OBJECTIVE: To investigate the treatment method and effectiveness of fresh closed fracture-dislocation of the midtarsal joint.
METHODS: Between April 2004 and April 2011, 73 patients (75 feet) with fresh closed fracture-dislocation of the midtarsal joint were treated with closed reduction combined with open reduction and internal fixation. There were 56 males (58 feet) and 17 females (17 feet), aged from 19 to 62 years (mean, 35.8 years). DATA SOURCES: The Cochrane Oral Health Group Trials Register, Cochrane Central Register of Controlled Trials, Medline and Embase were used for searches. In 11 patients with a Rolando's fracture of the first metacarpal, the mini-Hoffman external fixation was used to maintain closed reduction of the fracture, with two pins placed distally in the first metacarpal and one pin proximally in the trapezium. The results were excellent in 7 patients; in 3 patients mild arthritis was noted radiographically, and one patient had a poor result because of severe postraumatic arthritis 1-5 years (mean 30 months) after the injury. This method of treatment is proposed for patients with Rolando's fracture as it is very easily performed and provides good results

Acute hematogenous salmonella osteomyelitis is rare among immunocompetent adults. One of the basic principles of orthopedic surgery is that gas gangrene does not develop in closed fractures. That there are exceptions to this rule is demonstrated by the following report of an extremely rare case. Clostridium septicum myonecrosis developed after surgical treatment of a closed femoral fracture in a fit 45-year-old man, treated by antibiotics. The patient survived after having a right hip disarticulation and administration of high doses of intravenous penicillin. Prophylactically administered cephalosporins failed to prevent the Clostridial infection.

A 61-year-old man, involved in an automobile accident, sustained a complete Achilles tendon rupture with an ipsilateral, closed slightly displaced medial malleolus ankle fracture. The tendon rupture was not diagnosed before operation but was recognized at the time of open reduction of the ankle. The authors report two cases of closed reduction of the anterior wall of the frontal bone. In the first case, the fractured segment was successfully reduced with a small periosteal elevator. In the second case, a tip of the periosteal elevator was broken during maneuvering the fractured bone and lost into the sinus cavity. The sinus was opened to retrieve the broken piece and repaired by internal fixation. The technique has a few merits: (1) the procedure is simple; (2) the operation time is short; and (3) only a small incision is needed.

BACKGROUND: Historically, the standard of care for minimally or nondisplaced, closed, distal fibula fractures has been short-term immobilization and progressive weightbearing. The vast majority of such patients are expected to heal with excellent functional outcome after such treatment. BACKGROUND: Although sternal fractures after blunt chest trauma are markers for significant impact, the fracture itself is generally not associated with any specific wound complications.
