Eye movements and the search for the essence of schizophrenia.

Clinical Schizophrenia has eluded precise description because of its protean phenotypic manifestations and variable clinical course, a variability that also makes it difficult to discover genetic linkages. Co-familial traits have higher recurrence risk rates than schizophrenia itself and might serve as pointers to the underlying physiology of schizophrenic illness. A dysfunction of smooth pursuit eye movements is one such co-familial trait that occurs in about 40 to 80% of schizophrenic patients and about 25 to 40% of their first degree relatives. The eye movement abnormality appears only when

Approach to Quantify Eye Movements to Augment Stroke Diagnosis With a Non-Calibrated Eye-Tracker.

UNLABELLED: Automated eye-tracking technology could enhance diagnosis for many neurological diseases, including stroke. Current literature focuses on gaze estimation through a form of calibration. However, patients with neuro-ocular abnormalities may have difficulty completing a calibration procedure due to inattention or other neurological deficits. OBJECTIVE: We investigated 1) the need for calibration to measure eye movement symmetry in healthy controls and 2) the potential of eye movement symmetry to distinguish between healthy controls and patients. METHODS: We analyzed fixations, smoot

Narcolepsy and rapid eye movement sleep.

Since the first description of narcolepsy at the end of the 19th Century, great progress has been made. The disease is nowadays distinguished as narcolepsy type 1 and type 2. In the 1960s, the discovery of rapid eye movement sleep at sleep onset led to improved understanding of core sleep-related disease symptoms of the disease (excessive daytime sleepiness with early occurrence of rapid eye movement sleep, sleep-related hallucinations, sleep paralysis, rapid eye movement parasomnia), as possible dysregulation of rapid eye movement sleep, and cataplexy resembling an intrusion of rapid eye move

Eye movement abnormalities in essential tremor.

Essential tremor (ET) is the most prevalent movement disorder, characterized mainly by an action tremor of the arms. Only a few studies published as yet have assessed oculomotor abnormalities in ET and their results are unequivocal. The aim of this study was to assess the oculomotor abnormalities in ET patients compared with the control group and to find the relationship between oculomotor abnormalities and clinical features of ET patients. We studied 50 ET patients and 42 matched by age and gender healthy controls. Saccadometer Advanced (Ober Consulting, Poland) was used to investigate reflex

Eye movement control and its disorders.

We review the supranuclear control centers and pathways leading to individual cranial nerve nuclei in the brainstem. We discuss horizontal and vertical gaze and their abnormalities, and review the cranial nerves which subserve eye movements, III, IV, VI, including their clinical testing. We highlight the sites at which these nerves are clinically affected, which often result in characteristic associated features with neurological localizing value. Differential diagnoses of cranial nerve palsies including Duane and Möbius syndromes are also described. Lastly, we discuss the nature of childhood

Eye movement abnormalities in multiple sclerosis.

Patients with multiple sclerosis commonly describe visual symptoms that result from several eye movement abnormalities that occur from disruption of critical pathways in the brainstem, cerebellum, and cerebral hemispheres. These abnormalities include internuclear ophthalmoplegia, ocular motor palsy, ocular misalignment, pathologic nystagmus, impaired saccades, saccadic intrusions, and impaired pursuit. Detailed knowledge of these problems and their neuroanatomic localization will aid the physician by guiding diagnosis and therapeutic decision making.

Therapeutic considerations for eye movement disorders.

Advances made in understanding the pathophysiology of eye movement disorders have only recently with the publication of the first well-planned studies been translated into better treatment strategies. The following chapter summarizes the pharmacological treatment options for a variety of oculomotor syndromes. Cortisone is useful, for example, for acute vestibular neuritis to improve the restitution of the labyrinthine function. For the widespread benign paroxysmal positioning nystagmus, a series of liberating movements that free the semicircular canal from the causative otoconia is now a well-

Pursuit eye movement deficits in autism.

Oculomotor studies provide a novel strategy for evaluating the functional integrity of multiple brain systems and cognitive processes in autism. The current study compared pursuit eye movements of 60 high-functioning individuals with autism and 94 intelligence quotient, age and gender matched healthy individuals using ramp and oscillating target tasks. Individuals with autism had normal pursuit latency, but reduced closed-loop pursuit gain when tracking both oscillating and ramp targets. This closed-loop deficit was similar for leftward and rightward pursuit, but the difference between individ

Neuro-ophthalmology of eye-movement disorders.

Magnetic resonance and functional brain imaging continue to expand their roles in complementing the clinical examination to localize the causes of eye-movement disorders. A retrospective review from county-based clinical practices emphasized generally benign causes of third, fourth, and sixth cranial nerve palsies in children, but a study from a tertiary referral center found a high incidence of neoplasms causing sixth-nerve palsies in children. A prospective study found that 38% of diabetic patients with third-nerve palsies had anisocoria up to 2.5 mm. An initial attempt to develop an evidenc

Eye movement patterns in hemianopic dyslexia.

Homonymous parafoveal field loss impairs reading at the visual-sensory level. To elucidate the role of parafoveal visual field in reading, reading eye movements were recorded, by means of an infra-red registration technique, in 50 patients with homonymous hemianopia and visual field sparing ranging from 1 degree to 5 degrees; for comparison, a group of 25 normal subjects was studied. The degree of reading impairment in patients was found to depend on the extent of visual field sparing. Patients with right-sided loss of parafoveal visual field were more impaired than patients with left-sided lo

Eye movement abnormalities in rod monochromacy.

Eye movements were studied quantitatively using electro-oculography in seven patients with rod monochromacy. Attention was given to eye movement abnormalities that could help to differentiate rod monochromacy from other forms of congenital nystagmus. Horizontal nystagmus in center gaze had pendular and jerk waveforms with much lower amplitude than that in patients with other forms of congenital nystagmus and visual acuity of 20/200-20/400. Slow build-up of slow component velocity over many seconds and directional asymmetry of optokinetic nystagmus (OKN) were observed during monocular optokinet

[Eye movement disorders in visual agnosia].

By the aid of photooptical methods the authors studied eye movements in 6 patients with disorders of visual gnosis due to focal lesions in the occipitallbrain lobes. The examination included studies of a fixation of immovable objects, a tracing of linear and pendulum moving objects and a voluntary change of points of fixation in space. It was demonstrated that expressed symptoms of agnosia are accompanied by defects in the organization of all forms of opticomotor activity. The authors discuss problems of the pathogenesis of oculomotor disorders in optical agnosia.

Apathy in Alzheimer's disease: Eye movements characteristics and neurostructural basis.

BACKGROUND: We sought to evaluate the characteristics of eye movements in Alzheimer's disease (AD) patients with apathy (AD-A) and their ability to identify AD-A and explore the shared neurostructure of eye movements and apathy. METHODS: Total 32 normal controls, 36 AD-A, and 72 AD with no apathy (AD-NA) patients were recruited. Parameters of smooth pursuit, fixation, prosaccade, and antisaccade were compared among the three groups. Correlation analyses were conducted among apathy score, eye movement parameters, and gray matter volume (GMV) in AD patients. Receiver operating characteristic cu

Fixational eye movements as active sensation for high visual acuity.

Perception and action are inherently entangled: our world view is shaped by how we explore our environment through complex and variable self-motion. Even when fixating stable stimuli, our eyes undergo small, involuntary movements. Fixational eye movements (FEM) render a stable world jittery on our retinae, which can be expected to harm neural coding. Yet, empirical evidence suggests that FEM help rather than harm human perception of fine detail. Here, we elucidate this paradox by uncovering under which conditions FEM improve or impair retinal coding and human acuity. We combine theory and expe
