Grading of Recommendations Assessment, Development, and Evaluation and Confidence in Network Meta-Analysis showed moderate to substantial concordance in the evaluation of certainty of the evidence.

OBJECTIVES: Grading of Recommendations Assessment, Development, and Evaluation (GRADE) and Confidence in Network Meta-Analysis (CINeMA) are available to assess the confidence in network meta-analysis (NMA) results. They share common aspects, but their operationalization differs. We evaluated interrater reliability (IRR) among assessors, the approaches' concordance, and application time. METHODS: Two dichotomous ("seizure response", "ischemic stroke") and two continuous ("change in attention deficit hyperactivity disorder (ADHD)", "weight loss") outcomes with networks of different sizes, struc

Evaluation of grading of recommendations assessment, development, and evaluation approach adherence in clinical practice guidelines: a systematic survey of criteria and their applications.

OBJECTIVES: Many clinical practice guidelines (CPGs) claim to follow the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach, yet their actual adherence remains uncertain. We aimed to identify and evaluate existing criteria for evaluating adherence to GRADE in CPGs and describe the extent of adherence revealed by their application. STUDY DESIGN AND SETTING: We systematically searched PubMed, Embase, Web of Science, and the Cochrane Library from January 2004 to November 2024, supplemented by manual reviews of the GRADE handbook and related websites. Targeted Therapy for Antihistamine-Refractory Chronic Spontaneous Urticaria: A Critical Narrative Review and an Evidence-Graded Approach to Agent Selection.

Background/Objectives: Chronic spontaneous urticaria (CSU) is a mast-cell-driven disease with autoimmune mechanisms and an incompletely clarified aetiology, in which a substantial proportion of patients remain symptomatic despite second-generation H1-antihistamines; until recently, omalizumab was effectively the only targeted option, leaving a considerable non-responder fraction. Between February 2024 and September 2025 the landscape changed: dupilumab (anti-IL-4Rα) and the first oral Bruton tyrosine kinase (BTK) inhibitor, remibrutinib, were approved for CSU; omalizumab biosimilars reached th

Prognostic factors of mortality in patients with cancer infected with COVID-19: A systematic review and meta-analysis using the GRADE approach.

This study aimed to systematically synthesize the available evidence on prognostic factors affecting mortality in these patients. This review included cohort studies of adult patients with cancer diagnosed with Coronavirus disease 2019 (COVID-19). We searched MEDLINE and Embase for data from December 2019 to June 2025, and Cochrane Central Libraries and other databases for data to July 2023. Two independent reviewers screened references, extracted data, and assessed bias risk. The Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach was used to evaluate the certa

Unilateral approach to bilateral middle cerebral artery aneurysms: a large series and a proposed grading system to predict technical difficulties.

This study includes the surgical treatment of 31 patients with bilateral MCA (bMCA) aneurysms using a unilateral approach and is the second largest series in the literature. The main aim of this study is to present a proposed grading system to preoperatively predict the difficulties in this approach. The clinical files, radiological studies, operation records, intraoperative video recordings, complications and outcomes of 31 patients were retrospectively reviewed. In the first part of this study (2001-2010), we operated on 22 patients with bMCA aneurysms and were able to clip the contralateral

Multisocietal Consensus on the Use of Cytoreductive Surgery and HIPEC for the Treatment of Epithelial Ovarian Cancer: A GRADE Approach for Evidence Evaluation and Recommendation.

INTRODUCTION: The locoregional treatment of high grade serous ovarian cancer (HGSOC) comprises of cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC). Recent evidence form randomized trials, has led to controversy related to the use of HIPEC in addition to interval CRS (iCRS) and the role of secondary CRS (sCRS) in patients with the first platinum-sensitive recurrence from high-grade serous ovarian cancer (HGSOC). This multi-society consensus, coordinated by the Peritoneal Surface Oncology Group International (PSOGI) with inputs from ISSPP, SSO, ESSO, and IGCS, ev

An international unified approach to reporting and grading invasive breast cancer. An overview of the International Collaboration on Cancer Reporting (ICCR) initiative.

