature cleistothecia containing characteristic ascospores. Specific identification was confirmed by culture. In accordance with previous reports, Emericella nidulans should be considered as the major agent of guttural pouch mycosis in Equidae.

Use of Blount's epiphyseal staples to control massive iliac venous haemorrhage.

Significant injuries to major veins are life-threatening events and may lead to patient exsanguination. Treatment usually consists of direct suture repair. A case in which a serious iliac venous haemorrhage was not able to be controlled with the usual surgical procedures is reported. A desperate attempt to achieve haemostasis using orthopaedic staples was successful. The simplicity of this technique makes it a useful addition to the surgeon's armamentarium when dealing with life-threatening venous haemorrhage.

The Sengstaken-Blakemore tube: uses and abuses.

Due to its complications including oesophageal and gastric ulceration and perforation, the Sengstaken-Blakemore tube is used far less commonly in this new millennium where endoscopic intervention is available. We discuss in a 53-year-old Indian woman an unusual life-saving use of the Sengstaken-Blakemore tube in preventing fatal exsanguination from an aortoesophageal fistula, as well as rare but devastating consequences of the insertion and residence of the Sengstaken-Blakemore tube, including acute airway obstruction and bronchoesophageal fistula.

Penetrating cardiac injury--to close the pericardium or not? A case report.

Many trauma surgeons who treat penetrating cardiac injuries do not routinely suture the pericardium closed after repairing the heart. The patient reported here died from exsanguination during attempts to mobilise the heart which had adhered to the sternum. With the increasing demand for coronary artery surgery and the high incidence of carcinoma of the oesophagus in South Africa, both of which require access to the mediastinum, we recommend that in the trauma setting, the pericardium be closed to facilitate future surgery if required.

Narrow elastic disposable tourniquet (Hemaclear®) vs. traditional wide pneumatic tourniquet for creation or revision of hemodialysis angioaccesses.

PURPOSE: To choose the best arterial tourniquet for angioaccess surgery. METHODS: Preventive hemostasis with an arterial tourniquet prevents bleeding and provides better visualization. The surgeon may currently use a traditional wide nonsterile inflatable pneumatic cuff after exsanguination with an Esmarch bandage or a disposable sterile narrow elastic silicone ring (HemaClear®), available in different sizes according to the patient's limb circumference and blood pressure. RESULTS: The latter is easily rolled up the upper limb after surgical draping, to achieve exsanguination and occlusion o

Religious slaughter: evaluation of current practices in selected countries.

As part of the project "Religious slaughter (DIALREL): improving knowledge and expertise through dialogue and debate on issues of welfare, legislation and socio-economic aspects", this paper discusses an evaluation of current practices during Halal and Shechita slaughter in cattle, sheep, goats and poultry. During religious slaughter, animals are killed with and without stunning by a transverse incision across the neck that is cutting the skin, muscles (brachiocephalic, sternocephalic, sternohyoid, and sternothyroid), trachea, esophagus, carotid arteries, jugular veins and the major, superfici

Traumatic injuries to the portal vein: case study.

BACKGROUND: Injuries to the portal vein are rare but devastating. Contemporary studies have debated the most effective management for this injury. The purpose of this case study was to provide an update on portal vein injury and add information regarding its management. METHODS: A retrospective review investigated the 10-year experience with portal vein injury in a level 1 trauma center. RESULTS: Of the 18,900 trauma patients (0.08%) evaluated during a 10-year period, 15 sustained injuries to the portal vein. All the injuries resulted from penetrating trauma, and the overall survival rate wa

Penetrating iliac vascular injuries: recent experience with 233 consecutive patients.

During a recent 11-year period, 233 consecutive patients with 358 penetrating iliac vascular injuries were treated at our institution. Injuries of the common and external iliac arteries were most often repaired with lateral suture (31%) although several other techniques were also employed. Lateral suture and ligation were used with nearly equal frequency in the management of venous injuries. The hospital mortality rate for the series was 28%, and 56/66 deaths (85%) were due to exsanguination or shock. One patient, initially treated with an end-to-end anastomosis of the iliac artery, died a yea

Aortic Dissection and Cardiac Tamponade in an Elderly Female With Cushing's Syndrome: A Fatal Case.

A 77-year-old female with multiple cardiovascular comorbidities presented with increased work of breathing and was found to have hypotension, bradycardia, and hypoxia. Despite initial stabilization, the patient's condition declined, leading to a sudden cardiac arrest. Postmortem examination revealed the cause of death as aortic dissection, with exsanguination into the pericardium and left pleural space. This case illustrates the value of considering aortic dissection in patients with a history of Cushing's syndrome, even when their presenting symptoms are nonspecific.

Flood Syndrome.

Flood syndrome refers to the exsanguination of ascitic fluid following the spontaneous rupture of an umbilical hernia, and is a rare complication of liver cirrhosis with ascites. In this case report, we describe a 67-year-old patient with Flood syndrome who was initially managed conservatively in a community hospital run by primary care physicians, prior to transfer to a tertiary hospital for specialist surgical review and management. We also performed a literature review of the current treatment modalities to manage this condition.

Peptic-ulcer-induced aortoenteric fistula. Report of a case and review of the literature.

A 71-year-old woman with a remote history of esophagogastric resection for malignancy developed abdominal pain, nausea and vomiting. She died of shock 15 days after admission, during which time clinical investigation failed to reveal recurrent tumor or an ulcer. The autopsy revealed massive exsanguination secondary to a peptic-ulcer-induced aortoenteric fistula (AEF). The ulcer involved the third portion of the duodenum, was chronic, and complicated by growth of fungus. We review the literature on peptic-ulcer-related AEF.

Testing and Evaluation of a Novel Hemostatic Matrix in a Swine Junctional Hemorrhage Model.

INTRODUCTION: In an ongoing effort to improve survival and reduce blood loss from hemorrhagic injuries on the battlefield, new hemostatic dressings continue to be developed. This study aimed to determine the efficacy of a novel silicon dioxide-based hemostatic matrix (HM) and compare it with the current military standard Quikclot Combat Gauze (QCG) utilizing a lethal femoral artery injury model. MATERIALS AND METHODS: The femoral arteries of 20 anesthetized swine were isolated, and an arteriotomy was performed. After a 45 s free bleed, the wound was treated with either HM or QCG (n = 10 per g

Evaluation of hemostatic devices in a randomized porcine model of junctional hemorrhage and 72-hour prolonged field care.

BACKGROUND: Hemorrhage control in prolonged field care (PFC) presents unique challenges that drive the need for enhanced point of injury treatment capabilities to maintain patient stability beyond the Golden Hour. To address this, two hemostatic agents, Combat Gauze (CG) and XSTAT, were evaluated in a porcine model of uncontrolled junctional hemorrhage for speed of deployment and hemostatic efficacy over 72 hours. METHODS: The left subclavian artery and subscapular vein were isolated in anesthetized male Yorkshire swine (70-85 kg) and injured via 50% transection, followed by 30 seconds of hem

Causes of death after blunt chest injury.

During the 10 years from 1968 a total of 1119 patients with chest and associated injuries were admitted to the Royal Newcastle Hospital (RNH); 530 of these needed ICU treatment and fifty-nine died. Common causes of death included irreparable associated injuries to brain and exsanguination due to major vessel injury. However, the commonest cause encountered was refractory respiratory failure due to pulmonary sepsis. This condition was associated with hypovolaemic shock, intra-abdominal injuries, failure of conservative treatment and injuries in the older age groups.
