Iatrogenic facial nerve (FN) injury is one of the most feared complications of otologic surgery. Dehiscence of the bony covering of the FN within the temporal bone increases FN vulnerability to accidental injury. High-resolution computed tomography (HRCT) of the temporal bone is used preoperatively to assess middle ear and mastoid anatomy; however, it is unreliable for detecting facial canal dehiscence. In this study, our aim was to determine if preoperative percutaneous FN stimulation could predict middle ear facial canal dehiscence. Between January 2015 and February 2017, we performed preope

The assessment of facial nerve function plays a crucial role in the diagnosis and treatment of facial nerve disorders. The assessment system for facial nerve function is primarily categorized into subjective and objective systems. While the subjective assessment system is relatively simple, it lacks accuracy as it can be influenced by the subjectivity of evaluator.Whereas, the objective system offers higher precision and stability, providing more quantitative information. In recent years, benefited with advancements in computer vision and artificial intelligence,we have witnessed increasingly ac

Various aesthetic procedures have been developed over the past several decades for facial rejuvenation. One of the parallel goals is protection of vital structures such as the facial nerve. In this study, 10 cadaveric hemifacial dissections were performed, and the course of the lower portion of the facial nerve was studied with specific attention addressed toward the three-dimensional location of the nerve as it relates to the usual superficial tissue planes that are dissected during the course of an open facial rejuvenation procedure. The locations of the nerve as it runs within the different

INTRODUCTION: Facial palsy (FP) is a feared complication of parotidectomy. Facial nerve monitoring (FNM) has been poorly evaluated since it was created at the beginning of the 90's. The authors described a retrospective series and discussed it reviewing the literature data.
SUBJECTS AND METHOD: From January 2002 to January 2009, 75 parotidectomies were performed in 72 patients (34 males, 38 females), without FNM in 28 cases (group 1) and with FNM in 47 cases (group 2).
RESULTS: In group 1, 9 FP were noted, 5 were transient and 4 were definitive. In group 2, 12 FP were noted, 9 were transient

The aim of the study was to analyze the incidence of anatomical variations and abnormalities of the facial nerve found during stapedectomy. Specific management in the case of an anomalous course of the facial nerve was also described. A retrospective analysis of clinical records of 316 stapedectomies was carried out in search for facial nerve anomalies. In 25 (7.9%) patients the facial nerve had an abnormal course. Twenty patients exhibited partial and two total overhang of the nerve over the oval window. In two of them the facial nerve was dehiscent in the tympanic segment and in three patien

Hypertrophic neuropathy is a peripheral nerve lesion that is histologically characterized by onion bulb formations around axons. This histologic picture, which is usually seen in generalized hypertrophic neuropathies, can occasionally be observed in single nerves as localized hypertrophic neuropathy. Cranial involvement of such localized hypertrophic neuropathy represents a very rare entity; only a few cases have been reported in the literature. We report the history of a progressive facial paralysis with a tumorous enlargement of the seventh cranial nerve that was clinically suspected of bein

The author gives an account of his experience with the retrosigmoid approach, and emphasizes the preservation of the facial nerve and hearing. A detailed description of the operative technique is given. This approach is used when it is intended to conserve the hearing. The series consists of 2 stage III cases and 26 stage I and II cases. The two stage III cases had total deafness. For the other stages, 25 patients had normal facial function postoperatively. The hearing was preserved in 9 cases in the frequency range 500-1000-2000 Hz, with thresholds of less than 30 dB, and in 7 of cases these

Although the expansion properties of peripheral nerves have been a matter of considerable study in recent years, investigations of the plasticity of cranial nerves, including the facial nerve, have been lacking. Clinicians, however, have long recognized the tenacity of facial nerve function in patients with slow-growing benign tumors that enormously distort the nerve. An experimental study was designed to assess whether tissue expansion techniques can be applied to the extracranial portion of the facial nerves of cats. In eight cats the frontozygomatic branch of the facial nerve was expanded b

At the present time there is no final explanation for the etiology of Bell's palsy. The theory of pathophysiology involving a combined primary and/or secondary ischemia is now well-accepted. As such, it is supposed that there is a dysfunction of the blood vessels which supply the nerve. This is followed by a hyperpermeability and transudation which lead to edema of the nerve and compression of the blood supply. The vicious circle starts and the final result is facial paralysis. The blood supply of the facial nerve has been described previously in the literature, although there have been no exp

The recommended treatment of penetrating traumatic facial nerve injuries associated with immediate, total paralysis of the ipsilateral facial muscles generally includes facial nerve exploration and repair. We reviewed our experience with bullet injuries to the extratemporal facial nerve to determine the efficacy of this approach. Five patients with immediate, total facial nerve paralysis caused by bullet wounds near the extratemporal facial nerve were seen between July 1990 and December 1992. Of four patients who underwent surgical exploration, only one demonstrated complete transection of the

The occurrence of peptidergic nerve fibres containing substance P (SP), calcitonin gene-related peptide (CGRP), vasoactive intestinal peptide (VIP) and neuropeptide Y (NPY) in the cerebral arteries and arterioles is well known. NPY is a vasoconstrictor, while SP, CGRP and VIP induce cerebrovascular dilatation. Vascular dysregulation of the vasa nervorum of the facial nerve is believed to be a central factor in the pathogenesis of Bell's palsy. Hilger's theory of the primary and/or secondary oedema which is followed by the vicious circle is generally accepted. It was our aim to describe the occ

Isolated metastatic malignant melanoma to the facial nerve has never been reported. This presentation illustrates a primary melanoma of the helix of the ear that was treated by excisional biopsy and then wedge resection in 1983. The primary melanoma was Clark's level IV and 1.3 mm in thickness. In 1985, a facial paresis slowly developed. There was no gross evidence of recurrent melanoma in the ear or neck, but CT scan showed a mass in the region of the stylo mastoid foramen. A reoperation of the primary site revealed metastatic melanoma in the facial nerve, expanding it to approximately 10 tim
