Childhood systemic lupus erythematosus. Association with pancreatitis, subcutaneous fat necrosis, and calcinosis cutis.

Pancreatitis developed in a 12-year-old girl with a one-year history of systemic lupus erythematosus. The pancreatitis was first manifested by panniculitis of the lower extremities. Calcinosis cutis subsequently developed, both in areas of panniculitis and in areas free of panniculitis. The patient's medications at the time of onset of pancreatitis included prednisone, hydrochlorothiazide, and azathioprine, all of which have been reported to be causes of pancreatitis. We wish to alert physicians that pancreatitis associated with cutaneous panniculitis and calcinosis cutis may develop in childr

Video-assisted retroperitoneal debridement for infected retroperitoneal fat necrosis following laparoscopic distal pancreatectomy.

Video-assisted retroperitoneal debridement (VARD) is an established and minimally invasive treatment option for infected necrotizing pancreatitis. However, its application in the postoperative setting after pancreatic resection has rarely been described. We report a case of extensive infected retroperitoneal fat necrosis following laparoscopic distal pancreatectomy that was successfully managed using a step-up approach incorporating VARD. An 81-year-old woman underwent laparoscopic distal pancreatectomy for pancreatic ductal adenocarcinoma of the pancreatic tail. On postoperative day (POD) 7,

When Imaging and Biopsy Mislead: Recurrent Triple-Negative Breast Cancer Mimicking Fat Necrosis.

Triple-negative breast cancer (TNBC) is an aggressive subtype with variable imaging appearances that may mimic benign pathology. Distinguishing recurrence from post-treatment changes such as fat necrosis can be challenging, even with triple assessment. We report a case of a woman in her 40s with a prior history of grade 2 TNBC diagnosed in 2018, treated with neoadjuvant chemotherapy, wide local excision, axillary node clearance, and adjuvant chemoradiotherapy, achieving a complete pathological response. Several years later, a computed tomography (CT) scan performed for peritonitis incidentally

Epiploic Appendagitis Unveiled: A Case of Misleading Abdominal Pain With Omental Fat Necrosis.

Epiploic appendagitis is a benign inflammatory condition of the epiploic appendages, small fat-filled structures attached to the colon. Misdiagnosed frequently as more serious conditions like appendicitis or diverticulitis, it usually resolves with minimal treatment. This case report aims to emphasize the importance of recognizing epiploic appendagitis in differential diagnoses, highlighting the role of accurate imaging and surgical intervention in managing unusual presentations. We report a case involving a 27-year-old male who presented with acute, severe pain in the left iliac fossa. Initia

Subcutaneous Fat Necrosis of the Newborn After Whole-Body Cooling for Birth Asphyxia.

Therapeutic hypothermia (TH) is a challenging treatment for a neonate who suffers from hypoxic-ischemic encephalopathy. It has been shown to improve neurodevelopmental outcomes and survival in infants with moderate-to-severe hypoxic-ischemic encephalopathy. However, it has severe adverse effects such as subcutaneous fat necrosis (SCFN). SCFN is a rare disorder that affects term neonates. It is a self-limited disorder but can have severe complications such as hypercalcemia, hypoglycemia, metastatic calcifications, and thrombocytopenia. In this case report, we present a term newborn who develope

Epipericardial fat necrosis as cause of chest pain in patient after heart transplantation.

Epipericardial fat necrosis is an uncommon clinical condition of unknown etiology. It typically presents as acute pleuritic chest pain and should be differentiated from acute pulmonary embolism and acute coronary syndrome. This condition is diagnosed by characteristic chest computed tomography findings of an ovoid mediastinal fatty lesion with intrinsic and surrounding soft-tissue stranding. Treatment of epipericardial fat necrosis includes the administration of anti-inflammatory agents, and symptoms usually resolve within a few days after treatment initiation. This disease entity has rarely b

Fat necrosis after abdominal surgery: A pitfall in interpretation of FDG-PET/CT.

