Learning from disease registries during a pandemic: Moving toward an international federation of patient registries.

High-quality dermatology patient registries often require considerable time to develop and produce meaningful data. Development time is influenced by registry complexity and regulatory hurdles that vary significantly nationally and institutionally. The rapid emergence of the coronavirus disease 2019 (COVID-19) global pandemic has challenged health services in an unprecedented manner. Mobilization of the dermatology community in response has included rapid development and deployment of multiple, partially harmonized, international patient registries, reinventing established patient registry tim

Privacy-preserving federated learning with optimized ensemble weighting and knowledge distillation for COVID-19 detection from non-IID medical imaging data.

Medical imaging enables rapid and accurate diagnosis of COVID-19, with CT scans proving especially effective. However, data privacy concerns limit collaborative model development across hospitals. To address this issue, we introduce a novel federated learning framework. It is referred to as Independent Knowledge Distillation with post-Ensemble Federated Learning (IKDEFL). Differential Privacy (DP) is integrated into the framework to improve privacy guarantees. Three DP mechanisms are evaluated. These include Fixed Gaussian, Gaussian Adaptive, and Tree Adaptive. The evaluation has been conducte

Collaborative assessment of the risk of postoperative progression in early-stage non-small cell lung cancer: a robust federated learning model.

BACKGROUND: While the TNM staging system provides valuable insights into the extent of disease, predicting postoperative progression in early-stage non-small cell lung cancer (NSCLC) remains a significant challenge. An effective bioimaging prognostic marker for early-stage NSCLC, powered by artificial intelligence, could greatly assist clinicians in making informed treatment decisions. METHODS: A total of 926 patients from four centers (A, B, C, and D) with histologically confirmed stage I or II solid non-small cell lung cancer (NSCLC) who underwent surgical resection were retrospectively rev

An epilepsy prediction and management system based on federated learning combined with hybrid harmony search and mutual information (HAS-MI)-based feature selection approach.

Epilepsy represents a widespread neurological disorder that causes unexpected seizure occurrences which produce significant obstacles for daily life activities in people worldwide. Real-time seizure detection accuracy stands vital for safeguarding patients while ensuring timely interventions and bettering their quality of life. The present seizure detection methods suffer from multiple limitations including poor applicability across different cases and excessive incorrect alerts and inefficient feature selection procedures. Traditional deep learning algorithms generally fail to detect EEG patt

Federated reinforcement learning with constrained markov decision processes and graph neural networks for fair and grid-constrained coordination of large-scale electric vehicle charging networks.

The rapid proliferation of electric vehicles (EVs) and their spatially clustered charging behaviors have imposed unprecedented challenges on the stability, efficiency, and fairness of power distribution networks. Coordinating large-scale EV clusters across geographically distributed charging stations requires intelligent scheduling strategies that can simultaneously respect grid constraints, maximize user satisfaction, and enhance renewable energy utilization-all while safeguarding data privacy and computational scalability. This paper proposes a novel multi-agent cooperative dispatch framewor

Development and validation of random-forest based federated ensemble learning algorithms for delirium prediction using electronic medical records from eleven hospitals in Austria: a retrospective study.

BACKGROUND: Machine learning models have shown great potential in preventive medicine but require large datasets, which is a challenge due to strict privacy regulations in the healthcare sector. Federated learning is an approach that enables collaboration between institutions while preserving data privacy. The focus today in research is highly on developing federated learning methods using artificial neural networks. In this study, we aimed to contribute federated learning modelling methods applied for random forests with an use-case of predicting delirium in hospitalised patients using data f

A scalable federated learning solution for secondary care using low-cost microcomputing: privacy-preserving development and evaluation of a COVID-19 screening test in UK hospitals.

BACKGROUND: Multicentre training could reduce biases in medical artificial intelligence (AI); however, ethical, legal, and technical considerations can constrain the ability of hospitals to share data. Federated learning enables institutions to participate in algorithm development while retaining custody of their data but uptake in hospitals has been limited, possibly as deployment requires specialist software and technical expertise at each site. We previously developed an artificial intelligence-driven screening test for COVID-19 in emergency departments, known as CURIAL-Lab, which uses vita

Glycaemic control remains central in type 2 diabetes mellitus management: key learnings from the latest International Diabetes Federation guidelines.

The 2025 International Diabetes Federation (IDF) guidelines recognise global disparities in healthcare access, with ∼ 80% of people with type 2 diabetes mellitus (T2D) living in low-to-middle-income countries (LMICs). A panel of international experts discussed the evidence underlying these updated guidelines. Randomised trials demonstrate cardiovascular-kidney protection with sodium-glucose cotransporter 2 inhibitors (SGLT2is) and glucagon-like peptide-1 receptor agonists (GLP-1 RAs) in high-risk people with T2D, although their role among those with low-risk disease remains less clear. Whilst

A bias-resilient client selection analysis for federated brain tumor segmentation.

