Clinical characteristics and outcomes of follicular lymphoma patients with extranodal involvement: analysis of a series of 1090 cases in China.

BACKGROUND: Follicular lymphoma (FL) is the most common indolent B cell lymphoma in the world. The clinical features of extranodal involvement in FL were never extensively described. METHODS: We enrolled 1090 patients diagnosed as newly diagnosed FL at ten medical institutions in China from 2000 to 2020 and conducted this analysis and retrospectively explored clinical characteristics and outcomes of FL patients with extranodal involvement. RESULTS: 400 (36.7%) patients with newly diagnosed FL had no extranodal involvement, 388 (35.6%) patients had one site of extranodal involvement, and 302

Phase II activity trial of high-dose radiation and chemosensitization in patients with macrometastatic lymph node spread after sentinel node biopsy in vulvar cancer: GROningen INternational Study on Sentinel nodes in Vulvar cancer III (GROINSS-V III/NRG-GY024).

BACKGROUND: Standard treatment of early-stage vulvar cancer is a radical, wide, local excision of the primary tumor and a sentinel lymph node (SLN) procedure for the groins. An inguinofemoral lymphadenectomy is no longer necessary for patients who have a negative SLN or micrometastasis (≤2 mm). When there is macrometastasis (>2 mm) in the SLN, an inguinofemoral lymphadenectomy is indicated; however, this procedure is associated with major morbidity, such as wound healing, lymphoceles, and lymphedema. PRIMARY OBJECTIVE: To investigate the safety of replacing inguinofemoral lymphadenectomy by c

Hodgkin and non-Hodgkin lymphoma of the head and neck: clinical, pathologic, and imaging evaluation.

Lymphomas are subdivided into HL and NHL and are more specifically classified into subtypes of HL or NHL according to the WHO classification. HLs involve the lymph nodes predominantly and only approximately 5% arise in extranodal sites, whereas 30% of NHLs present in extranodal sites. Imaging studies, including CT and MR imaging, cannot distinguish [figure: see text] HL from NHL, and cannot differentiate their various subtypes, necessitating a pathologic diagnosis. Clinical parameters, however, can be helpful in differentiating the two broad categories of lymphomas, and subtypes of lymphomas h

Making routine head and neck pathology Easy: From dissection to diagnosis.

Resections from the mobile tongue, the oropharynx and neck dissections constitute a large proportion of routine head and neck pathology workload. Histologically detected proximity to margins and prognostic factors like depth of invasion, perineural or lymphovascular invasion, or extranodal extension guide adjuvant radiotherapy and/or chemotherapy. This review discusses practical approaches to macroscopic examination of the various types of tongue and oropharyngeal resections and neck dissections. Differential inking and radial sections demonstrate proximity of tumour to margins. The macroscopi

Intra-arterial chemotherapy targeting metastatic cervical lymph nodes in head and neck cancer.

BACKGROUND: Large cervical lymph nodes and the extranodal extension of metastatic lymph nodes are considered poor prognostic factors in head and neck squamous cell carcinoma (HNC). AIMS/OBJECTIVES: The efficacy of intra-arterial chemotherapy (iaCT) targeting lymph node (LN) in HNC was examined. MATERIALS AND METHODS: We performed a retrospective review of 41 patients with laryngeal and hypopharyngeal cancer showing metastatic cervical LN larger than 20 mm treated with iaCT with concurrent radiotherapy. The administration of cisplatin into LN was divided into three groups: no administration (

Sentinel lymph node biopsy is a reliable method for lymph node evaluation in neoadjuvant chemotherapy-treated patients with breast cancer.

Sentinel lymph node (SLN) mapping in patients with breast cancer treated with neoadjuvant chemotherapy has been debated by surgeons as a result of potential compromise of lymphatic drainage. Whether clinicopathologic variables traditionally associated with SLN positivity differ in patients who have been treated with neoadjuvant chemotherapy has not been well studied. Patients diagnosed with breast carcinoma who underwent neoadjuvant chemotherapy, definitive breast surgery, sentinel node biopsy (SNB), and axillary lymph node dissection (ALND) were retrospectively identified over a 75-month peri

Prophylactic pelvic lymph node dissection in patients with penile cancer.