OBJECTIVE: We describe FDG-PET/CT findings of postoperative fat necrosis in patients following abdominal surgery, and evaluate their changes in size and FDG uptake over time. METHODS: FDG-PET/CT scans from January 2007-January 2016 containing the term 'fat necrosis' were reviewed. Lesions meeting radiological criteria of fat necrosis in patients with prior abdominal surgery were included. RESULTS: Forty-four patients, 30 males, mean age 68.4 ± 11.0 years. Surgeries: laparotomy (n=37; 84.1 %), laparoscopy (n=3; 6.8 %), unknown (n=4; 9.1 %). CTs of all lesions included hyperdense well-defined

Drug injection-related fat necrosis of the breast with FDG PET-CT uptake.

A 52-year-old woman was found to have a lung mass and bilateral breast lesions on computed tomography (CT). Subsequent positron emission tomography/CT demonstrated marked uptake in the lung mass and mild uptake within the breast lesions. A diagnostic mammogram and targeted ultrasound were performed to exclude primary breast malignancy or metastases from presumed pulmonary malignancy. A pertinent history of recent intravenous drug use with heroin injection into bilateral breasts, together with imaging features, facilitated diagnosis of fat necrosis. Fat necrosis is a common diagnosis in breast

[Subcutaneous fat necrosis after moderate therapeutic hypothermia in two newborns of African origin].

Moderate therapeutic hypothermia (MTH) significantly reduces the risk of death or long-term neurodevelopmental disability. Subcutaneous fat necrosis (SCFN) is a rare particular form of panniculitis in full-term newborns. It can occur in preterm infants with predisposing risk factors (birth asphyxia, gestational diabetes, etc.). A few cases of SCFN after MTH have been described. We report two original cases of SCFN after MTH in newborns of African origin. Ethnic origin has not yet been reported in the literature as a risk factor. We describe here the possibility of a new risk factor of SCFN. De

Adipose stromal/stem cells assist fat transplantation reducing necrosis and increasing graft performance.

Autologous fat transfer (AFT) is a procedure for adipose tissue (AT) repair after trauma, burns, post-tumor resections and lipodystrophies still negatively impacted by the lack of graft persistence. The reasons behind this poor outcome are unclear and seem to involve damages in either harvested/transplanted mature adipocytes or on their mesenchymal progenitors, namely adipose stromal/stem cells (ASC), and due to post-transplant AT apoptosis and involution. A rabbit subcutaneous AT regeneration model was here developed to first evaluate graft quality at different times after implant focusing on

Hypercalcemia due to subcutaneous fat necrosis in a newborn after total body cooling.

Subcutaneous fat necrosis is an inflammatory disorder of adipose tissue. Although patients need long-term follow-up to prevent hypercalcemia, the prognosis is generally favorable. We herein present a case of a newborn who developed subcutaneous fat necrosis-related hypercalcemia after hypothermia treatment for hypoxic ischemic encephalopathy. Widespread use of hypothermia treatment for hypoxic ischemic encephalopathy in the neonatal intensive care unit may increase the risk of developing subcutaneous fat necrosis and subsequently hypercalcemia. Great care should be taken to recognize skin find

Nodular-cystic fat necrosis. A reevaluation of the so-called mobile encapsulated lipoma.

We describe five patients with distinct posttraumatic subcutaneous nodules that usually evolved for several months before diagnosis. The nodules occurred in the subcutis of the elbow or hip of women, 33 to 74 years old, and in the hip of a 16-year-old boy. Histologically the fully developed lesions were totally or nearly totally encapsulated by thin, fibrous tissue. All contained well-preserved outlines of nonnucleated adipocytes; there was no inflammation or saponification. In one patient's lestion the viable subcutaneous tissue merged into several partially encapsulated necrotic nodules. In

Radiologic Evaluation of Subcutaneous Fat Necrosis Complicated by an Abscess in a Newborn: A Case Report.

