Use of high-flow nasal cannula and intravenous propofol sedation while performing flexible video bronchoscopy in the intensive care unit: Case reports.

Flexible video bronchoscopy is a procedure that plays an important role in diagnosing various types of pulmonary lesions and abnormalities. Case 1 is a 68-year-old male patient with a lesion in the right lung apex of approximately 4 mm × 28 mm with atelectasis bands due to a crash injury. High-flow system with 35 L/min and fraction of inspired oxygen (FiO2) 0.45 and temperature of 34 °C was installed prior to the video bronchoscopy. SpO2 was maintained at 98%-100%. The total dose of sedative was 50 mg of propofol. In Case 2, a 64-year-old male patient with bronchiectasis, cystic lesions and pu

Cilia-driven transport in confined ducts: an active porous media model.

Ciliated organs transport viscous fluids through confined ducts, yet how duct morphology and ciliary activity jointly set the limits of flow rate and sustainable pressure remains unclear. Here, we model dense arrays of beating cilia lining duct walls as an active porous medium driven by prescribed metachronal waves, and identify two key morphological parameters that govern transport: the ciliary confinement ratio and the mean ciliary fraction. The resulting flows are described by the incompressible Navier-Stokes-Brinkman equations, which we solve numerically using a spectral method in the low-

Implications of soil mixing for NAPL source zone remediation: Column studies and modeling of field-scale systems.

Soil remediation is often inhibited by subsurface heterogeneity, which constrains contaminant/reagent contact. Use of soil mixing techniques for reagent delivery provides a means to overcome contaminant/reagent contact limitations. Furthermore, soil mixing reduces the permeability of treated soils, thus extending the time for reactions to proceed. This paper describes research conducted to evaluate implications of soil mixing on remediation of non-aqueous phase liquid (NAPL) source zones. The research consisted of column studies and subsequent modeling of field-scale systems. For column studie

Evaluation of mitral regurgitation using a digitally determined color Doppler flow convergence 'centerline' acceleration method. Studies in an animal model with quantified mitral regurgitation.

BACKGROUND: The imaging and measurement of the proximal flow convergence region in the left ventricle have been reported to be useful for identifying the site of mitral regurgitation (MR) and for evaluating its severity. However, the application of this method has not gained general acceptance. There have been few in vivo studies with quantified reference standards for determining regurgitant volume, and those that have been reported used spectral Doppler standards and/or nonsimultaneously performed contrast ventriculography. The purpose of the present study was to evaluate the proximal flow c

Characterization of Post-Operative Hemodynamics Following the Norwood Procedure Using Population Data and Multi-Scale Modeling.

Children with hypoplastic left heart syndrome (HLHS) must undergo multiple surgical stages to reconstruct the anatomy to a sustainable single ventricle system. Stage I palliation, or the Norwood procedure, provides circulation to both pulmonary and systemic vasculature. The aorta is reconstructed and attached to the right ventricle and a fraction of systemic flow is redirected to the pulmonary arteries (PAs) through a systemic-to-PA shunt. Despite abundant hemodynamic data available 4-5 months after Norwood palliation, data is very scarce immediately following stage I. This data is critical in

Robust Critical Connectivity Threshold in Ranked Percolation of Granular Packings.

The formation of sintering bridges in amorphous powders affects both flow behavior and perceived material quality. When sintering is driven by surface tension, bridges emerge sequentially, favoring contacts between smaller particles first. Predicting the connectivity percolation threshold is key to understanding and controlling the onset of sintering. We investigate ranked percolation in granular packings, where particles connect based on contact number. While the percolation threshold defined by the fraction of connected particles is non-universal and highly sensitive to particle size dispers

Regional deposition of the allergens and micro-aerosols in the healthy human nasal airways.

