UNLABELLED: Internal fixation is the most common and effective fracture treatment, and the design of Internal Fixation Implants (IFI) is important for fracture healing. In recent decades, IFI have been designed from the aspects of materials, geometry, fixation methods and functional characteristics. However, there has been no comprehensive summary on the evaluation method and design methods of IFI. This paper aims to review and analyze the key issues involved in the design of IFI, to provide references for IFI design. Firstly, the main factors affecting the healing effect are summarized and th

Micromotion-defined as controlled cyclic axial movement at the fracture site-has emerged as a promising approach to enhance bone fracture healing. This review aims to evaluate micromotion-based fixation systems across biomechanical, preclinical, and clinical domains, highlighting their benefits, limitations, and technological progress. We summarize key micromotion technologies applied across various fixation systems, including far cortical locking and dynamic locking mechanisms in screws, suspension-based and shape-memory alloy-driven adjustments in plates, dynamization approaches in intramedu

OBJECTIVE: Odontoid fractures in older adults often lead to substantial morbidity and mortality. This study explores the outcomes of water-based endoscope-assisted anterior odontoid screw (AOS) fixation, a minimally invasive technique, in managing these fractures.
METHODS: Six patients (5 men, 1 woman) underwent endoscope-assisted AOS fixation for displaced odontoid fractures. Surgical procedures and follow-up assessments were reviewed retrospectively. Inclusion criteria included type IIa or type IIb odontoid fractures with potential nonunion/instability.
RESULTS: Surgical procedures demonst

Advancements in fracture fixation have significantly reshaped orthopedic trauma management by improving mechanical stability and promoting biological healing. This retrospective observational study assessed clinical and biomechanical outcomes in 500 patients who underwent surgical fixation for radiologically confirmed fractures. Patients were categorized into two groups: Group A received conventional fixation methods (e.g., standard plates and screws), while Group B was treated with advanced systems, including locking plates, intramedullary nails, minimally invasive percutaneous osteosynthesis

BACKGROUND: Dynamic compression plating is a fundamental type of bone fracture fixation used to generate interfragmentary compression. The goal of this study was to investigate the mechanics of the surgical application of these plates, specifically how plate prebend, screw location, fracture gap, and applied torque influence the resulting compressive pressures.
METHODS: Synthetic bones with transverse fractures were fixed with locking compression plates. One side of the fracture was fixed with locking screws. On the other side of the fracture, a nonlocking screw was inserted eccentrically to

Mandibular fractures are rare in children and normally result from high-impact trauma. Clinical protocols have evolved in recent years, especially after the introduction of rigid internal fixation methods and the development of novel resorbable materials. The purpose of this report is to present the case of a three-year-old toddler who fell from a bunk bed at home, resulting in a unilateral fracture of the mandibular parasymphysis and a joint fracture on the left side. The patient was treated under general anesthesia with rigid internal fixation with biodegradable plates and screws. Pediatric

INTRODUCTION: Patella fractures account for approximately 1% of all bone fractures. The tension band wiring technique has been used in surgical treatment. However, there is no clear information about the location of the K-wires in sagittal plane. Thus, a transverse fracture line was created in the patella finite element model and fixed with Kirchner (k) wires and cerclage at different angles and compared with two different standard tension band models.
MATERIALS AND METHODS: A total of 10 finite element models were created to study AO/OTA 34-C1 patella fractures. Two models used the classical

BACKGROUND: Design freedom offered by additive manufacturing allows for the implementation of functional gradients - where mechanical stiffness is decreased along the length of the implant. It is unclear if such changes will influence failure mechanisms in the context of rib fracture repair. We hypothesized that our novel functionally graded rib implants would be less stiff than controls and decrease occurrence of secondary fracture at implant ends.
METHODS: Five novel additively manufactured rib implants were tested along with a clinically used Control implant. Fracture reconstructions were

BACKGROUND: Dorsal spanning plate fixation can be used to treat comminuted distal radius fractures not amenable to volar plating. However, there is a relative paucity of outcomes data; so, the aim of this retrospective study was to investigate outcomes and complications after dorsal spanning plate fixation for distal radius fractures.
METHODS: Distal radius fractures treated with dorsal spanning plate fixation at a level-1 trauma center were retrospectively identified via Current Procedural Terminology coding and chart review from 2014 to 2019. Patient demographics, fracture pattern character

INTRODUCTION: Concerning rates of nonunion in articular distal humerus fractures indicate an unsolved problem. The fixation principles of O'Driscoll describe linking the fractured articular segment to the distal humerus columns with compression screws which creates a stable fixed angle construct. A novel device has been introduced which utilizes an interlocking beam through the articular segment to connect the distal aspect of the medial and lateral plates, creating a linked construct. We sought to evaluate the stability of this linked construct using an articular model of distal humerus fract

BACKGROUND: Closed reduction and splinting followed by outpatient management is standard of care for temporizing most ankle fractures. However, ankle fracture-dislocation potentially warrants a different approach based on the propensity for loss of reduction. The purpose of this study was to determine the rate of complications associated with closed reduction and splinting of unstable ankle fracture-dislocations. Further, we sought to determine the efficacy of immediate external fixation as an alternative to splinting in cases too swollen for acute operation.
METHODS: This retrospective chart

BACKGROUND:: The Centers for Medicare and Medicaid services (CMS) have implemented initiatives to improve postdischarge care and reduce unnecessary readmissions. Readmissions within 30 days are frequent and represent an economic burden on both patients and the healthcare system. The aim of this study was to evaluate the frequency and causes for urgent care visits within 30 days of discharge after ankle open reduction and internal fixation (ORIF) and determine factors correlated with such visits.
METHODS:: This was a retrospective analysis of prospectively collected data. All patients who unde

BACKGROUND: We sought to define the rate of syndesmotic instability after anatomic reduction of the posterior malleolus when posterior stabilization of a trimalleolar or trimalleolar equivalent ankle fracture was chosen vs when a supine position and initially conservative management of the posterior elements was chosen.
METHODS: The types of syndesmotic and posterior malleolar fixation used to treat adult patients with ankle fractures involving the posterior malleolus at our level I trauma center were retrospectively assessed (N = 198). Specifically, both bimalleolar and trimalleolar fracture

BACKGROUND: The incidence of severe dysphagia requiring gastrostomy tube (GT) placement following operative fixation of traumatic cervical spine fractures is unknown. Risk factors for severe dysphagia are not well identified and GT placement is often delayed due to the belief that it will resolve quickly. We hypothesized that patient and clinical factors could be used to predict severe dysphagia requiring GT placement in this population.
METHODS: A retrospective multicenter review of all adult patients requiring operative fixation of cervical spine fractures was performed. Data on demographic

INTRODUCTION: Distal radius fractures are common. In cases where surgical treatment is needed, volar plates can be used to obtain stable, long-lasting fixation. The design of these plates has continually improved over the years, but complications remain a problem.
PURPOSE: The goal of this study was to evaluate the types of complications that occur with different types of volar plates with a view towards preventing them.
MATERIALS AND METHODS: The emergency department at our hospital saw 524 patients with distal radius fractures between 2006 and 2008. Some of these were treated surgically wi