PURPOSE: Pelvic lymph node involvement in penile cancer carries a poor prognosis. Therefore, there is controversy about the curative role of pelvic lymph node dissection. We analyzed the characteristics of tumor positive inguinal regions predictive for pelvic lymph node involvement in patients prophylactically treated with pelvic lymph node dissection. MATERIALS AND METHODS: All chemonaïve consecutive cases treated with prophylactic pelvic lymph node dissection at our institution since 2001 were included in analysis. A generalized estimating equation model was used to predict pelvic node invo

Radiology of the lymphomas.

Lymphomas are a group of diseases that provide the radiologist and clinician with unusual challenges. In most cases, an understanding of the histologic classification and staging is essential to design the imaging workup for the patient. Precise staging is critical, particularly for patients with HD and extranodal NHL. The malignant lymphomas share many similar radiographic features, yet have notable differences. An understanding of the pattern of extension of HD is useful in choosing an imaging modality for both staging and follow-up. Contiguous extension of HD follows lymphatic channels and

Disease Progression in Cutaneous Squamous Cell Carcinoma Patients With Satellitosis and In-transit Metastasis.

BACKGROUND/AIM: Satellitosis/in-transit metastasis (S-ITM) has prognostic value in melanoma and Merkel cell carcinoma, but is not incorporated into cutaneous squamous cell carcinoma (cSCC) staging. PATIENTS AND METHODS: From our IRB-approved registry, patients with high-risk cSCC, including patients with S-ITM, were identified. Univariate (UVA) and multivariate (MVA) analyses were performed to compare disease progression (DP) and overall survival (OS). Cumulative incidence of DP and OS analyses were performed using Fine-Gray and Kaplan-Meier methods, respectively. RESULTS: A total of 18 S-IT

Angioimmunoblastic T-Cell Lymphoma: A Case Report.

Previously believed to be an exaggerated immune response and not a lymphoma, angioimmunoblastic T-cell lymphoma (AITL) is now recognized as a rare variant of peripheral T-cell lymphoma with an aggressive clinical course and poor response to current therapies. There is no standard of care for treatment, but the identification of extranodal involvement is useful for prognostic purposes since the involvement of more than one extranodal site can escalate the patient's risk category on the International Prognostic Index (IPI). Here we present the case of a patient with AITL who initially presented

Predictive significance of anterior and posterior minimal extrathyroidal extension for central lymph node metastasis in cN0 papillary thyroid carcinoma.

The specific role of minimal extrathyroidal extension (mETE)-particularly the differences between anterior mETE (ant-mETE) and posterior mETE (post-mETE)-in relation to Central lymph node metastasis (CLNM) for papillary thyroid carcinoma (PTC) remains uncertain. This study aimed to examine the effects of ant-mETE and post-mETE on CLNM and to construct a preoperative risk-stratification nomogram for CLNM to guide central lymph node dissection (CLND). This retrospective study analyzed 1694 consecutive clinically node-negative (cN0) PTC cases treated between 2017 and 2024. Univariate and multivar

Lymphomas of the head and neck: CT findings at initial presentation.

CT findings were reviewed in 68 patients with untreated head and neck lymphoma. More than half of the patients with either Hodgkin's disease or non-Hodgkin's lymphoma were detected in the earlier stages (stage I or II). Four types of abnormalities were identified with CT: nodal involvement alone (type 1), extranodal involvement alone (type 2), a combination of extranodal and nodal disease (type 3), and multifocal extranodal disease with or without nodal involvement (type 4). In the 18 patients with Hodgkin's disease, a subgroup of mixed cellularity was most common; type 1 was the prevailing CT

Unusual presentation of large B cell lymphoma: a case report and review of literature.

