Facial pain is a perplexing problem confronting all who practice the healing arts. The purpose of this article is to concentrate on one aspect of facial pain--trigeminal neuralgia. An overview of this entity is presented by demonstrating current concepts in its etiology, diagnosis, and treatment. Classically, trigeminal neuralgia has been described as a paroxysmal, lancinating, knifelike pain which is limited to the anatomic pathways of the fifth cranial nerve. It is a chronic facial pain that is amenable to medical and surgical treatment when correctly diagnosed. A knowledge of the anatomy of

The sphenopalatine ganglion (SPG) has been assumed to be involved in the genesis of several types of facial pain, including Sluder's neuralgia, trigeminal neuralgia, persistent idiopathic facial pain, cluster headache, and atypical facial pain. The gold standard treatments for SPG-related pain are percutaneous procedures performed with the aid of fluoroscopy or CT. In this technical note the authors present, for the first time, an SPG approach using the aid of a neuronavigator.

BACKGROUND: Glossopharyngeal neuralgia is a rare cause of facial pain, occasionally associated with neurocardiogenic syncope, termed vagoglossopharyngeal neuralgia CASE SUMMARY: We present the case of a 72-year-old man with recurrent syncope triggered by paroxysmal facial pain. Investigations revealed a vascular loop compressing the glossopharyngeal nerve. The patient achieved symptom resolution with carbamazepine therapy.
DISCUSSION: This case highlights the importance of considering neurocardiogenic causes in patients with unexplained syncope and emphasizes the role of multimodal investigat

Trigeminal neuralgia, facial spasm, tinnitus, vertigo, and glossopharyngeal neuralgia are believed to be the symptoms complex of hyperactive dysfunction of the cranial nerve caused by vascular cross compression at the root entry (exit) zone of the appropriate nerve. Posterior cranial fossa approach for the neurovascular decompression was enhanced by Jannetta et al (1975). From their experiences of surgery, they emphasized that these symptoms were relieved by surgery. In this report, we will discuss the etiology of the disease, the neurotological examination, the angiographic findings, the oper

PURPOSE OF REVIEW: Pain arising from cranial neuralgias represents a significant health burden. Successful treatment depends on accurate diagnosis, which requires knowledge of neuroanatomy and pathophysiology as well as familiarity with the varied clinical presentations encountered in neurologic practice. This article delineates the relevant anatomy, clinical features, and management of the most common primary and secondary cranial neuralgias.
RECENT FINDINGS: Trigeminal neuralgia, which can result from neurovascular compression or demyelination, is a particularly severe form of facial pain.

Neurosurgeons must have knowledge about the epidemiology of trigeminal neuralgia and facial spasm. The annual incidence of trigeminal neuralgia is 4.3-28.9 per 100,000 persons, with a prevalence of 76.8 per 100,000 persons, increasing with age. It is more common in women and on the right side, with SCA being the most common causative vessel. The long-term efficacy of MVD for trigeminal neuralgia is 80% with complete resolution of pain and 5.2% with complications, which is safe and highly effective when performed by an expert surgeon. Hemifacial spasm has an annual incidence of 0.78/100,000 wit

Idiopathic trigeminal neuralgia is almost always associated with pathological nerve/vessel contact. Symptomatic forms of trigeminal neuralgia include cases of multiple sclerosis, infratentorial tumours and postherpetic neuralgia. Vascular malformations causing neuralgia have rarely been reported. We present the case of a 55-year old woman, who suffered from facial pain and ptosis on her left side. Repeated neurological examinations as well as repeated magnetic resonance imaging did not lead to a definite diagnosis or therapy. The pain suddenly stopped three weeks before admission and only a sl

The lesion of the sensorial nerves of the face is expressed by painful phenomena called neuralgias of the brainstem or its branches. These may be idiopathic or essential and secondary or symptomatic and their clinical evidence and course are different from the former. Essential neuralgia (E.N.), or tic douloureux, is the most frequent facial pain neuralgia. The aim of this paper is to review this subject with evidence of secondary trigeminal neuralgia from oral causes (dental, traumatic, post herpes zoster, etc). The author establishes the differential diagnoses with other etiologies of facial

Intraoperative monitoring of spontaneous facial nerve electromyographic activity during surgery for microvascular decompression in trigeminal neuralgia was evaluated. Fifteen patients with trigeminal neuralgia underwent surgery for microvascular decompression. During the entire operation, free-running facial nerve electromyographic signals were recorded. The data were analyzed with respect to waveform patterns known from vestibular schwannoma-surgery. Special regard was given to the occurrence of A-trains that are associated with postoperative paresis in patients operated on vestibular schwann

