BACKGROUND This is a case report of a male patient who presented with a history of right flank pain based on renal infarction. Initially the symptoms were misdiagnosed as acute pyelonephritis. CASE REPORT A 47-year-old male with a history of familial hypercholesterolemia and cerebral infarction presented at the Emergency Department with a 3-day history of acute right-sided flank pain. Physical examination revealed hypertension, subfebrile temperature, and costovertebral angle tenderness. Blood tests were unremarkable except for renal impairment, a high C-reactive protein level of 215 mg/L (nor

INTRODUCTION: To evaluate the efficacy of laparoscopic pyeloplasty (LPP) for lower functioning kidney, we investigated the outcome of this procedure for patients with ureteropelvic junction obstruction with decreased renal function, defined as less than 20% split renal function.
PATIENTS AND METHODS: Between October 1998 and June 2015, we performed transperitoneal dismembered LPP in 224 patients. Among them, 15 patients with less than 20% split renal function were included in this study. Patient characteristics, perioperative split renal functions, complications, and surgical outcomes were re

Posterior nutcracker syndrome (PNCS) is a rare vascular disorder involving the compression of the retro-aortic left renal vein between the aorta and the vertebral column. It often presents with left flank pain, hematuria, and varicocele. We report the case of a 39-year-old male who presented with diffuse abdominal pain, acute urinary retention, and left flank pain. Radiologic evaluation revealed retro-aortic left renal vein compression with proximal dilatation, consistent with PNCS. The atypical presentation and non-specific clinical findings led to a conservative management approach. This cas

Spontaneous renal subcapsular haematoma is not a common clinical condition. It is mainly caused by renal tumours, vascular diseases, trauma, anticoagulation, and urological interventions. Patients present with sudden onset abdominal pain mainly in the flank region, haematuria, and abdominal tenderness. We report a case of 42-year-old diabetic patient who presented with a history of fever and left flank pain with clinical signs of pyelonephritis, on whom abdominal ultrasound and computed tomography demonstrated a subcapsular renal haematoma which was confirmed by percutaneous aspiration. Nutcracker syndrome is a rare disorder characterized by extrinsic compression of left renal vein by the superior mesenteric artery and aorta. Prevalence of the disease is unknown, but presents most commonly with gross hematuria and flank pain. Diagnosis requires a high index of suspicion and treatment consists of a wide range of vascular surgical options with a more recent focus using an endovascular approach. We present a case of a 29-year-old female with continuous gross hematuria and flank pain from a segmental artery pseudoaneursym secondary to nutcracker and pelvic congestion syndrome.

A 36-year-old diabetic man came to our institution presenting with constant left flank pain. Left renal embolic infarction was found by abdominal computed tomography. Silent ST segment elevation myocardial infarction was noted on 12-lead electrocardiogram. Emergent coronary angiography revealed large thrombus burdens with complete occlusion at the left anterior descending artery ostium, which may be the embolic origin. Silent ST segment elevation myocardial infarction with acute flank pain and multiple segmental renal infarction is an unusual presentation. High vigilance may prevent delay of t

Benign tumors in the upper parts of the urinary tract are rare. The clinical symptoms of flank pain and/or macrohematuria often present problems concerning differential diagnosis. As radiological sign the smoothly bound filling defect with possible ureter obstruction is characteristic but not convincing. Therefore the diagnosis can only be made by operation and a simultaneous histological analysis. This report is based on a female patient with symptoms of flank pain and macrohematuria. By operative exploration a fibrous ureteral polyp was found which was then excised through an ureterotomy.

A 28-year-old woman came for non-traumatic right flank pain with hypotension and right flank mass. She had multiple hyperpigmented skin papules located on the centre area of her face, and angiomas on her toes. She was anaemic and had a blood transfusion on top of aggressive fluid resuscitation. Abdominal CT showed bilaterally enlarged kidneys and fluid collection in the right perirenal space (haemorrhage). The consideration was an angiomyolipoma in spontaneous perinephric haemorrhage. We considered tuberous sclerosis complex (TSC) and did genetic testing. Results revealed mutations in the TSC2

INTRODUCTION: Urinalysis (UA) is performed routinely as a diagnostic screening test for trauma patients in most centers. This study aimed to examine the relationship between patients' clinical signs and symptoms with UA findings.
METHODS: This cross-sectional study was carried out on multiple trauma patients between 18 to 65 years old, who were referred to the Emergency Department. UA was performed for all patients and its association with clinical signs and symptoms (pain, tenderness, abrasion, ecchymosis, hematoma, etc.) in abdomen, back, flank, and inferior hemi-thorax was evaluated.