Diffuse large B cell lymphoma (DLBCL) is the largest subtype of non-Hodgkin's lymphomas (NHLs) and is characterized by relatively frequent extranodal presentation. In these cases, the most common extranodal localizations are stomach, CNS, bone, testis and liver. Simultaneous detection of multiple extranodal involvement at presentation is quite uncommon, with the majority of these cases characterized by gastric or intestinal disease localization. Retrospective analysis concerning multifocal extranodal NHLs never pointed out disease features such as those described here. We report a patient with

Analysis of Risk Factors for High-Risk Lymph Node Metastasis in Papillary Thyroid Microcarcinoma.

BACKGROUND: Papillary thyroid microcarcinoma (PTMC) is associated with certain features that carry an increased risk of local recurrence, underscoring the importance of preoperative risk assessment. This study investigated the clinicopathological factors associated with high-risk lymph node metastasis (HRLNM) and patient outcomes. HRLNM is defined as ≥5 metastatic lymph nodes and/or lateral neck metastasis. METHODS: We conducted a retrospective review of 985 patients with PTMC who underwent thyroidectomy at the Kaohsiung Chang Gung Memorial Hospital from 2013 to 2022. RESULTS: Among the 985

Multicentric extranodal Rosai Dorfman disease--a cytological diagnosis, with histological corroboration.

BACKGROUND: Rosai Dorfman disease (RDD) typically presents with massive bilateral cervical lymphadenopathy, a viral-like prodrome, elevated erythrocyte sedimentation rate, and polyclonal hypergammaglobulinemia. Other lymph nodes may be less commonly involved. Extranodal RDD is quite rare, and orbital disease accounts for only 10% of the extranodal sites of involvement. Multicentric disease has also been described, which is usually accompanied by lymphadenopathy either initially or later in the disease course. CASE: We report an extremely rare extranodal multicentric disease in a diabetic pati

Recurrent Extranodal Diffuse Large B-Cell Lymphoma Manifesting as a Cecal Mass Over 10 Years After Remission.

Diffuse large B-cell lymphoma (DLBCL) frequently involves extranodal sites, most commonly the gastrointestinal tract; however, predominant colorectal involvement and very late relapse after long-term remission are uncommon. A 70-year-old woman with a history of DLBCL in complete remission for more than 10 years after 6 cycles of R-CHOP presented with progressive weight loss, early satiety, lower abdominal pain, and new-onset urinary urgency and frequency. Cross-sectional imaging revealed a large cecal mass with retroperitoneal extension causing right-sided hydronephrosis. Colonoscopy demonstra

Distribution of adverse pathological features and prognosis across tongue, buccal, gum, and other oral cancer subsites: A nationwide study.

BACKGROUND: Staging of oral squamous cell carcinoma (OSCC) does not account for anatomical subsites. However, these subsites demonstrate considerable pathological heterogeneity and survival differences. Most prior studies have not systematically examined the influence of distinct pathological factors across subsites. Therefore, this study aimed to evaluate the clinical and pathological characteristics, as well as survival outcomes of OSCC across major oral subsites, using a nationwide cancer registry. METHODS: A total of 17,118 patients with surgically treated OSCC diagnosed between 2018 and

Axillary lymph node metastasis: Fine-needle aspiration biopsy sensitivity as a function of node size, percent replacement of lymph node by tumor and tumor deposit size.

BACKGROUND: Fine-needle aspiration (FNA) is commonly used to investigate lymphadenopathy of suspected metastatic origin. While diagnostic accuracy of FNA for lymph node disease is well described, the relationship between node size, percent tumor replacement, and size of metastatic deposit with diagnostic accuracy is less well documented. METHODS: All axillary lymph nodes undergoing ultrasound-guided FNA for suspected breast metastases were correlated with subsequent surgical excision specimens. FNAs were judged as positive or negative for malignancy and the percent of false negative FNAs was