Brain tumor segmentation is difficult because of a number of technical problems, including its complex morphology, individual differences in anatomy, irregular shapes, overlapping, homogeneous gray matter and white matter intensity values, abnormalities that might not contrast with normal tissues, and the possibility of additional complications from various modalities. Expert radiologists may make different conclusions as a result of these difficulties. Regarding this, deep learning techniques, particularly CNN models, can be trained to handle these MRI artifacts and automatically extract feat

Dynamic variance-aware federated tuning for efficient autonomous vehicle perception under non-IID settings.

INTRODUCTION: Federated learning enables multiple autonomous vehicles (AVs) to collaboratively train machine learning models while preserving data privacy. However, performance degrades significantly under non-independent and identically distributed (non-IID) data conditions commonly encountered in real-world driving scenarios. Existing aggregation methods, particularly Federated Averaging (FedAvg), struggle to effectively handle client update divergence, leading to inefficient communication, unstable convergence, and increased privacy risks. METHODS: To address these challenges, we propose a

Forecasting Weekly Dengue Cases by Integrating Google Earth Engine-Based Risk Predictor Generation and Google Colab-Based Deep Learning Modeling in Fortaleza and the Federal District, Brazil.

Efficient and accurate dengue risk prediction is an important basis for dengue prevention and control, which faces challenges, such as downloading and processing multi-source data to generate risk predictors and consuming significant time and computational resources to train and validate models locally. In this context, this study proposed a framework for dengue risk prediction by integrating big geospatial data cloud computing based on Google Earth Engine (GEE) platform and artificial intelligence modeling on the Google Colab platform. It enables defining the epidemiological calendar, delinea

dsLassoCov: a federated Lasso approach incorporating covariate control.

Machine learning has been widely adopted in biomedical research, fueled by the increasing availability of data. However, integrating datasets across institutions is challenging due to legal restrictions and data governance complexities. Federated learning allows the direct, privacy preserving training of machine learning models using geographically distributed datasets, but faces the challenge of how to appropriately control for covariate effects. The naive implementation of conventional covariate control methods in federated learning scenarios is often impractical due to the substantial commu

Multicenter Privacy-Preserving Federated Models for Predicting Postoperative Delirium and Acute Kidney Injury in Older Patients.

Postoperative delirium (POD) and acute kidney injury (AKI) are serious complications in older patients undergoing surgery, yet predictive model development is often constrained by single-center data limitations and privacy concerns that preclude centralized data sharing. To address these challenges, we retrospectively evaluated a simulated federated learning (FL) framework using multicenter datasets partitioned by hospital source, without sharing raw patient data across centers. A total of 7,216 non-cardiac, non-neurosurgical patients aged 65 years or older were included across five centers, w

Short-Term Outcomes and Lessons Learned From the Federal HIV Health Improvement Affinity Group for State Medicaid/Children's Health Insurance Program Agencies and State Health Departments.

BACKGROUND: Medicaid is the single largest source of health care coverage for people living with HIV (PLWH) in the United States. Therefore, high-quality HIV care and associated viral suppression among Medicaid beneficiaries have the potential to greatly impact the HIV epidemic. The HIV Health Improvement Affinity Group (HHIAG) supported state efforts to improve health outcomes for PLWH enrolled in Medicaid through new or enhanced collaborations between state public health departments and state Medicaid agencies. METHODS: Supported by multiple federal health agencies for 1 year, state health

Distributed training of foundation models for ophthalmic diagnosis.

Vision impairment affects nearly 2.2 billion people globally, and nearly half of these cases could be prevented with early diagnosis and intervention-underscoring the urgent need for reliable and scalable detection methods for conditions like diabetic retinopathy and age-related macular degeneration. Here we propose a distributed deep learning framework that integrates self-supervised and domain-adaptive federated learning to enhance the detection of eye diseases from optical coherence tomography images. We employed a self-supervised, mask-based pre-training strategy to develop a robust founda

A federated attention-based stacked LSTM framework for interpretable malaria diagnosis under simulated non-IID federated conditions.

Malaria remains a major global health burden, particularly in low-resource regions where microscopic diagnosis relies heavily on expert interpretation and is prone to variability. Although deep learning models have demonstrated strong classification performance for automated malaria detection, most existing approaches operate as black-box systems and assume centralized, independent and identically distributed (IID) data. These limitations restrict clinical trust and motivate the exploration of privacy-preserving learning paradigms for distributed healthcare environments. To address these chall
