[Clinical and experimental study on facial paralysis in temporal bone fracture].

To study the main prognostic factors and significance of facial nerve decompression of facial paralysis in temporal bone fracture, the main relative prognostic factors of 64 patients with facial paralysis were analyzed. Experimental model of facial paralysis was made. Expansion rate of facial nerve in facial canal opening group and non-opening group was measured and observed under electron microscope. The main factors affecting the prognosis were facial nerve decompression and selection of surgery time. Expansion rate of facial nerve in opening group was significantly higher than that of non-o

[Perioperative antibiotic prophylaxis in orthodontic bone operations of the facial skull].

Intraoral orthognathic surgical procedures are clean-contaminated operations because of the facultative pathogenic flora of the oral cavity. Without antibiotic prophylaxis in this kind of operation a postoperative wound infection can be expected in 20-31% of cases. Therefore, a retrospective analysis of the clinical course of 545 patients with various dentofacial deformities was performed to evaluate the importance of perioperative antibiotic prophylaxis. The total rate of wound infections was 2.8% and lower than rates published in other comparable studies without antibiotic prophylaxis. The r

Complications associated with rigid internal fixation of facial bone fractures.

Rigid internal fixation is now a common method for treating fractures of the facial skeleton. However, it is a very precise technique that requires more time than intraosseous wiring. This article reports the complications associated with rigid internal fixation of facial fractures in 223 patients and compares the findings with those accompanying intraosseous wiring.

New aspects of facial nerve pathology in temporal bone fractures.

Electron microscopic examination of intratemporal facial nerve segments removed from 12 patients with persisting facial paralysis following temporal bone fractures revealed that traumatic injury at the geniculum induces retrograde degeneration through the labyrinthine and distal meatal segments of the facial nerve. Fibrosis may occur in the traumatized labyrinthine segment and block regenerating motor fibers. The surgical treatment of traumatic facial nerve injuries should be aimed to avoid or eliminate fibrosis within the labyrinthine segment of the Fallopian canal.

Facial bone fracture associated with carotid-cavernous sinus fistula.

Out of 989 cases of facial bone fracture, ten patients had carotid-cavernous sinus fistulas (1.01%). Their ages ranged from 25 to 48 years. Seven were male and three female. Two of the ten patients had lower third, three patients had middle third, three patients had upper third, and two patients had combined middle and lower third facial bone fractures. The signs and symptoms of a fistula appeared from the first postinjury day up to 50 days after the injury (mean, 21 days). Four patients had symptoms after operation for facial bone fracture. Most fistulae were identified by arteriography befor

[Use of the calvarium for bone grafting in cranio-maxillo-facial surgery].

Bone grafts's traditional donor sites in cranio-maxillo-facial surgery have been for many years and are still in some occasions the ribs, iliac crest and tibia. Bone grafts taken from the calvaria have been used by some surgeons in the past but its wide acceptance was only achieved after Paul Tessier had reported his own experience. The calvaria is composed of inner and outer tables that encloses a layer of cancellous bone called the diploe. A high degree of variability exist with respect to skull thickness. Nevertheless parietal bones is the preferable site for the harvesting of the graft. Th

Surgical correction of late sequelae from facial bone fractures.

This study has described the UCLA experience over a 5 year period in the treatment of secondary traumatic deformities utilizing the craniofacial techniques originally proposed by Tessier. Because of scarring, absent parts, and the malposition of segments, correction of long-standing traumatic deformities remains difficult and treatment results remain less than ideal. Primary correction utilizing the principles we have described herein still provides the best functional and aesthetic results. Facial fractures can no longer be considered as an independent and unrelated entity when dealing with t

Clinical and experimental correlations in temporal bone fracture: the facial nerve.

In this review of 50 patients with fracture of the temporal bone, 11 patients had ipsilateral facial paralysis, three of immediate and eight of delayed onset. Eight patients with complete paralysis of the facial nerve had facial nerve decompression; six of these had incomplete return of function. Only one patient with a delayed complete facial paralysis had complete return of function. The poor functional results in patients with delayed complete facial paralysis due to temporal bone fracture encourage facial nerve decompression in all stable patients with the complete facial paralysis of temp

Tomographic osteometry of the zygomatic bone applied to traumatology of facial bones: Preliminary retrospective study of zygomatic summit in 28 patients.

BACKGROUND: The available referential measures defining the zygomatic bone morphology are adequate enough for full determination of the zygoma positioning, especially for trauma surgery, but osteometric analysis of the zygoma positioning based on tomographic examinations could be useful in peroperative and postoperative imaging. In this study, we propose referential measures, taking into account the morphology and the symmetry of the zygoma. In addition, the reliability and reproducibility are considered. METHODS: Thirty normal computed tomography (CT) scans were used to establish the referen

[Evaluation of the diagnostic value of color Doppler ultrasound examination of salivary gland neoplasms and metastatic tumors from the facial bones].

The aim of the study was to evaluate usefulness of colour Doppler ultrasound examination in diagnosing the salivary gland tumours and the metastatic tumours of the neck originating from the facial part of the skull. Epidemiology and histopathology of the neoplasms involving the salivary glands and the facial skeleton were discussed including the route of their spreading to the neck. The author presents update techniques of bony face radiologic imaging and basic principles of modern colour Doppler ultrasound. The examinations with the use of a colour Doppler equipment-Acuson 128-XP 10 were perf

The clinical availability of facial nerve enhancement in temporal bone MRI for the patients of idiopathic acute peripheral facial palsy.

PURPOSE: This study is to evaluate the duration of facial nerve enhancement in gadolinium-enhanced temporal bone MRI after the onset of acute facial palsy. METHODS: Gd-enhanced MRI imagines were examined in 13 patients with idiopathic acute facial palsy within 14 days after the onset. The degree of facial nerve function was measured according to the House-Brackmann (H-B) grading system at their first visit at outpatient clinic. The follow-up MRI was taken about 16.5 months (7-24 months) after onset of disease. The degree of facial nerve enhancement was measured with signal intensity (SI) whic

Hypoglossal-facial nerve "side-to-side" neurorrhaphy for facial paralysis resulting from closed temporal bone fractures.

BACKGROUND: Closed temporal bone fractures due to cranial trauma often result in facial nerve injury, frequently inducing incomplete facial paralysis. Conventional hypoglossal-facial nerve end-to-end neurorrhaphy may not be suitable for these injuries because sacrifice of the lesioned facial nerve for neurorrhaphy destroys the remnant axons and/or potential spontaneous innervation. OBJECTIVE: we modified the classical method by hypoglossal-facial nerve "side-to-side" neurorrhaphy using an interpositional predegenerated nerve graft to treat these injuries. METHODS: Five patients who experienc

A Rare Case of Metal Exposure After Internal Fixation of Facial Bone Fracture With Autogenous Bone Graft.

Few studies have reported complications in metal fixation systems, such as infection or device exposure. Here, we report our experience with exposed metal screws after the reduction of facial bone fracture. This will be a useful guide to using a metal fixation system in situations in which metal fixation systems should not be used.
