Real-time monitoring of on-duty firefighters has the potential to reduce the risk of cardiovascular events and injuries by enabling early detection of fatigue and occupational hazards. Recent work uses wearable sensors to track metrics like heart rate and body temperature. However, most existing systems lack integrated environmental sensing, making it difficult to discern whether elevated vitals stem from physical exertion, ambient heat, or a combination of the two. To address this, a wireless sensing system is proposed that synchronizes physiological and environmental measurements during acti

Exposure biomonitoring is limited to quantifying the internal dose of specific contaminants. However, it fails to capture the complex total biological burden comprising chemical mixtures (such as fire emissions) and physical and psychological stress. Effect biomonitoring, particularly using untargeted metabolomics, is required to comprehensively assess the systemic health effects of these complex exposures. This pilot study aimed to identify metabolic changes by analyzing urine samples collected from two individual firefighters before and after two controlled training scenarios. One scenario m

OBJECTIVES: Firefighters are frequently exposed to psychologically and physically stressful experiences. This volatile occupational environment is thought to contribute to the risk of firefighter musculoskeletal disorders (MSDs). However, investigations of firefighter MSDs are limited, and to our knowledge, have not been collectively examined. The purpose of this novel systematic review was to systematically identify, critically appraise, and synthesize evidence regarding the relationship between MSDs and personal, physical, and psychosocial factors among career firefighters to guide future st

Tobacco control is a public health priority. However, Korea has recently experienced a reversal in its long-term decline in smoking prevalence. To address this trend, it is essential to broaden advocacy networks by engaging new stakeholders. We propose that firefighters represent a strategic yet under-represented partner in tobacco control because their professional interests closely align with tobacco-related policy goals. First, cigarette butts have consistently been the leading cause of fire incidents, and tobacco use directly undermines firefighters' core mission to protect life and public

OBJECTIVE: This study aimed to characterize the smoke exposure of firefighters who attended the Grenfell Tower fire during the initial 20 hours.
METHODS: As no compilation of exposure data exists, data were compiled from nine unconnected sources, including the Grenfell Tower Inquiry, firefighters' statements, incident logs, and the UK Firefighter Cancer and Disease Registry.
RESULTS: Of the 628 firefighters who attended, information was available from 524. Eighty-nine percent reported exposure to smoke without respiratory protection equipment (RPE), of whom 23% also reported exposure to exce

In response to the escalating complexity and frequency of wildland fires, this study investigates the feasibility of using wearable devices for real-time monitoring of cardiac, respiratory, physical, and environmental parameters during live wildfire suppression tasks. Data were collected from twelve male firefighters (FFs) from the Italian National Fire Corp during a simulated protocol, including rest, running, and active fire suppression phases. Physiological and physical metrics such as heart rate (HR), heart rate variability (HRV), respiratory frequency (fR) and physical activity levels wer

The James M. Inhofe National Defence Authorization Act for Fiscal Year 2023 established that certain illnesses and diseases are to be deemed as proximately caused by employment in federal fire protection activities. However, cancers affecting female wildland firefighters were not included on this list and are recommended for further evaluation. We present the best available scientific evidence for the risk of breast and gynaecological cancers associated with exposures that are commonly measured in the wildland fire environment, including benzene, polycyclic aromatic hydrocarbons, and particula

Wildland firefighters can work at high intensity in hot environments for extended periods of time. The resulting heat strain may be modified by the environmental conditions (i.e., ambient temperature and humidity [RH]) even at equal wet-bulb globe temperatures (WBGTs). This investigation assessed if a hot and dry condition would create greater strain than moderate and high humidity at equivalent WBGT (28 °C). Twelve participants (age 24 ± 2 year) walked at 40%-50% maximum aerobic capacity for 90 and 40 min separated by a 20 min rest in dry (40 °C, 20% RH), moderate-humidity (34 °C, 50% RH), an

In February 2023, two catastrophic earthquakes in Turkey resulted in over 50,000 fatalities and over 300,000 injuries. This study investigated the psychological effects on firefighters who engage in rescue operations following these earthquakes, focusing on trauma-related symptoms, depression, anxiety, stress, and resilience. The research employed validated psychometric instruments, including the Post-earthquake Trauma Level Determination Scale (PETLDS); Depression, Anxiety, Stress Scale-21 (DASS-21); PTSD Checklist for DSM-5 (PCL-5); and Brief Resilience Scale (BRS), to assess the psychologic

