Best Practices for Integrating Community Health Workers Into Care Teams and Organizations: Lessons From the Front Line.

This qualitative case study examined community health worker (CHW) integration within the CHW for COVID Response and Resilient Communities (CCR) program across 7 CCR recipient sites in the United States. The study questions addressed how CHWs are integrated into health organizations and care teams and facilitators and barriers to the integration of CHWs. From July to August 2023, we conducted 25 interviews with CHWs (n = 11) and program staff or partners (n = 14). Guided by the Consolidated Framework for Implementation Research (CFIR), we analyzed data thematically to identify the impact of fa

Examining Differences in Community Health Worker (CHW) Training Based on Organizational Experience With CHWs: A Call for CHW Workforce Sustainability.

As the COVID-19 crisis unfolded, community health workers (CHWs) in the United States were quickly identified as an essential workforce of health professionals that could reach community members who were at the highest risk for the negative effects of COVID-19. The reliance on CHWs during the COVID-19 pandemic has accelerated decades-long advocacy for sustainable funding for CHWs. Advocates argue, in part, that CHW programs must be up and running with trained CHWs and existing relationships with communities to most effectively respond to public health emergencies. Here we provide insight into

The development of a smartphone app to enhance post-traumatic stress disorder treatment in high-risk workers.

OBJECTIVE: Post-traumatic stress disorder (PTSD) is highly prevalent in certain populations. However, evidence indicates that many individuals do not respond to treatment. Digital supports hold promise for increasing service provision and engagement but there is a lack of evidence on blended care options and still less research guiding the development of such tools. This study details the development and overarching framework used to build a smartphone app to support PTSD treatment. METHODS: The app was developed in line with the Integrate, Design, Assess, and Share (IDEAS) framework for the

Silver linings: will the COVID-19 pandemic instigate long overdue mental health support services for healthcare workers?

A study in this month's journal adds to the growing body of evidence regarding the potential mental health impacts on frontline healthcare staff working during the COVID-19 pandemic. As clinical academics representing critical care, nursing, and medicine, and a psychologist guiding support for frontline health and social workers, we offer our perspectives on this study. We discuss the balance between pragmatic and rigorous data collection on this topic and offer perspectives on the observed differential impact on nurses. Finally, we suggest that the pandemic might have a positive effect by ins

Essential work, unequal places: Neighborhood characteristics of the essential workforce in the United States during the COVID-19 pandemic.

INTRODUCTION: At the onset of the COVID-19 pandemic, more than 55 million U.S. workers continued providing "essential" services crucial to public health, safety, and economic stability, even as large segments of the workforce sheltered at home. Despite the central role these workers played in safeguarding the economy and the greater American public, little is known about the neighborhood environments in which essential workers lived and how well these communities were equipped to support the health and well-being of essential workers during the pandemic. Therefore, the primary objective of thi

COVID-19 Infection Among Healthcare Workers: Serological Findings Supporting Routine Testing.

A growing body of evidence demonstrates that asymptomatic and pre-symptomatic transmission of SARS-CoV-2 is a major contributor to the COVID-19 pandemic. Frontline healthcare workers in COVID-19 hotspots have faced numerous challenges, including shortages of personal protective equipment (PPE) and difficulties acquiring clinical testing. The magnitude of the exposure of healthcare workers and the potential for asymptomatic transmission makes it critical to understand the incidence of infection in this population. To determine the prevalence of asymptomatic SARS-CoV-2 infection amongst healthca

Human preparedness: Relational infrastructures and medical countermeasures in Sierra Leone.

This paper examines health worker experiences in two areas of post-epidemic preparedness in Sierra Leone - vaccine trials and laboratory strengthening - to reflect on the place of people in current models of epidemic response. Drawing on ethnographic research and interviews with health workers in the aftermath of Ebola, it explores the hopes and expectations that interventions foster for frontline workers in under-resourced health systems, and describes the unseen work involved in sustaining robust response infrastructures. Our analysis focuses on what it means for the people who sustain healt

Prevalence of post-traumatic stress disorder in healthcare workers before and during COVID-19: a systematic review and meta-analysis.

UNLABELLED: The COVID-19 pandemic exacerbated many known risk factors for post-traumatic stress disorder (PTSD) among healthcare workers. This systematic review and meta-analysis examines pooled prevalence estimates of probable PTSD among this cohort prior to COVID-19 compared to during COVID-19 and investigates time trends in prevalence. Systematic multi-database literature searches were conducted to identify studies published between January 2017 and July 2023. Included studies reported the prevalence of probable PTSD, measured by validated screening tools, in clinical healthcare workers. Tw

Occupational Transmission of Extensively Drug-Resistant Tuberculosis, France.

We report occupational transmission of extensively drug-resistant tuberculosis (TB) to a healthcare worker in France receiving tumor necrosis factor α inhibitor therapy. Despite airborne precautions, the healthcare worker contracted TB working in a high-risk unit. This case underscores that immunocompromised healthcare workers should not be assigned to frontline TB care in high-risk settings.

Prescriptive or Interpretive Regulation at the Frontlines of Care Work in the "Three Worlds" of Canada, Germany and Norway.

This paper examines the tension between macro level regulation and the rule breaking and rule following that happens at the workplace level. Using a comparative study of Canada, Norway, and Germany, the paper documents how long-term residential care work is regulated and organized differently depending on country, regional, and organizational contexts. We ask where each jurisdiction's staffing regulations fall on a prescription-interpretation continuum; we define prescription as a regulatory tendency to identify what to do and when and how to do it, and interpretation as a tendency to delineat