Subcutaneous fat necrosis of the newborn (SCFN) is a condition that may affect neonates within the first weeks of life. SCFN lesions often present as firm, well-defined nodules or plaques under the skin, accompanied by redness or discoloration. These lesions typically present on areas such as the trunk, limbs, or cheeks. SCFN generally resolves on its own but may be associated with metabolic or hematologic complications. This report describes the sonographic findings in a unique case of a seven-day-old female neonate with SCFN complicated by both hypercalcemia and a subcutaneous abscess. The r

Epipericardial fat necrosis diagnosed by cardiac CT in a patient with apical hypertrophic cardiomyopathy.

Epipericardial fat necrosis (EFN) is a rare benign cause of chest pain, that is frequently overlooked. EFN involves the necrosis of fat tissue in the mediastinum, and presents on computed tomography (CT) as an ovoid lesion of fat attenuation surrounded by a rim of soft tissue attenuation. This case report describes a case of a 50-year-old man diagnosed with EFN on cardiac CT, which was incidentally associated with apical hypertrophic myocardiopathy. Notably, the detection of EFN proved difficult on arterial phase images during coronary CT angiography, whereas it was much easier to detect on de

Subcutaneous Fat Necrosis and Hypercalcemia with Nephrocalcinosis in Infancy: Case Report and Review of the Literature.

Subcutaneous fat necrosis is an uncommon benign panniculitis affecting more commonly full-term newborns. It has been associated with birth asphyxia and meconium aspiration, as well as therapeutic hypothermia. Although the prognosis is generally favorable, complications such as hypercalcemia, thrombocytopenia, hypoglycemia and hypertriglyceridemia may complicate its course. The most serious complication is hypercalcemia that may reach life threatening levels and can be associated with nephrocalcinosis. We thereby describe a case of subcutaneous fat necrosis after therapeutic hypothermia, which

Subcutaneous fat necrosis in a newborn following icebag application for treatment of supraventricular tachycardia.

Cases of subcutaneous fat necrosis of the newborn (SCFN) and neonatal cold panniculitis have been reported most often secondary to perinatal distress or hypothermia. We present a case of a newborn infant who developed erythematous, indurated plaques on both cheeks and right shoulder following ice pack application for supraventricular tachycardia. The distinction between SCFN and cold panniculitis is important as SCFN may have complications such as hypercalcemia, whereas cold panniculitis is not associated with such sequelae. Clinicians should be aware of the diagnostic similarities and differe

Simultaneous occurrence of fat necrosis and carcinoma after breast injury in a traffic accident.

An old female patient presented after a car accident with clinical and sonographic evidence of two lesions, located along the seat-belt line. Despite the recent history of trauma and the localization in the traumatized area, the discrepancy between the sonographic size of the smaller lesion and the findings on palpation, together with the lack of typical mammographic findings for fat necrosis rose the suspicion of malignancy. A fine needle aspiration biopsy was performed to confirm the nature of the smaller lesion.

Chronic relapsing pancreatitis with pseudocyst erosion into the portal vein and disseminated fat necrosis.

A 63-year-old man with the triad of chronic relapsing pancreatitis, disseminated fat necrosis with subcutaneous nodular panniculitis, and polyarthritis was found to have a pseudocyst which had eroded into the portal vein. These lesions were associated with markedly elevated serum pancreatic enzymes. Although recent immunological studies by others suggest a possible contribution of immune-mediated tissue injury, clinical and pathological evidence in the present case including direct tissue immunofluorescent findings do not support an immune pathogenesis of this association. The present study fu

Epicardial Fat Necrosis as a Rare Cause of Chest Pain: Peeling Back the Layers.

BACKGROUND: Epicardial fat necrosis (EFN) is a rare, self-limiting condition that presents as acute severe chest pain. Symptoms can mimic life-threatening conditions (myocardial infarction, pulmonary embolism, and pericarditis). CASES SUMMARY: We present 2 cases of EFN with acute, left-sided pleuritic chest pain without a clear precipitating factor. Blood work revealed elevated D-dimer, C-reactive protein, and normal troponin. Chest computed tomography (CT) scan showed characteristic EFN appearance. They were managed with nonsteroidal anti-inflammatory drugs with clinical and imaging resoluti
