Facial animation in children with Möbius syndrome after segmental gracilis muscle transplant.

Möbius syndrome is a complex congenital anomaly involving multiple cranial nerves, including the abducens (VI) and facial (II) nerves, and often associated with limb anomalies. Muscle transplantation has been used to address the lack of facial animation, lack of lower lip support, and speech difficulties these patients experience. The purpose of this study was to investigate the results of bilateral, segmental gracilis muscle transplantation to the face using the facial vessels for revascularization and the motor nerve to the masseter for reinnervation. The outcome of the two-stage procedure w

[Prognostic value of the stapedius muscle and electroneurography in facial paralysis a frigore, or Bell's palsy].

A retrospective study was made of 170 patients diagnosed as Bell's palsy in a 7-year period (August 1990 to April 1997). We evaluated two prognostic factors: stapedial reflex and electroneurography findings. The condition was classified by nerve affection into three types: less than 10%, 10 to 30%, and over 30%. The evolution of our patients was: complete recovery in 86.4%, slight impairment in 2.9%, major impairment in 3.5%, and unknown in 7%. The stapedial reflex was present in 87 patients, absent then present in 26 patients, and absent in 5 patients. Electroneurography revealed less than 10

Treatment of chronic facial palsy by transplantation of the neurovascularized free rectus abdominis muscle.

We performed neurovascularized free rectus abdominis muscle transplantations in two patients with chronic facial palsy. In one patient, the postoperative course was uneventful, but the patient died from rupture of esophageal varices. In the other patient, both morphologic and functional results were satisfactory. Therefore, the rectus abdominis muscle is considered to be a suitable donor for muscle transplantation for the treatment of chronic facial palsy. The rectus abdominis muscle is advantageous in that (1) simultaneous operations by two teams are possible with the patient in the supine po

Trigeminal and facial branchiomotor neurons have shared muscle targets in zebrafish larvae.

The trigeminal and facial branchiomotor neurons are believed to have unique sets of peripheral muscle targets in the first and second branchial arches, respectively. However, their innervation patterns in zebrafish have only been precisely mapped up to 48 h postfertilization (hpf). In this study, we dissected the final peripheral destinations of the trigeminal and facial motor nerves in the ventral branchial musculature of first-feeding zebrafish larvae. Using a combination of transgenic lines, we found that around 56 hpf, the facial nerve elongates anteriorly to reach the intermandibularis gr

Delayed repair of the facial nerve and its negative impacts on nerve and muscle regeneration.

BACKGROUND: In this experimental protocol, we evaluated the immediate and delayed repair of the buccal branch of the facial nerve (BBFN) with heterologous fibrin biopolymer (HFB) as a coaptation medium and the use of photobiomodulation (PBM), performing functional and histomorphometric analysis of the BBFN and perioral muscles. METHODS: Twenty-eight rats were divided into eight groups using the BBFN bilaterally (the left nerve was used for PBM), namely: G1 - control group, right BBFN (without injury); G2 - control group, left BBFN (without injury + PBM); G3 - Denervated right BBFN (neurotmesi

Latency shift in compound muscle action potentials during electroneurography in facial palsy.

OBJECTIVE: Electroneurography (ENoG) reliably predicts the prognosis of facial palsy. However, the results of ENoG are dependent on the location, where the wave is detected, as a compound muscle action potential (CMAP) arising from the facial muscles. To minimize errors in prognostic prediction, we analysed the latencies of facial CMAPs. MATERIALS AND METHODS: Fifty-seven patients with unilateral peripheral facial palsy and 24 healthy volunteers were enrolled. Amplitudes, negative peak latencies (NPL), and rise latencies (RL) of CMAPs were measured on the paralysed and healthy sides in patien

Variability in facial-muscle innervation: A comparative study based on electrostimulation and anatomical dissection.

Facial-nerve palsy is the most common complication during facial surgery. However, there are few detailed reports on the distribution of the terminal branches of the facial nerve to the mimetic muscles. This also applies to the communicating branches. The aim of our study was to assess the variability of communicant and terminal branches of the facial nerve in humans. This prospective study involved anatomical dissections and intraoperative electric stimulation of facial nerves. We first performed 30 dissections to define the branching patterns of the extracranial facial nerve, with particular

Characterization and Treatment of Unilateral Facial Muscle Spasm in Linear Scleroderma: A Case Report.

