Trauma-Informed Care for Youth in Foster Care.

For decades, evidence has shown an undeniable connection between childhood trauma and chronic adverse reactions across the lifespan (Bilchik & Nash, 2008; Perry, 2001; Perry, 2006). Childhood traumatic experiences are associated with serious and persistent, long-term physical, psychological, and substance abuse issues. In addition to adverse effects on physical health, research indicates that early childhood trauma has particularly adverse effects on adolescent self-esteem, coping skills, school performance, self-regulation, critical thinking, self-motivation, and the ability to build healthy

Second-generation immigrant children: health prevention for a new population in terms of vaccination coverage and health assessment.

BACKGROUND: In recent years the total number of foreigners taking up residence in Italy is increasing: the number of children born in Italy to foreign parents currently account for 15% of all babies born in the country. This population is generally referred to as "second-generation immigrants". We evaluated the health conditions of this particular population by investigating the vaccination coverage and auxological data in a group of foreign children living in a foster care setting and by comparing them to those regarding a group of foreign children living with their own parents. METHODS: Thi

Dissociation in middle childhood among foster children with early maltreatment experiences.

OBJECTIVE: This study examined levels of dissociation in school-aged foster children who had been maltreated before age 5. METHOD: Data were collected from 118 children (age in years: M = 9.34, SD = 1.02) and their caregivers. Chi-Square and ANOVA were used to compare foster children (n = 67) to community comparison children (n = 51). Regression analyses examined predictors of dissociation for the foster children. RESULTS: The foster children showed significantly higher levels of dissociation than the control children. Within the foster care group, number of placement transitions, female gen

Mental health service utilization by children in foster care in California.

An examination of Medi-Cal-paid claims was undertaken to assess the utilization of mental health services by children in California's foster care system. Using unduplicate counts of service use and diagnoses, it was determined that children in foster care account for 41% of all users of mental health services even though they represent less than 4% of Medi-Cal-eligible children. When partitioned into specific service categories, children in foster care account for 53% of all psychologist visits, 47% of psychiatry visits, 43% of Short Doyle/Medi-Cal inpatient hospitalization in public hospitals

Cognitive and functional outcomes at age 21 after prenatal cocaine/polydrug exposure and foster/adoptive care.

OBJECTIVE: Prenatal cocaine exposure (PCE) has been linked to specific cognitive deficits and behavioral outcomes through early adolescence but there is little information on adult outcomes nor on the relationship of environmental interventions, such as foster/adoptive care, to outcomes. METHODS: At 21 years, data were available on 325 young adults, [163 PCE and 162 non-exposed (NCE)], primarily African-American, with low SES, who were followed from birth in a prospective longitudinal cohort study. Participants were administered the Wechsler Abbreviated Scale of Intelligence (WASI-II) and sur

Prevalence and Treatment of Maternal Substance Use Disorder in Child Welfare.

IMPORTANCE: Parental substance use is a primary driver of child welfare system involvement, and child welfare services primarily seek to address caregiver challenges, including connecting parents to treatment. Although research highlights that formal child welfare services connect children to health care professionals, less is known about how the system affects caregivers' substance use treatment. OBJECTIVE: To examine the prevalence of maternal substance use disorder (SUD) in the Pennsylvania child welfare system and the association between formal child welfare system response and mothers' t

The effect of institutionalization on psychomotor development of preschool aged children.

Development can be altered by several factors which can either facilitate or obstruct development. The aim of the current study was the examination and the detection of differences in the developmental profiles of preschool aged children living in conventional institution facilities (N=28), in SOS villages (N=20) and in natural family environment (N=48). The psychomotor development of the 96 children, aged 4-6 years old, was assessed using the six scales of the Griffiths Test No II. Two-way analysis of variance designs showed that family reared children had better performance in all scales of

Caregiver Perspectives on Challenges of Caring for Individuals with Intellectual Disability: A Qualitative Study.

This study examined the challenges encountered by caregivers of individuals with intellectual disability in a residential care home and their suggested solutions. A basic qualitative design within the interpretive paradigm was employed, using purposive sampling to recruit 21 caregivers. Thematic analysis revealed three overarching themes: disability-related challenges, care home-related challenges, and strategies to improve quality of life. Disability-related challenges involved behavioral issues, cognitive impairments, and difficulties in managing chronic conditions. Care home-related challen

Reunification in foster care: Influences on decision-making.

An important goal of out of home care is to prepare the family and child for reunification. Practitioners are often required to make the decision whether to reunify a foster child with their biological family. This study examines this complex reunification decision in Portugal. Using the Judgments and Decisions Processes in Context model, the authors presented to Portuguese child protection workers and higher education students (n = 400) a case vignette of a child in foster care in which reunification is considered. The vignette consider two scenarios related to the child's wish to go back or

IQ at age 12 following a history of institutional care: Findings from the Bucharest Early Intervention Project.