Effective management of facial pain syndromes requires a correct clinical diagnosis. The temporal pattern of chronic pain is the most important aspect to be considered. It allows the identification of three groups of patients: (1) those who have paroxysmal pain, (2) those with mixed paroxysmal and constant pain, and (3) those with strictly constant pain. The less is the paroxysmal component, the more likely it seems to be that surgical intervention is useless or even dangerous. In particular, when the diagnosis is atypical facial pain, that is, a diffuse, nonanatomic orofacial pain of unknown

The lancinating facial pain of trigeminal neuralgia usually responds to medical treatment. However, in some patients, trigeminal neuralgia is refractory to drug therapy. In addition, some patients are unable to tolerate the side effects of medication. In these cases, several safe and effective surgical procedures are available for the treatment of this common facial pain syndrome.

Idiopathic trigeminal neuralgia is the most common type of cranial neuralgia. Trigeminal neuralgia is a form of facial pain characterized by proximal lancinating pain confined to the somatosensory distribution of the trigeminal nerve. Bilateral trigeminal neuralgia is a rare situation with an incidence of 1% to 6% in large trigeminal neuralgia series. There is no single or standard method for the treatment of bilateral trigeminal neuralgia. Microvascular decompression for treatment trigeminal neuralgia is an effective and relatively safe method in older patients. We report a rare case of bilat

Previous investigations have identified focal areas of alveolar bone tenderness, increased mucosal temperature, abnormal anesthetic response, radiographic abnormality, increased radioisotope uptake on bone scans, and abnormal marrow within the quadrant of pain in patients with chronic, idiopathic facial pain. The present case reports a 53-year-old man with multiple debilitating, "idiopathic" chronic facial pains, including trigeminal neuralgia and atypical facial neuralgia. At necropsy he was found to have numerous separate and distinct areas of ischemic osteonecrosis on the side affected by t

BACKGROUND: Contralateral trigeminal neuralgia as a false localizing sign in patients with posterior cranial fossa tumors is rare. Persistent contralateral trigeminal neuralgia after removal of the posterior fossa expanding lesion with microsurgical exploration of the affected trigeminal nerve root has been described in only a few reports. Displacement of the brainstem and the trigeminal nerve root, arachnoid adhesions, and vascular compression of the nerve root entry zone have been reported as causes of persistent contralateral trigeminal neuralgia.
METHODS: One patient developed transformat

BACKGROUND: Idiopathic facial pain syndromes are relatively rare. A uniform classification system for facial pain became available only recently, and many physicians and dentists are still unfamiliar with these conditions. As a result, patients frequently do not receive appropriate treatment.
METHODS: This article is based on pertinent publications retrieved by a selective search in PubMed, focusing on current international guidelines and the International Classification of Orofacial Pain (ICOP).
RESULTS: The ICOP subdivides orofacial pain syndromes into six major groups, the first three of

Many pharmacological and surgical treatments are available for the treatment of chronic facial pain. However, many of them are expensive and often very ineffective. Past publications suggested that bulbar trigeminal stereotactic nucleotractotomy is a very useful procedure for the treatment of neuropathic or oncologic facial pain. The authors describe the results of treatment with stereotactic nucleotractotomy in 58 patients with chronic facial pain. The intensity of the pain was evaluated according to the visual analogue scale, and daily life activities were also evaluated. The conclusion was

Trigeminal neuralgia is universally considered the neuropathic facial pain most and best known in medical practice. We propose a short review on trigeminal neuralgia reporting its main clinical aspects, unsolved problems and highlighting differential diagnosis between classical and symptomatic trigeminal neuralgia.

This prospective, open-label study aimed to evaluate the efficacy of pregabalin treatment in patients suffering from trigeminal neuralgia with and without concomitant facial pain. Fifty-three patients with trigeminal neuralgia (14 with concomitant chronic facial pain) received pregabalin (PGB) 150-600 mg daily and were prospectively followed for 1 year. The primary outcome was number of patients pain free or with reduction of pain intensity by > 50% and of attack frequency by > 50% after 8 weeks. Secondary outcome was sustained pain relief after 1 year. Thirty-nine patients (74%) improved afte

PURPOSE OF REVIEW: To review current treatments for trigeminal neuralgia, with an emphasis on determining which patients may benefit from neurosurgical procedures.
RECENT FINDINGS: A detailed history is the most helpful tool for diagnosing trigeminal neuralgia (TN) and predicting response to neurosurgical treatments. Patients with classic trigeminal neuralgia will describe severe, unilateral, intermittent facial pain that is triggered by innocuous sensory stimuli. In most cases, pain is caused by compression of the trigeminal nerve by a blood vessel near the brainstem. Magnetic resonance imag