INTRODUCTION: Exertional heat stroke is a life-threatening emergency necessitating immediate treatment with rapid body cooling. A field-expedient alternative may be tarp-assisted cooling, requiring only water and a tarp. The objective of this study was to compare core temperature (Tc) cooling rates of tarp-assisted cooling using the limited resources available to a wildland firefighter and the current standard care provided in wilderness settings.
METHODS: This cross-over, randomized control trial of 17 healthy individuals consisted of exercise in a 42±1°C, 32±4% relative humidity environment

The clothes and special equipment of firefighters can be a source of indoor air pollution. Nevertheless, it has not been investigated so far what the scale of the release of various compounds from such materials into the indoor air can be. The following study analysed the results of an experiment involving the passive measurement of concentrations of selected compounds, i.a. benzene, toluene, ethylbenzene, m,p-xylene, o-xylene, styrene, isopropylbenzene and n-propylbenzene (BTEXS) in the air of a room where firefighters' special clothing, which had been previously exposed to emissions from sim

Firefighter injures caused by burns and thermal stress occupies around 5%-10% of the total injuries annually. Glove is the thinnest/weakest components among the firefighter turnout gear, which can put firefighters, are at risk of severe wrist and hand burns during fire calls. Burns can occur quickly and enhancing the thermal protective performance of firefighters' gloves will prevent these burns. One-dimensional (1D) heat transfer modeling and simulations were performed through the COMSOL Multiphysics software to investigate the improvement of thermal protective performance when integrating a

This study aimed to investigate the current situation of firefighting burn injuries and personal protective equipment (PPE) in Korea using a questionnaire. A questionnaire was constructed and then distributed to fire stations via the Fire & Disaster Headquarters of Korea. Responses from a total of 536 firefighters who were currently responsible for suppressing fire or doing rescue work were analysed (39.2 ± 8.58 y in age, 173.8 ± 5.0 cm in height, 73.4 ± 8.9 kg in body weight). The results showed that 22% of firefighters had experienced burns but of these 93% of were burns of less than 1% of t

UNLABELLED: Resistance exercise is an important strategy to reduce injuries and improve performance in the fire service. However, given the large age range of firefighters, it is important to determine if age influences their recovery following an acute bout of resistance exercise.
PURPOSE: To examine the changes in indirect markers of muscle damage in young and older firefighters following a feasible worksite resistance exercise routine.
METHODS: Nineteen young (25.5 ± 3.4 years) and 19 older male career firefighters (50.3 ± 3.5 years) completed pre-testing, an acute bout of resistance exer

The scope of tasks of chemical and ecological rescue procedures includes prevention of terrorist attacks with biological weapons. After each action, firefighters are obliged to clean and disinfect their outfits to prevent the potential spreading of harmful microorganisms. This study aimed to analyze the effectiveness of decontamination fluids used to disinfect firefighter's suits. Two types of clothes were analyzed: special combat clothing (NOMEX), and the heavy gas-tight chemical type 1a suit. Swabbed places were cut out and sterilized mechanically using detergent and alcohol. Each time, smea

Firefighters' skin may be exposed to chemicals via permeation/penetration of combustion byproducts through or around personal protective equipment (PPE) or from the cross-transfer of contaminants on PPE to the skin. Additionally, volatile contaminants can evaporate from PPE following a response and be inhaled by firefighters. Using polycyclic aromatic hydrocarbons (PAHs) and volatile organic compounds (VOCs) as respective markers for non-volatile and volatile substances, we investigated the contamination of firefighters' turnout gear and skin following controlled residential fire responses. On June 20, 2011, the Indiana Department of Homeland Security notified the Indiana State Department of Health (ISDH) of an Indiana fire station that reported gastrointestinal illness among a substantial percentage of their workers, causing missed workdays and one hospitalization as a result of cryptosporidiosis. All ill firefighters had responded to a barn fire in Michigan, 15 miles from the Michigan-Indiana border on June 6; responding firefighters from Michigan also had become ill. ISDH immediately contacted the Michigan Department of Community Health (MDCH) concerning this outbreak. The inv
