Tibial shaft fractures complicated by compartment syndrome: treatment with immediate fasciotomy and locked unreamed nailing.

Ten patients with an acute tibial shaft fracture complicated by a compartment syndrome were treated with immediate decompressive fasciotomies, bony stabilization with a locked unreamed intramedullary nail, and early active joint motion. There were seven closed and three open injuries. All fractures healed. Knee motion was excellent, but ankle and subtalar motion were not uniformly normal. Nine patients were without any long-term neurologic sequelae. This treatment protocol simplifies the bony and soft tissue management in patients with this combination of injuries.

More Harm Than Good: It is Time to Reconsider Prophylactic Fasciotomy in Lower-Extremity Vascular Injury.

IntroductionFour-compartment calf fasciotomy (CF) can be limb-saving. Prophylactic fasciotomy (PP) is advised in high-risk situations to prevent limb loss. Calf fasciotomy can cause significant morbidity, particularly if performed unnecessarily. We hypothesized that selective use of fasciotomies (SF) after lower-extremity vascular injury would lead to a lower rate of overall fasciotomies without an increase in limb complications than prophylactic fasciotomies (PFs).MethodsTrauma patients who sustained lower-extremity vascular injury that required operative repair at a high-volume trauma center

Orchio-Septopexy: A new technique to cover and fix detorsed testis undergoing fasciotomy of tunica albuginea.

PURPOSE: Compartment Syndrome (CS) has been recognized as a potential factor that worsens testicular viability after detorsion, especially in borderline cases of prolonged ischemia. Fasciotomy of the testicular tunica albuginea to relieve the pressure associated with CS has been proposed to accommodate edema after detorsion, embracing the raw fasciotomy area with tunica vaginalis flap (TVF) or graft. Fashioning the TVF can be tedious in cases of severe scrotal edema. Herein we present a technique that facilitates and expedites the procedure, maintaining the fasciotomy area decompressed. MATER

Influence of prior fasciotomy on infection after open reduction and internal fixation of tibial plateau fractures.

BACKGROUND: High-energy tibial plateau fractures may present with an acute or impending compartment syndrome requiring emergent fasciotomy. There is little information regarding the optimal management of the fasciotomy wound with respect to the subsequent definitive internal fixation of the tibial plateau fracture. The purpose of this study was to compare the rate of infection complication after internal fixation of tibial plateau fractures in patients requiring fasciotomy for compartment syndrome with patients in whom a fasciotomy was not required. METHODS: We retrospectively reviewed all ti

The Evaluation of a Golden Period of Fasciotomy for High Voltage Electrical Burn Injury Patients With Compartment Syndrome.

Electrical burn injuries can cause various acute manifestations that require surgeons to make an early decision, such as fasciotomy for compartment syndromes. Early decompression can become a 'golden period'for limb salvation. This study evaluates the duration of burn to fasciotomy (B-F time) and amputation. A cross-sectional study was performed on medical records. Inclusion criteria were patients with high voltage electrical injuries and compartment syndrome. Exclusion criteria were patients whose extremities were already non-vital on admission and those lost to follow up. Demographic informa

The efficacy and safety of fasciectomy and fasciotomy for Dupuytren's contracture in European patients: a structured review of published studies.

A structured review of published papers was done to assess the efficacy and safety of fasciectomy and fasciotomy in European patients with Dupuytren's contracture. The outcomes varied across 48 studies. For fasciectomy, outcomes and results were as follows: the proportions of patients with a 100% correction in contracture angle ranged from 61 to 97%, the mean improvement in contracture angle ranged from 58 to 79%, and cases judged excellent/good ranged from 63 to 90%. Fasciotomy had similar outcomes, with a mean improvement in contracture angle ranging from 46 to 88%. Immediate failures upon r

[Minimally invasive fasciotomy in the treatment of chronic exertional anterior compartment syndrome of the leg: personal technique].

Numerous fasciotomy techniques have been proposed for the treatment of chronic exertional anterior compartment syndrome. In this work we evaluate the efficacy of a personal surgical technique whereby the fasciotomy is performed through a small cutaneous incision using endo mini-shears. From 2004 to 2006 19 patients, all professional skaters, were submitted to bilateral fasciotomy (38 legs). No complications were observed. Only two patients used pain-killers. In the follow-up (12 months), resumption of competitive activity was achieved by the fourth week in 63.2% of cases (12 patients), by the

[Treatment of epicondylitis by deep fasciotomy of the extensor carpi radialis brevis and supinator: a review of 18 cases].

PURPOSE OF THE STUDY: Epicondylalgia is a frequent condition although the cause and appropriate treatment remain a subject of debate. We present our experience with surgical treatment based on the hypothesis that the condition results from excessive tension on the deep aponeurosis of the carpal and digital extensors. MATERIAL AND METHODS: Between 1996 and 1998, sixteen patients underwent surgical treatment of epicondylalgia unresponsive to medical treatment. There were six men and ten women, men age 43 years, two underwent bilateral procedures. The dominant side was involved in 75% of the cas

Bilateral Lower Extremity Chronic Exertional Compartment Syndrome With Recurrent Symptoms After Prior Fasciotomy: A Case Report.

Chronic exertional compartment syndrome (CECS) is an exercise-related cause of lower extremity pain characterized by reproducible pain, tightness, cramping, and neurologic symptoms that improve with rest. Diagnosis may be delayed because chronic lower leg pain has a broad differential diagnosis, and currently used diagnostic strategies remain imperfect. We report the case of a young woman who began experiencing bilateral lower extremity pain during high school sports that progressed to pain with prolonged standing. Treadmill-provoked bilateral four-compartment pressure testing reproduced sever

Ultrasound-Guided Fasciotomy for Chronic Exertional Compartment Syndrome: A Case Report With Long-Term Sonographic Outcomes.

We report the case of a 38-year-old recreational female runner with anterior and lateral chronic exertional compartment syndrome treated with ultrasound-guided fasciotomy using a V-shaped meniscotome. She began her return-to-run protocol 8 days postprocedure and achieved full symptom resolution. At 6 months, ultrasound revealed persistent fascial separation without complications, representing the first report of long-term sonographic outcomes following fasciotomy for chronic exertional compartment syndrome. These findings support further prospective studies to evaluate the clinical and sonogra

Beyond damage control: Functional preservation and avoidance of iatrogenic harm in combat vascular injury and fasciotomy.

BACKGROUND: Damage-control interventions are essential in combat vascular trauma, but their benefits must be balanced against iatrogenic injury and long-term functional impairment. Extensive vascular exposure, temporary shunting, vessel ligation, and prophylactic fasciotomy may be lifesaving in selected patients, yet their indiscriminate use can increase operative burden and complicate definitive reconstruction. METHODS: A focused narrative review of PubMed/MEDLINE and Google Scholar was performed through July 2026 using predefined combinations of terms related to vascular trauma, temporary s

Collagenase Clostridium Histolyticum Versus Percutaneous Needle Fasciotomy for Dupuytren's Disease: A Systematic Review and Meta-Analysis.

Minimally invasive treatments for Dupuytren's disease (DD), such as percutaneous needle fasciotomy (PNF) and collagenase clostridium histolyticum (CCH), have become alternatives to open surgeries. This meta-analysis compared these treatments in terms of complications, patient satisfaction, clinical outcomes, and recurrence. Relevant studies up to June 2024 were identified through major databases, following PRISMA guidelines, and the study was registered on PROSPERO. Statistical analysis using Review Manager 5.4 found PNF had lower post-operative rates of oedema (RR = 0.15, 95% CI [0.09, 0.27],
