Central Fetal Monitoring With and Without Computer Analysis: A Randomized Controlled Trial.

OBJECTIVE: To evaluate whether intrapartum fetal monitoring with computer analysis and real-time alerts decreases the rate of newborn metabolic acidosis or obstetric intervention when compared with visual analysis. METHODS: A randomized clinical trial carried out in five hospitals in the United Kingdom evaluated women with singleton, vertex fetuses of 36 weeks of gestation or greater during labor. Continuous central fetal monitoring by computer analysis and online alerts (experimental arm) was compared with visual analysis (control arm). Fetal blood sampling and electrocardiographic ST wavefo

Extended noninvasive fetal monitoring by detailed analysis of data measured with phonocardiography.

This paper presents a complex heuristic method for the evaluation of fetal heart sounds, applying simultaneously several algorithms, where the autocorrelation technique has been completed with the wavelet transform and the matching pursuit methods. In this way, a more reliable heart rate variability can be achieved and further parameters of the cardiac operation can be assessed in addition to the conventional cardiotocographic examination. This also comprises those parameters, which can be investigated only with long-term or continuous monitoring, and those, which rely on a very accurate estim

Computerized fetal heart rate monitoring after vibroacoustic stimulation in the anencephalic fetus.

BACKGROUND: To quantify changes in fetal heart rate (FHR) parameters after vibroacoustic stimulation (VAS) and to evaluate the usefulness of VAS testing (VAST) in anencephalic fetuses. Our findings may also help to clarify the route(s) of vibration and sound transmission during VAST. STUDY DESIGN AND SUBJECTS: We obtained the antepartum FHR tracings of 16 anencephalic fetuses, including both the nonstress test (NST) and VAST. Using a computerized monitoring system, HYFM, we determined all FHR parameters from data collected for 10 min before and 10 min after VAS, at successive gestational stag

The use of intrapartum electronic fetal heart rate monitoring: a national survey.

This study describes the use of intrapartum electronic fetal monitoring in Ireland. Electronic fetal monitoring (EFM) has become routine in the assessment of fetal wellbeing during labour. Current evidence indicates that the routine use of EFM leads to an increased caesarean section and operative vaginal delivery rate and a reduction in the rate of neonatal seizures. Practices and service provision related to the use and interpretation of and educational provision for electronic fetal monitoring have not been investigated in Ireland. A national survey of all (n = 22) maternity units in Ireland

Historical controversy in health technology assessment: the case of electronic fetal monitoring.

Electronic fetal monitoring (EFM) was introduced in the late 1950s as an alternative to traditional auscultation by stethoscope or fetoscope in the management of labor and delivery. The new technology was seen as a valuable tool in the prevention of cerebral palsy and other adverse fetal outcomes and diffused rapidly into clinical practice. In the late 1970s, some scepticism began to be voiced about the evidence for the effectiveness of EFM. The authors published a systematic review of the evidence in 1979 that concluded that there was insufficient evidence for the effectiveness of the routine

Adverse outcome after prenatal diagnosis of gastroschisis: the role of fetal monitoring.

Fifty-seven fetuses with gastroschisis presented between 1982 and 1995 were studied by retrospective review of medical records. There were three late intrauterine deaths (IUD). Fetal distress, as determined by reduced fetal movements or abnormal cardiotopograph (CTG), was encountered in 23 of the 54 liveborn infants (43%), all of whom had delivery expedited either by emergency caesarean section (n = 19) or induction (n = 4). Six infants had abnormal neurological outcome: two died in the neonatal period of severe perinatal brain injury, neonatal fits were observed in four, two of whom developed

[Monitoring of maternal-fetal hepatitis B virus transmission by molecular hybridization technique].

Maternal venous blood (MB), umbilical blood (UB) and placental tissue (PT) were collected from 40 HBsAg positive mothers and their neonates, and also blood from 17 babies aged 3-6 months old of this group (BB). All samples were determined for hepatitis B virus (HBV) DNA by molecular hybridization technique using Bio-HBV DNA probe. The results showed: HBV DNA positive rate of MB was 35.0%, 47.5% for UB, 75.0% for PT and 29.4% for BB. 32 P-HBV DNA probe was also used to examine MB and PT. The positive rates of HBV DNA were 30.0% and 70.0% respectively. There was no significant difference between

Fetal heart rate monitoring during labour--too frequent intervention, too little benefit?