Young children removed from institutions and placed into foster care or adoptive homes have been shown to experience significant gains in IQ relative to children who remain in institutions. Less is known about the long-term impact of severe early deprivation on development in late childhood. Data are presented from a follow-up of children at 12 years of age in the Bucharest Early Intervention Project, a randomized clinical trial of foster care for institutionally reared children. Of the original 136 children in the study, 107 were tested with the Wechsler Intelligence Scale for Children-IV. Re

Estimating the number of children in formal alternative care: Challenges and results.

Given the relatively large body of literature documenting the adverse impacts of institutionalization on children's developmental outcomes and well-being, it is essential that countries work towards reducing the number of children in alternative care (particularly institutional care), and, when possible, reunite children with their families. In order to do so, reliable estimates of the numbers of children living in such settings are essential. However, many countries still lack functional administrative systems for enumerating children living outside of family care. The purpose of this paper i

Childhood antecedents of homelessness in psychiatric patients.

OBJECTIVE: This study examined the relationship between childhood experience and homelessness in psychiatric patients. METHOD: Three large and diverse samples of homeless patients (N = 512) were compared with a sample of patients who had never been homeless (N = 271), with respect to childhood experience of foster care, group home placement, and running away. One of the homeless samples and the never homeless sample were drawn from patients admitted to a state mental hospital. In this state hospital population, risk ratios for lifetime prevalence of homelessness could be derived. RESULTS: In

Neglect of chronically ill children.

Child neglect is the most prevalent form of maltreatment; more children die of neglect than of abuse. Neglect has been found to be more prevalent than physical abuse among the population of children with chronic illnesses. Data from a retrospective chart review illustrate the ways in which neglect affected 61 children with chronic illnesses. Types of neglect include medical care neglect, educational neglect, abandonment, emotional neglect, and physical neglect. The state Child Protection Agency was notified of neglect in 67 instances. Two children died subsequent to neglect. Sixteen children w

Altered associations between white matter structure and psychopathology in previously institutionalized adolescents.

Previously institutionalized adolescents show increased risk for psychopathology, though placement into high-quality foster care can partially mitigate this risk. White matter (WM) structure is associated with early institutional rearing and psychopathology in youth. Here we investigate associations between WM structure and psychopathology in previously institutionalized youth. Adolescent psychopathology data were collected using the MacArthur Health and Behavior Questionnaire. Participants underwent diffusion MRI, and data were processed using fixel-based analyses. General linear models inves

Antipsychotic medications and admission to psychiatric residential treatment facilities among youth in foster care diagnosed with disruptive behavior disorders.

Many youth in foster care are diagnosed with disruptive behavior disorder (DBD), a diagnosis indicative of aggression and behavior problems. These youth, who are at high risk for being placed in psychiatric residential treatment facilities (PRTF), are commonly prescribed antipsychotic (AP) medications off-label. However, treating children in the community is an important goal, and although AP medications can have severe side effects, these prescriptions may help to achieve this goal. In this study, we used Medicaid data to determine whether AP medications reduce the risk of admission to PRTF a

Mental Health and Developmental Disorder Diagnoses of Foster and Nonfoster Children Enrolled in Medicaid.

OBJECTIVE: This study used Medicaid claims to compare the prevalence of mental health disorders (MHDs) and developmental disorders (DDs) between foster and nonfoster youth. METHODS: Medicaid claims data were used to identify MHDs and DDs. Diagnosis codes were categorized into 16 MHD and 8 DD groups. Children were enrolled in their respective Medicaid plan for at least 30 days and had at least 1 health care encounter generating a claim. Prevalence was compared between children in foster care and children not in foster care overall and by age group. Logistic regression was used to compare the o

Placement and Delinquency Outcomes Among System-Involved Youth Referred to Multisystemic Therapy: A Propensity Score Matching Analysis.

Multisystemic therapy (MST) was developed to help youth with serious social, emotional, and behavioral problems. Research on the efficacy and effectiveness of MST has shown positive outcomes in different domains of development and functioning among various populations of youth. Nonetheless, even with a large body of literature investigating the treatment effects of MST, few studies have focused on the effectiveness of MST through large-scale dissemination efforts. Utilizing a large sample of youth involved in a statewide dissemination of MST (n = 740; 43% females; 14% Black; 29% Hispanic; 49%

Housing and Child Well Being: Implications for Research, Policy, and Practice.

Inadequate housing and homelessness represent significant barriers to family stability and child development. An accumulating body of evidence documents the relatively high risk of family separation among families experiencing housing instability and homelessness, the extent of housing problems experienced by families involved in the child welfare system, and the disproportionately high rates of homelessness among youth aging out of foster care. Vulnerable youth and families interact frequently with various social service programs intended to mitigate multifaceted and multilevel risks, however
