Oral contraceptives and the risk of gallbladder disease: a meta-analysis.

OBJECTIVES: This study was designed to assess the risk of gallbladder disease due to oral contraceptive use by conducting a thorough literature review. METHODS: Controlled epidemiologic studies published through March 1992 were systematically searched and evaluated. Of 25 studies (27 publications), 9 could stand the test of critical appraisal with respect to validity. Restriction to these studies was judged to circumvent publication bias at the same time. RESULTS: Oral contraceptive use is associated with a slightly and transiently increased rate of gallbladder disease. The results of six se

Drug-induced gallbladder disease. Incidence, aetiology and management.

A great variety of drugs is reported to induce gallbladder disease by various pathogenetic mechanisms. Early epidemiological studies indicated a doubled risk of gallbladder disease in women taking oral contraceptives. More recent studies, however, have failed to confirm those findings; these conflicting results might be explained by the different methods used to define gallbladder disease. It was shown that the lithogenic index of the bile is increased during intake of oral contraceptives. Estrogens cause hypersecretion of cholesterol in bile, due to increase in lipoprotein uptake by the hepat

[Differential ultrasound diagnosis of the healthy, inflamed and diseased gallbladder].

Sonography is helpful in detection of gallstones and can also provide information on pathological changes of the gallbladder. The diagnosis of changes of gallbladder walls is difficult without clinical symptoms. "Pharmakocholocystosonography" can provide information by diseases of gallbladder. It proves that the "asymptomatic" gallstone is one of symptoms of this disease. The sonography is helpful too in the diagnosis of the acute cholecystitis if the clinical symptoms are respected.

Age profiles of benign gallbladder disease in 2,000 patients.

Hospital charts and operative notes on 2,000 consecutive cholecystectomies performed between 1965-1984 for benign gallbladder disease were reviewed and computer analyzed. Patients were divided into five age categories (1-29, 30-49, 50-69, 70-79, 80 years and above). Fifteen variables were examined for each age category, including total number, sex, presenting symptoms (jaundice, acute cholecystitis, pancreatitis), intraoperative findings (presence of stones, common bile duct width and stones), operative procedures, bile cultures, histology, postoperative course and mortality. In addition to cr

Oral contraceptives and other risk factors for gallbladder disease.

Prior studies of the association between oral contraceptives (OCs) and gallbladder disease (GBD) have yielded conflicting results. To clarify this association, a retrospective (historical) cohort study was performed on a very large data base including 1980 and 1981 Medicaid billing data from the states of Michigan and Minnesota in which 138,943 users of OCs were compared with 341,478 nonusers. The crude relative risk (RR) and 95% confidence interval (CI) for symptomatic GBD resulting in medical care was 1.14 (CI 1.09 to 1.20), with a clear dose-response (P less than 0.001). Age markedly modifi

Gallbladder disease in patients on long-term parenteral nutrition.

Recent anecdotal reports suggest that total parenteral nutrition may be associated with an increased incidence of both acalculous cholecystitis and cholelithiasis. The validity of this association, however, has not been tested in a large population of patients on long-term total parenteral nutrition. Therefore, we assessed the incidence of gallbladder disease among our patients 15 yr and older who had received a minimum of 3 mo of total parenteral nutrition. Of the patients meeting these criteria, 128 were on total parenteral nutrition a mean of 13.5 mo. Nineteen had gallbladder disease before

The place of hepatobiliary isotope scanning in the diagnosis of gallbladder disease.

The present trend towards a more aggressive operative policy in acute cholecystitis has led to a search for more accurate methods of diagnosis. A possible contender in this field is hepatobiliary isotope scanning. Ninety-nine patients with right-sided upper abdominal pain were studied after the intravenous injection of 99Tcm-pyridoxylideneglutamate. In addition, contrast radiology was performed in 87 of these. Subsequently 23 were shown to have acute cholecystitis, 45 to have other gallbladder disease and 31 to have no evidence of cholecystic pathology. Isotope scanning was found to be reliabl

[Radiologic signs in the colon in disease of the gallbladder (author's transl)].

Acute and chronic inflammation of the gallbladder may simulate pseudotumors of the right half of the colon; this may at times be obvious on straight films of the abdomen. Analysis of the changes of relief in contrast-examinations of the colon will lead to the correct diagnosis. Inflammatory reactions of the colon mostly appear near the mesocolic tenia. Cholecystocolic fistulae are mostly due to perforation of stones, only exceptionally to tumors. Adhesions appear to be the precondition for malignant infiltration and stoneperforation.

Acute gallbladder disease; experience with 103 consecutive cases.

One hundred and three cases of acute cholecystitis in patients ranging in age from 19 to 88 years were reviewed. Operation was done in all cases. Seventy per cent of the patients were women. Primary cholecystectomy was done in 72.8 per cent of the series. Primary cholecystostomy was performed in the remainder, and one-fourth of these patients had a secondary cholecystectomy. No specific time, with relation to interval after onset of symptoms, was chosen for operation. Jaundice was present in 14.5 per cent of patients at the time of admittance to hospital. Serum amylase was above normal in five

Correlation Between Gastric and Gallbladder Helicobacter pylori Infection in South Indian Patients Undergoing Cholecystectomy for Gallbladder Disease.

