Citrobacter koseri, an unusual exogenous endoftalmitis.

Citrobacter koseri is a bacillus that causes infrequent endophthalmitis. 6% of cultures in endophthalmitis are Gram -, and as in these, C. koseri is associated with a poor visual prognosis. We present a 65-year-old man who works in an animal laboratory. He went to emergencies with loss of vision in his left eye due to a vitreous hemorrhage. A vitrectomy was performed and 3 days later, endophthalmitis was diagnosed. Vancomycin and Ceftazidime were applied in eye drops and in two intravitreal injections. 24 h later he returned with a lens extrusion. Due to the severity of the condition, an evisc

Intraocular Plasmablastic Lymphoma in a HIV Patient.

Plasmablastic lymphoma (PBL) is a rare B-cell lymphoma occurring mainly in HIV patients. The tumor frequently involves extranodal sites such as the oral cavity, nasal cavity, gastrointestinal tract, skin, and lungs. The neoplastic cells are characterized by a plasmablastic appearance and typical immunophenotype that indicates plasma cell differentiation. Herein, we report a case of intraocular involvement by plasmablastic lymphoma in a HIV patient with a long history of cytomegalovirus retinitis and loss of vision. After an evisceration performed to control the blind, painful eye a diagnosis o

Piggyback Photochromic Contact Lens for Visual Rehabilitation and Photophobia Management in Traumatic Aniridia.

We report herein a case of fitting with a photochromic silicone hydrogel contact lens under a rigid gas-permeable lens (piggyback system) for photophobia and low vision correction after traumatic aniridia and aphakia. A 40-year-old woman was referred to our practice for contact lens fitting in her right eye, which was left aphakic after an open globe injury. She also presented traumatic aniridia in the right eye, and her left eye had been previously eviscerated. A successful fitting was obtained with a photochromic silicone hydrogel (senofilcon A) contact lens, with a Dk/t of 121 × 10-9, under

Arrow injuries to the eye.

Two male siblings aged 10 and 17 years, respectively, presented to our hospital with two days history of being shot in the right eyes with arrows. The patients presented with severe pains, bleeding, swelling, discharge and loss of vision. There was positive history of application of traditional eye medicine and an unsuccessful attempt was made to remove the arrows. Both patients presented with panophthalmitis. X-rays of the orbits and para nasal sinuses indicated the tip of the arrow was lodged to the apex of the orbit in the younger patient, and the arrow vertically traverses the eye/ orbit a

Neuroepithelial adenocarcinoma of the eye.

A 29-year-old male patient with a microphthalmos on the right side since birth was seen in June 1987 in the University Eye Clinic because of a massive exophthalmus of the former microphthalmic eye. Computer tomography revealed a diffuse, infiltrating tumor of the right orbit. The bulbus was not demarcated, and the whole orbita had to be eviscerated. Histologically, we found a solid, undifferentiated tumor with clear signs of malignancy. It consisted of epithelial cells, partly in nest formation, which were keratin positive and vimentin negative, and of more spindle-shaped formations which were

Panophthalmitis complicated with asymptomatic pyogenic liver abscess caused by Klebsiella pneumoniae: A case report.

RATIONALE: Panophthalmitis is a severe, vision-threatening condition that may result from endogenous spread of infection, often presenting with systemic symptoms. Klebsiella pneumoniae (Kp) is a known cause of endogenous panophthalmitis, typically associated with underlying conditions such as diabetes and often linked to pyogenic liver abscesses. However, cases presenting solely with ocular symptoms are exceedingly rare. This report presents a unique case of Kp panophthalmitis with a concurrent asymptomatic liver abscess, emphasizing the importance of systemic evaluation in atypical ocular inf

Autoimmune phenomena of the conjunctiva and cornea. A case report.

A 20-year-old woman had erosions of the corneal and conjunctival epithelium and sterile stromal ulcerations of the cornea necessitating multiple corneal grafts, which were unsuccessful. An immunopathological basis for her disease was suggested by the demonstration of tissue-fixed antibodies and complement in her own conjunctival epithelium by direct immunofluorescence. In addition, circulating antibodies to normal corneal and conjunctival epithelium were demonstrated in the patient's serum by indirect immunofluorescent techniques and the titer of circulating antibodies was found to correlate w