For many obstetricians and midwives continuous electronic fetal heart rate monitoring during labour has replaced the traditional method of intermittent auscultation. Of the eight prospective randomised controlled trials designed to assess its value in obstetric care, four were concerned with mothers defined as being at high-risk, three with normal or low-risk patients, and the eighth with the total population of a maternity hospital over several months. None suggested any major advantage of continuous fetal heart rate monitoring over intermittent surveillance in terms of neonatal mortality, mo

Recognition and significance of maternogenic fetal acidosis during intensive monitoring of labor.

FHR monitoring and microanalysis of fetal blood are mutually complementary procedures, and optimal knowledge of the fetal state is achieved by making use of both, the former for the preliminary screening of all cases at risk and the latter for the purpose of deciding on obstetric management where pathological changes are evident in the FHR. The major difficulty in obtaining a precise value for the fetal acid-base balance lies in the occurence of "falsely abnormal" cases, i.e. cases in which the fetal pH falls during labor but the clinical condition at birth is good (APGAR greater than or equal

Effect of Remote Fetal Monitoring on Maternal and Fetal Outcomes in High-Risk Pregnancies: A Systematic Review and Meta-Analysis.

This review evaluates remote fetal monitoring's impact on high-risk pregnancies through a meta-analysis of 19 studies. It highlights remote monitoring's effectiveness in reducing adverse outcomes like neonatal asphyxia and preterm birth, while also increasing natural delivery rates. Despite these benefits, further high-quality research is needed.

Electronic fetal monitoring characteristics of a patient with sudden onset of placental abruption and intrauterine fetal demise: A case report.

INTRODUCTION: Placental abruption (PA) is a serious complication of pregnancy, associated with significant perinatal complications, including intrauterine fetal demise (IUFD). Continuous electronic fetal monitoring (EFM) has been widely applied in China in recent decades. Exploration of potentially PA-specific patterns of EFM contributes to early detection of PA occurrence. PATIENT CONCERNS AND DIAGNOSIS: A 33-year-old woman (gravida 3, para 1) was referred to our hospital at 33 weeks gestation due to non-reassuring fetal heart rate (FHR) pattern, and suffered sudden onset of severe PA and su

Fetal heart rate monitoring category 3 during the 2nd stage of labor is an independent predictor of fetal acidosis.

OBJECTIVE: To determine whether fetal heart rate (FHR) monitoring categories during the 1st and 2nd stage of labor can predict arterial cord pH <7.2. MATERIALS AND METHODS: A case control study was conducted including 653 consecutive term deliveries (37 weeks gestation and above) that were divided according to fetal pH ≤ 7.2 (n = 315) and fetal pH > 7.2 (n = 338). Deliveries occurred during the year 2013 in tertiary medical center, where arterial cord pH is routinely taken after birth. Intrapartum FHR monitoring categorization was defined according to the ACOG committee guidelines by two obst

A filamentary soft robotic probe for multimodal in utero monitoring of fetal health.

Fetal surgery offers valuable opportunities to address severe congenital disabilities, yet accurate evaluation of fetal physiological changes during in utero procedures to mitigate the risk of operative complications remains an unmet need. Conventional unimodal approaches lack predictive value, specificity and compatibility with minimally invasive interventions. Here we present a bioelectronic system featuring a multimodal, steerable filamentary probe that interfaces directly with the fetus in utero, enabling reliable and minimally invasive monitoring of various physiological parameters. Is it possible to monitor fetal movements with a wearable device? A review of novel technologies.

Stillbirth is often preceded by reduced fetal movements and obstetric intervention is necessitated for stillbirth prevention. Yet, despite fetal movements being one of the few tangible ways a pregnant woman and the clinical team can assess the wellbeing of her baby, there are few validated, objective means for pregnant women to quantify the frequency and nature of an unborn baby's movements. Traditional methods of assessing fetal wellbeing such as cardiotocography and fetal movement charts have a lack of diagnostic accuracy, and often lead to false positive intervention. The need for fetal mov