Standardised reporting of breast cancer key pathology data has become the norm in some parts of the world, but are based on national or regional guidelines that differ in certain aspects, resulting in divergent reporting practices and a lack of comparability of data internationally. The International Collaboration on Cancer Reporting (ICCR), a global alliance of major (inter-)national pathology and cancer organizations, have recently produced a new international dataset for the pathology reporting of breast cancer, including resection specimens with invasive cancer and ductal carcinoma in situ

Minimally invasive anterior and lateral transpsoas approaches for closed reduction of grade II spondylolisthesis: initial clinical and radiographic experience.

OBJECTIVE Minimally invasive anterior and lateral approaches to the lumbar spine are increasingly used to treat and reduce grade I spondylolisthesis, but concerns still exist for their usage in the management of higher-grade lesions. The authors report their experience with this strategy for grade II spondylolisthesis in a single-surgeon case series and provide early clinical and radiographic outcomes. METHODS A retrospective review of a single surgeon's cases between 2012 and 2016 identified all patients with a Meyerding grade II lumbar spondylolisthesis who underwent minimally invasive later

Recanalization of the Duodenum Following a High-Grade Post-Laparoscopic Cholecystectomy Duodenal Injury: Personalized Approach Presentation and Systematic Review of Management Options.

BACKGROUND: Laparoscopic cholecystectomy is considered the gold standard surgical technique for the treatment of gallbladder diseases worldwide. Nonetheless, despite its safety and popularity, it comes with certain complications. Duodenal injury is an extremely rare, but potentially fatal complication. The rarity of duodenal injuries, combined with under-reporting of incidents, has resulted in a scarcity of references in the international literature. CASE PRESENTATION: We present the case of a 72-year-old male patient, initially subjected to laparoscopic cholecystectomy that was intraoperativ

Impact of anti-seizure medication duration on postoperative seizures following supratentorial high-grade glioma resection: a mixed-model and tree-based approach.

PURPOSE: Postoperative seizures are a significant complication following glioma surgery. While prophylactic antiseizure medications (ASMs) are widely prescribed, the optimal duration of prophylaxis remains unclear. Current guidelines lack specificity regarding high-risk subgroups that may benefit from extended ASM therapy. Here, we aimed to determine whether ASM duration affects postoperative seizure occurrence and to identify patient subgroups in whom longer ASM prophylaxis significantly reduces seizure risk. METHODS: We conducted a retrospective cohort study of 206 adult high-grade glioma p

A Case of Acute Pancreatitis after Pancreatectomy in Grade C Leading to Walled-Off Necrosis Successfully Treated with Necrosectomy by Retroperitoneal Approach.

INTRODUCTION: Grade B or C post-pancreatectomy acute pancreatitis (PPAP) is associated with a higher incidence of postoperative complications and mortality. The reason for this is the activation of proteolytic processes that can lead to pancreatic destruction and the activation of systemic reactions that can have adverse consequences such as systemic inflammatory response syndrome, sepsis, and death. We report a case of a patient with Grade C PPAP with walled-off necrosis (WON) who was successfully treated with necrosectomy using a step-up approach. CASE PRESENTATION: A 73-year-old man was re

Safety and effectiveness of day-surgery laparoscopic cholecystectomy is still uncertain: meta-analysis of eight randomized controlled trials based on GRADE approach.

BACKGROUND: Currently, there is no consensus on whether laparoscopic cholecystectomy (LC) performed as day-surgery is safe and effective and can be considered as the standard for the management of symptomatic gallbladder disease. We conducted a meta-analysis of randomized controlled trials (RCTs) to evaluate the safety and effectiveness of this intervention based on the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. METHODS: We conducted a systematic search of several databases from their inception to November 10, 2016 for entries on the mortality, morbidi

Surgical approach and prognosis in patients with ≤2 cm grade 3 lung adenocarcinoma: a multivariable analysis of disease-free and overall survival.

BACKGROUND: High-grade histological patterns are established risk factors for poor prognosis in lung adenocarcinoma. Whether sublobar resection is oncologically equivalent to lobectomy in ≤2 cm grade 3 lesions remains unclear. This study compared the effects of different surgical approaches on perioperative outcomes and long-term survival. METHODS: We retrospectively reviewed 239 patients who underwent curative-intent resection for grade 3 invasive lung adenocarcinoma ≤2 cm between January 2014 and December 2023. Patients were stratified into lobectomy (n=121), segmentectomy (n=74), and wedge
