A woman with pituitary-dependent Cushing's disease remained hypercortisolemic after bilateral adrenalectomy. A search for an adrenal remnant by venous catheterization study suggested persistent cortisol-secreting tissue in the left adrenal bed. The aim of this study was to determine the causes of fever of unknown origin, to evaluate new diagnostic tests and to elucidate risk factors for chronic or life-threatening disorders. Dermatitis artefacta (DA) is a rare and challenging-to-diagnose factitious dermatological disorder, most commonly affecting late adolescents and young adults. Factitious disorders may trouble any physician during his/her career. A considerable number of cases of Munchausen's syndrome (self-induced disorder) are misdiagnosed in clinical practice. A 36-year-old woman was admitted to our hospital because of fever and abdominal pain. Physical examination showed signs of inflammation of the abdominal wall, and histological examination revealed severe panniculitis. Gilles de la Tourette syndrome (GTS) is a genetic neuropsychiatric disorder, which is more common than previously thought. Ganser syndrome is an uncommon disorder, particularly in children. We present the first report of a young girl aged 7 who was diagnosed independently as having both GTS and Ganser syndrome and who was otherwise well and remained well and improved at follow-up 8 years later. This highlights how the clinicians must always be vigilant when atypical behaviours begin, especially when one diagnosis has already been given. There may well be treatment implications.