The nasal cavity is the inlet to the human respiratory system and is responsible for the olfactory sensation, filtering pollutant particulate matter, and humidifying the air. Many research studies have been performed to numerically predict allergens, contaminants, and/or drug particle deposition in the human nasal cavity; however, the majority of these investigations studied only one or a small number of nasal passages. In the present study, a series of Computed Tomography (CT) scan images of the nasal cavities from ten healthy subjects were collected and used to reconstruct accurate 3D models

Moving cracks in drying colloidal films.

Drying colloidal films are encountered in many applications ranging from paints and coatings to ceramic and semiconductor processing. In many cases, shrinkage stresses are generated during drying, which can fracture the film. While much of the previous experimental and theoretical work has focused on cracking in static cracks, there are very few studies on the dynamics of cracks in colloidal coatings. Here, we derive an analytical solution for the stress, displacement, and pressure fields near the crack tip for a steadily moving crack. We consider first the two extreme cases, namely, the undra

Improved characterization of EV preparations based on protein to lipid ratio and lipid properties.

In recent years the study of extracellular vesicles has gathered much scientific and clinical interest. As the field is expanding, it is becoming clear that better methods for characterization and quantification of extracellular vesicles as well as better standards to compare studies are warranted. The goal of the present work was to find improved parameters to characterize extracellular vesicle preparations. Here we introduce a simple 96 well plate-based total lipid assay for determination of lipid content and protein to lipid ratios of extracellular vesicle preparations from various myeloid

4D flow MRI quantification of mitral and tricuspid regurgitation: Reproducibility and consistency relative to conventional MRI.

BACKGROUND: In patients with mitral or tricuspid valve regurgitation, evaluation of regurgitant severity is essential for determining the need for surgery. While transthoracic echocardiography is widely accessible, it has limited reproducibility for grading inlet valve regurgitation. Multiplanar cardiac MRI is the quantitative standard but requires specialized local expertise, and is thus not widely available. Volumetric 4D flow MRI has potential for quantitatively grading the severity of inlet valve regurgitation in adult patients. PURPOSE: To evaluate the accuracy and reproducibility of vol

Learning hemodynamic scalar fields on coronary artery meshes: A benchmark of geometric deep learning models.

Coronary artery disease involves the narrowing of coronary vessels due to atherosclerosis and is currently the leading cause of death worldwide. The gold standard for its diagnosis is the fractional flow reserve (FFR) examination, which measures the trans-stenotic pressure ratio during maximal vasodilation. However, the invasiveness and cost of this procedure have prompted the development of computer-based virtual FFR (vFFR) computation, which simulates coronary flow using computational fluid dynamics (CFD) techniques. Geometric deep learning algorithms have recently shown the capability to le

Advancements in oil recovery and waste treatment at Shaybah utilizing hydraulic shock and cavitation technology.

This paper reports a single-site, pilot-scale field deployment of an integrated hydraulic shock, hydrodynamic cavitation, and attrition treatment system for hydrocarbon-contaminated sludge and sand at Saudi Aramco's Shaybah Gas Oil Separation Plant (GOSP). The study design was a continuous-flow field trial conducted over a period of 4 weeks against a documented pre-existing disposal baseline (800 km transport to off-site landfill with no resource recovery). There was no concurrent experimental control arm, as the full waste volume generated during a turnaround and inspection (T&I) event was pr

Measuring atrial stasis during sinus rhythm in patients with paroxysmal atrial fibrillation using 4 Dimensional flow imaging: 4D flow imaging of atrial stasis.

BACKGROUND: Paroxysmal atrial fibrillation (PAF) is associated with cardioembolic risk, however events may occur during sinus rhythm (SR). 4D-flow cardiac magnetic resonance (CMR) imaging allows visualisation of left atrial blood flow, to determine the residence time distribution (RTD), an assessment of atrial transit time. OBJECTIVE: To determine if atrial transit time is prolonged in PAF patients during SR, consistent with underlying atrial stasis. METHOD: 91 participants with PAF and 18 healthy volunteers underwent 4D flow analysis in SR. Velocity fields were produced RTDs, calculated by