BACKGROUND: Linear scleroderma has been associated with muscle spasms ipsilateral to skin lesions. Typically, spasms are located in trigeminal innervated muscles, leading to hemimasticatory spasm (HMS). CASE REPORT: We report a case of linear scleroderma associated with spasm of muscles innervated not only by the trigeminal but also by the facial nerve. DISCUSSION: We review the patient's successful treatment with incobotulinumtoxinA, a formulation of botulinum toxin that has not been reported for use in this condition.

Temporalis Muscle Tendon Unit Transfer for Smile Restoration After Facial Paralysis.

Temporalis muscle tendon unit (MTU) transfer may be used as a single-stage procedure for dynamic reanimation of the paralyzed face. Principles and biomechanics of muscle function and tendon transposition are essential in optimizing outcome. Critical steps and pearls for success include minimizing scarring, maintaining glide plains, mobilizing adequate tendon length, insertion of MTU at ideal tension based on intraoperative dynamic tension-excursion relationship, and insertion of tendon as close to the lip margin as possible. Because muscles adapt to tension, load, and task changes by altering

Assessment of functional outcomes of temporalis muscle transfers for patients with longstanding facial paralysis.

BACKGROUND: Usually, clinical evaluation of facial reanimation provides accurate information about contraction of the mimetic muscles and phonation but fails to identify smile recovery and to quantify the motility of the lower third of the face during a smile. The purpose of this study was to verify that, in longstanding facial palsy, the modified temporalis muscle transfer (MTMT) can result in the ability to smile, not only voluntarily with chewing, but also spontaneously with a sudden emotional stimulus, and to confirm that a symmetric smile can be obtained. METHODS: Ten patients of the tre

Masticatory features, EMG activity and muscle effort of subjects with different facial patterns.

It has been suggested that craniofacial morphology plays an important role in masticatory function, however, there are controversies and unsolved questions that still require elucidation. The aims of this study were to evaluate masticatory performance, mandibular movement, electromyographic (EMG) activity and muscle effort of masseter and anterior temporal muscles during mastication. Seventy-eight dentate subjects were selected and divided into three groups according to vertical facial pattern: brachyfacial, mesofacial and dolichofacial. Silicon-based material was used for chewing tests. Masti

Fat autograft muscle injection (FAMI): new technique for facial volume restoration.

BACKGROUND: Restoration of depleted facial volume is becoming increasingly recognized by cosmetic surgeons as a necessary component of facial rejuvenation. Autologous fat has been used for years, but asymmetry and transient duration have been problematic. OBJECTIVE: To describe a new procedure called fat autograft muscle injection (FAMI), which is designed to maximize the survival of the adipocyte by placement within the muscles of facial expression. METHODS: The technique involves gentle harvesting of fat and centrifugation for 3 minutes. Fat is transferred to 1 cc syringes and attached to

Role of temporomandibular disorders (TMD) in facial pain: occlusion, muscle and TMJ pain.

Temporomandibular disorders (TMD) which comprise myogenic and arthralgic components have been reported to predispose subjects to headache and facial pain. The aim of this study was to evaluate the role of these components in patients with facial pain and to investigate the influence of treatment of TMD on pain of these patients. The subject group consisted of 25 patients suffering from facial pain. The clinical stomatognathic examination was performed before conservative treatment of TMD, and one-two weeks, three months and one year after treatment. The severity of TMD was assessed using the a

Congenital hypoplasia of the depressor anguli oris muscle in the differential diagnosis of facial paralysis.

Facial paralysis presents an interesting diagnostic challenge. Etiologies ranging from facial nerve neuromas and CPA tumors to inflammatory disease and traumatic injuries need be considered and evaluated. However, an asymmetric facial appearance may result from abnormalities of facial musculature as well as facial innervation. Two cases of congenital hypoplasia of the depressor anguli oris muscle, a child and an adult, are reviewed to examine its presentation and diagnostic differentiation from other forms of facial paralysis. Photographs presenting the facial deformity and electrodiagnostic s
