Jack vertebral dilator kyphoplasty for treatment of osteoporotic vertebral compression fractures.

Osteoporotic vertebral compression fractures (OVCFs) are common in the elderly population and often involve the thoracolumbar vertebrae. Clinical symptoms of OVCFs include severe pain, loss of vertebral height, progressive kyphosis and increased mortality. Jack vertebral dilator kyphoplasty is a recently developed OVCFs treatment modality, with few systematic studies present in the literature. This retrospective study was designed to investigate the safety and efficacy of Jack vertebral dilator kyphoplasty for treating thoracolumbar OVCFs. Sixteen elderly patients (55-85 years) with solitary t

Percutaneous vertebroplasty for pathological vertebral compression fractures secondary to multiple myeloma.

BACKGROUND: Vertebral compression fractures are common in multiple myeloma. Percutaneous vertebroplasty is used to stabilize vertebral collapse and treat the pain. The major technical drawbacks of percutaneous vertebroplasty are the potential for neural comprise and pulmonary embolism of cement from leakage of polymethylmethacrylate into epidural space and perivertebral veins. We have retrospectively evaluated the safety and complication of percutaneous vertebroplasty in the vertebral compression fractures resulting from multiple myeloma. METHODS: From August 2003 to July 2008, we describe 24

Clinical evaluation of locking compression plate fixation for comminuted olecranon fractures.

BACKGROUND: In patients managed with plate fixation for the treatment of an olecranon fracture, the placement of an axial intramedullary screw may obstruct the placement of bicortical screws in the ulnar shaft. To overcome this problem, unicortical screws can be applied with use of a contoured locking compression plate. The present study was designed to assess the effectiveness of this fixation method. METHODS: Nineteen consecutive patients with an acute comminuted olecranon fracture were managed with a contoured locking compression plate and intramedullary screw fixation. Sixteen patients we

Does timing matter in performing kyphoplasty? Acute versus chronic compression fractures.

The objective of this prospective consecutive cohort study was to compare the clinical outcomes, the radiographic outcomes and the complication rates of symptomatic acute (< 10 weeks) and chronic (> 16 weeks) osteoporotic vertebral compression fractures (VCFs) treated with kyphoplasty. Twenty-eight consecutive patients had 52 symptomatic osteoporotic VCFs treated with kyphoplasty; 5 of these patients were treated in two sessions. The Oswestry Disability Index (ODI) for back pain, a Visual Analog Scale for pain assessment, a patients' satisfaction scale, and medication usage served to evaluate

[Treatment of severe osteoporotic thoracic vertebral compression fractures by percutaneous kyphoplasty].

OBJECTIVE: To investigate clinical outcomes of percutaneous kyphoplasty with balloon in the treatment of severe osteoporotic thoracic vertebral compression fracture (SVCF). METHODS: From May 2006 to July 2007, percutaneous unilateral kyphoplasty with single balloon was performed in 7 vertebras of 6 SVCF patients, with 2 injured vertebras in 2 males and 5 in 4 females, who were from 64 to 83 years old. The injured vertebras included 1 in T5, 2 in T8, 3 in T10 and 1 in T12 and the compression rates were 60% to 75% in 5 vertebras and > 75% in 2 vertebras. All the injured vertebras were old fract

Diffusion-weighted imaging for predicting new compression fractures following percutaneous vertebroplasty.

BACKGROUND: Percutaneous vertebroplasty (PVP) is a technique that structurally stabilizes a fractured vertebral body. However, some patients return to the hospital due to recurrent back pain following PVP, and such pain is sometimes caused by new compression fractures. PURPOSE: To investigate whether the apparent diffusion coefficient (ADC) of adjacent vertebral bodies as assessed by diffusion-weighted imaging before PVP could predict the onset of new compression fractures following PVP. MATERIAL AND METHODS: 25 patients with osteoporotic compression fractures who underwent PVP were enrolled

Locking compression plate fixation of Vancouver type-B1 periprosthetic femoral fractures.

BACKGROUND: Fractures occurring at or near the distal tip of a hip prosthesis with a stable femoral stem (Vancouver type-B fractures) are associated with many complications because of the inherently unstable fracture pattern. Locking compression plates use screws that lock into the plate allowing multiple points of unicortical fixation. Such unicortical fixation may lower the risk of damage to the cement mantle or a stable femoral stem during the treatment of a periprosthetic femoral fracture. The purpose of this study was to analyze clinically and radiographically a group of patients with a V

Intracorporal bone grafting for vertebral compression fractures with intraosseous vacuum phenomenon.

From April 1998 to January 2000 we treated 14 patients with vertebral osteoporotic compression fractures and intraosseous vacuum phenomenon with intracorporal bone grafting using a mixture of autogenous bone graft and calcium sulfate and posterior instrumentation. The mixture was packed into the fractured vertebral body through two tunnels created in the pedicles. Eleven patients were followed for a minimum of 24 months. All vertebral bodies healed. There were no perioperative complications. The average vertebral height restoration was 31%. On the visual analogue scale, there was an improvemen

Malignant and benign compression fractures: differentiation and diagnostic pitfalls on MRI.

The distinction between malignant and benign compression fractures is a common problem in clinical practice. Various imaging techniques (plain radiography, computed tomography, bone scintigraphy) have been used to differentiate these conditions but they are often inadequate in distinguishing the nature of compression fracture. This review illustrates the magnetic resonance imaging (MRI) features of malignant and benign compression fractures with emphasis on the usefulness, limitations and pitfalls of MRI.

The cluster phenomenon in patients who have multiple vertebral compression fractures.

Little is known about the natural history of patients who have multiple compression fractures. During an eight-year period, eight patients who suffered relentless nonmechanical back pain after an initial compression fracture were evaluated. Radiographs and radionuclide bone scans were obtained in all eight patients and showed the temporal clustering of at least five spontaneous thoracic or lumbar vertebral compression fractures within a period of eight months. The patient database during this eight-year period included 21 patients with a similar number of fractures but without the phenomenon o

[Compression fractures of the vertebrae in children with acute lymphoblastic leukemia].

Compression fractures of vertebrae were noted in 20 out of 1,700 children with the acute lymphoblastic leukemia. Usually prognosis in these cases has been favourable (70% of patients are alive from 5 months to 19 years). Percentage of recovery from compression fractures has been relatively high. Lymphoblastic leukemia with infiltrations localized in the spine is relatively non-aggressive, develops slowly, and despite extensive lesions to the bones its outcome results are favourable. Main symptom of spinal involvement include severe and persisting back aches which make walking impossible. Such

Systemic mastocytosis presenting with severe spinal osteopenia and multiple compression fractures.

Systemic mastocytosis is a rare condition in which mast cells infiltrate various organs, including the skeleton. Because the mast cell secretes various bioactive substances that may induce bone resorption, this condition may cause generalized osteoporosis. We describe a case of a 28-year-old woman who presented with a painful thoracolumbar kyphosis due to generalized osteopenia and multiple pathological compression fractures and was found to have mastocytosis. She underwent operative stabilization of her kyphotic deformity with anterior interbody fusion and posterior Cotrel-Dubousset (CD) inst