We report two cases of a pain syndrome caused by large adrenal metastases in patients with lung cancer. A review of the literature identified 23 previously reported patients with primary lung cancers who appear to have had a similar syndrome, although in none of these cases were other likely causes of the pain syndrome carefully excluded. The syndrome characteristically includes unilateral flank pain but may have abdominal components as well, and has only been reported in patients with large metastases (> or = 5 cm in largest diameter). Although the mechanism by which large adrenal metastases

Pyogenic vertebral osteomyelitis (PVO) is included in a spectrum of spine infections called spondylodiscitis. It is commonly caused by hematogenous seeding. The annual incidence of spondylodiscitis is 2.4 per 100,000 person-years and the incidence triples for people over seventy. We report a case of a previously healthy 14-year-old male with a rare presentation of vertebral osteomyelitis associated with a large retroperitoneal abscess of unknown origin, manifested with flank pain, back pain, and fever, and treated with antibiotics with good short-term outcomes. We concluded that vertebral oste

The clinical presentation of acute onset of nausea, vomiting, and flank pain in combination with acute elevation of blood pressure should raise high suspicion of renal infarction. However, because of its nonspecific presentation, diagnosis may be delayed. We report the case of a 63-year-old man who presented with a 2-day history of right flank pain that was treated initially as a renal stone. He had a background history of atrial fibrillation. Further investigations confirmed this as a case of renal infarction. Renal infarction is underdiagnosed because of the similarity of its presentation to

A 63-year-old man admitted to our hospital with severe pain in his right flank on June 4, 1987. After admission, his pain increased progressively and he complained of tenderness from the right flank to the right lower quadrant. Computerized tomography and intravenous pyelography demonstrated spontaneous rupture of the right renal pelvis. A retrograde ureteral catheter was indwelling into the right renal pelvis. Because extravasation had disappeared on the retrograde pyelogram, the Catheter was withdrawn after 12 days. After removal of the ureteral catheter, he did not complain of pain. The 42

BACKGROUND: A 7-year-old previously healthy girl was injured in a traffic accident and presented to the emergency room with abdominal pain, microscopic hematuria, and wide skin defects and deep lacerations on the left flank, left upper abdomen, and right inguinal area. Initial CT of the abdomen was unremarkable. 3 weeks later, the patient complained of abdominal distension, left flank pain, and fever.
INVESTIGATIONS: Blood and urine tests, CT of the abdomen, chest X-ray, antegrade pyelography, intravenous urography, renal ultrasonography and diuretic renal scan.
DIAGNOSIS: Complete avulsion

The herniation of ureter is rare. A 59-year-old woman with hydronephrosis caused by herniation of ureter through psoas muscle fascia was observed. The patient referred to our clinic with four weeks' history of left flank pain and fever. Computed tomography urography and antegrade combined with retrograde urography revealed tortuous and proximal dilated ureter and hydronephrosis on the left side. Herniation of ureter through psoas muscle fascia was confirmed in operation and laparoscopic ureteroplasty with end to end anastomosis was done. No obvious hydronephrosis or flank pain was observed in

A male patient, 40 years of age, arrived at our Institute with diffuse abdominal tenderness, right flank pain, hematuria and early stage of hemorrhagic shock with anemia and initial hypotension. The immediate clinical history revealed no significant previous trauma, only subsequently was reported inconstant pain in the right flank for 4-5 days with pallor and asthenia, signs and symptoms that the patient had not investigated. Abdominal CT scan with angiographic evaluation was performed showing right kidney mass and perirenal fluid collection by blood component. Immediate nephrectomy was perfor

A 41-year-old woman with recurrent urinary tract infections presented with right flank pain, fever, and anorexia. Physical examination revealed right-sided flank pain and a right-sided unilateral renal mass. Computed tomography of the abdomen and pelvis revealed xanthogranulomatous pyelonephritis with perinephric extension. The bear's paw sign was visualized. The classic radiology triad (unilateral large kidney, renal pelvis stone, and a nonfunctioning or poorly functioning kidney) was also found on computed tomographic imaging. She completed a course of antimicrobial therapy and underwent a n

Acute renal infarction is a well known, although relatively unfrequent, cause of flank pain resistant to administration of spasmolytic and nonsteroidal anti-inflammatory drugs. We present an original case of a 41-year-old man, complaining of acute severe left flank pain, resistant to common analgesic therapy, who was diagnosed of segmental renal infarction of a branch of left renal artery. Pathophysiology of renal damage in cocaine users is multifactorial, and it has been postulated that the right kidney was more prone to ischaemia. Left kidney represents an extremely unusual site of cocaine-r

Previously reported cases of spontaneous rupture of the kidney or ureter reflect underlying renal pathology and have been reported both spontaneously and in relationship to delivery, but not in relation to other operative procedures. A 27-year-old woman at 19 weeks' gestation developed severe right flank pain in the operating room immediately prior to cerclage placement. Postoperative renal ultrasound examination and intravenous pyelogram performed to evaluate persistent flank pain demonstrated renal pelvis rupture. No other renal abnormality was present. We postulated that increased urine flo