BACKGROUND: Helicobacter pylori has been established as an etiological agent in gastric adenocarcinoma and mucosa-associated lymphoid tissue (MALT) lymphoma in the stomach. Recent reports have implicated this bacterium in the causation of benign and malignant gallbladder disease. We investigated the correlation between gallbladder and gastric mucosal H. pylori in patients with symptomatic cholelithiasis. AIMS: To estimate the prevalence of H. pylori colonization in the gallbladder and assess its correlation with gastric H. pylori in South Indian patients undergoing cholecystectomy. METHODS:

Gut Microbiota-Bile Acid Axis in Type 2 Diabetes-Associated Gallbladder Diseases: Mechanisms and Therapeutic Potential.

Gallbladder diseases spanning cholelithiasis, cholecystitis, and gallbladder cancer represent a clinically heterogeneous continuum in which type 2 diabetes mellitus (T2DM) acts as a key metabolic modifier. Conventional models centered on bile supersaturation alone do not sufficiently account for the persistent inflammation and inter-individual variability frequently observed in practice. Here, we synthesize emerging evidence implicating the gut microbiota-bile acid (BA) axis as an integrative mechanism linking metabolic dysregulation, barrier dysfunction, and biliary pathobiology in the diabet

Gallbladder wall thickening in patients with IgG4-related diseases, with special emphasis on IgG4-related cholecystitis.

OBJECTIVES: Gallbladder (GB) wall thickening sometimes occurs in patients with autoimmune pancreatitis (AIP), a condition for which the name, IgG4-related cholecystitis, was proposed. We examined the radiological findings of the GB in patients with IgG4-related diseases and clinical features of patients with GB wall thickening and presented a hypothesis of its pathogenesis. MATERIALS AND METHODS: GB wall thickening was defined by thickness ≥ 4 mm. GB wall thickness was examined in 258 patients with IgG4-related disease. Clinical and imaging findings of 200 patients with AIP with and without G

Epigenetic profiling of gallbladder cancer and gall stone diseases: Evaluation of role of tumour associated genes.

BACKGROUND: As on today, the global mortality rate of gallbladder cancer is still very high. Both genetic and epigenetic alterations play pivotal roles in the development of cancer. We selected seven tumour associated genes, implicated in other cancers, to assess their methylation status in gallbladder cancer and gallstone diseases. AIM OF STUDY: To study the promoter methylation of certain tumour associated genes in the molecular pathogenesis of gallbladder cancer and gall stone diseases. MATERIALS AND METHODS: Methylation specific PCR for seven tumour associated genes, viz., MASPIN, 14-3-3

[The significance of the duodenal papilla within the framework of diseases of gallbladder and bile ducts].

On the basis of 3185 biliary tract operations performed at the city hospitals of Braunschweig and Lüneburg, 12% of which included papillotomies, the frequency and pathogenesis of Vater's papilla stenosis is discussed. About 5% of all stenoses of Vater's papilla do not appear in connection with lithiasis in the biliary tract; these are defined as primary stenoses. 95% of Vater's papilla stenoses occur with lithiasis and inflammatory diseases of biliary tract. Stenosis of Vater's papilla is most often seen in connection with severe cholecystitis and pericholecystitis. There exists in most cases

Impact of the COVID-19 Pandemic on the Management of Gallbladder, Biliary Tract, and Pancreatic Diseases.

Introduction Biliary diseases are a major acute general surgical burden. Laparoscopic cholecystectomy is the gold standard surgical procedure, although it was discontinued during an outbreak. Effective management permits decisive therapy, symptom alleviation, and fewer hospitalizations and complications. Throughout the initial COVID-19 situation, surgical procedures for patients were delayed. Invasive services were required to employ conservative or non-operative therapy, which could lead to increased recurring presentations and biliary-pancreatic problems. Aim Examining the impact of COVID-19

Our initial single port robotic cholecystectomy experience: A feasible and safe option for benign gallbladder diseases.

BACKGROUND: Although single-port laparoscopic cholecystectomy has been performed for over 25 years, it is still not popular. The narrow working space used in this surgery limits the movement of instruments and causes ergonomic challenges. Robotic surgery not only resolves the ergonomic challenges of single-port laparoscopic surgery but is also considered a good option with its additional technical advantages, like a three-dimensional display and not being affected by tremors. However, the extent to which these technical and ergonomic advantages positively affect the surgical outcomes and how s

Key Imaging Findings for the Prospective Diagnosis of Rare Diseases of the Gallbladder and Cystic Duct.

There are various diseases of the gallbladder and cystic duct, and imaging diagnosis is challenging for the rare among them. However, some rare diseases show characteristic imaging findings or patient history; therefore, familiarity with the imaging presentation of rare diseases may improve diagnostic accuracy and patient management. The purpose of this article is to describe the imaging findings of rare diseases of the gallbladder and cystic duct and identify their pathological correlations with these diseases.

Population-based cohort study of variation in the use of emergency cholecystectomy for benign gallbladder diseases.

BACKGROUND: The aims of this prospective population-based cohort study were to identify the patient and hospital characteristics associated with emergency cholecystectomy, and the influences of these in determining variations between hospitals. METHODS: Data were collected for consecutive patients undergoing cholecystectomy in acute UK and Irish hospitals between 1 March and 1 May 2014. Potential explanatory variables influencing the performance of emergency cholecystectomy were analysed by means of multilevel, multivariable logistic regression modelling using a two-level hierarchical structu
