The SymbioCare initiative: management of irritable bowel syndrome, comparison between Italian gastroenterologists and general practitioners.

BACKGROUND: Irritable bowel syndrome (IBS) is a common medical condition characterized by different phenotypes. Diarrhea is usually prevalent in IBS patients, but constipation and meteorism are also common. Pharmacological therapies do not modify the IBS natural history. Thus, food supplements are used in clinical practice. The present Italian educational activity investigated the characteristics of IBS patients and compared the attitudes of gastroenterologists (GEs) and general practitioners (GPs). METHODS: Fifty-three gastroenterologists and 42 GPs enrolled 2442 IBS patients. This initiativ

Investigating Human Liver Tissue-Resident Memory T Cells from the Perspectives of Gastroenterologists and Hepatologists.

Liver tissue-resident memory T (TRM) cells play a pivotal role in hepatic immune responses. Their unique residence within liver sinusoids allow continuous antigen surveillance. In this review, we highlight the role of liver TRM cells in protective immunity and disease pathology. Comparisons between human and murine liver TRM cells reveal species-specific characteristics, suggesting the need for human-focused studies. One key finding is the involvement of liver TRM cells in viral hepatitis, where they can both control infection and contribute to liver damage. Liver TRM cells also exhibit dual r

A National Survey on Paediatric Gastroenterologists' Role in Caring for Anorectal Malformations and Hirschsprung's Disease.

AIM: Most paediatric surgeons regard themselves as crucial and responsible for the follow-up of patients with anorectal malformations (ARM) and Hirschsprung's disease (HD). In this study, we explored to what extent paediatric gastroenterologists (PG) want to be involved in the care, if they experienced any knowledge gaps, and how they preferred to increase their knowledge. METHODS: All Norwegian paediatric departments received a questionnaire covering different aspects of follow-up of patients with ARM and HD. One PG was asked to answer on behalf of the department. RESULTS: 13/18 (72%) depar

Ankaferd blood stopper: A novel additional strategy for less experienced gastroenterologists in gastrointestinal bleeding treatment.

The Ankaferd Blood Stopper (ABS) proves effective in managing various bleedings, particularly in surgical and dental procedures. This study assesses ABS efficacy endoscopically by less-experienced endoscopists for non-variceal upper gastrointestinal bleeding (NVUGB). Between 2016 and 2021, our hospital's Gastroenterology Department Endoscopy Unit conducted a retrospective data analysis of 653 patients who underwent endoscopy for NVUGB. The study included 202 patients who underwent endoscopic interventions performed by endoscopists with less than 3 years of experience. Based on the method used

Review article: restorative proctocolectomy, indications, management of complications and follow-up--a guide for gastroenterologists.

BACKGROUND: Restorative proctocolectomy with ileal pouch-anal anastomosis is the procedure of choice for the majority of patients with ulcerative colitis who require surgery. Over 2500 patients in the UK have undergone restorative proctocolectomy. It is now increasingly being performed in district general hospitals as well as in specialist inflammatory bowel disease units. Gastroenterologists are increasingly involved in the management of patients following restorative proctocolectomy. AIM: To provide gastroenterologists with a clear understanding of the investigation and evidence-based manag

Role of Endoscopy in Inflammatory Bowel Disease: What Every Gastroenterologist Should Know.

Endoscopy plays a pivotal role in managing inflammatory bowel disease (IBD). The role of endoscopy has evolved over the years, from diagnostic to surveillance to now doing therapeutic procedures to manage patients with IBD. From the initial diagnostic endoscopy, describing the extent and severity of the disease is an essential step in differentiating between Crohn's disease and ulcerative colitis. Patients with IBD frequently undergo surgeries, and understanding various postoperative configurations and performing endoscopy in postoperative patients can pose a challenge. A thorough understandin

Small bowel vasculitis? what a gastroenterologist should know - from diagnosis to management.

PURPOSE OF REVIEW: This article provides gastroenterologists with an overview of small bowel involvement in systemic vasculitis. Though various vasculitides can impact the small bowel, we highlight those with a more frequent and clinically significant GI involvement. RECENT FINDINGS: Recent advances, including increased accessibility to cross-sectional imaging, capsule endoscopy and device-assisted enteroscopy, have improved detection of gastrointestinal manifestations in systemic vasculitis. Studies have also explored the genetic and inflammatory pathways involved in these diseases, although

Narrative review of the role of gastroenterologist in the diagnosis, treatment and palliation in gastric and gastroesophageal cancer.

Esophageal cancer (EC) and gastric cancer (GC) carry a high mortality rate. Unfortunately, a majority of patients are asymptomatic and at the time of diagnosis, the disease may invariably be in its advanced stages with limited curative options. Thus, it is imperative to recognize certain risk factors including gastroesophageal reflux disease (GERD), male gender, pre-existing Barrett's esophagus, smoking history, obesity, Helicobacter pylori infection, atrophic gastritis among others for both EC and GC, intervene on time with screening and surveillance modalities if indicated and optimize treat

What does a gastroenterologist need to know in the time of SARS-CoV-2 pandemia?

Coronavirus disease 2019 (COVID 19) is an emerging infectious disease caused by a novel coronavirus SARS-CoV2 that was first identified in Wuhan, China 2019 and that led to a worldwide pandemia. In addition to typical respiratory signs (dry cough, shortness of breathing), some patients may develop gastrointestinal and hepatological complications including diarrhea or acute hepatitis, respectively. Due to the close contact to the patient's secretion, the gastroenterologists are at increased risk of getting the infection. Therefore, a proper individual risk stratification before every endoscopic

Laryngopharyngeal Reflux and Functional Laryngeal Disorder: Perspective and Common Practice of the General Gastroenterologist.

Laryngopharyngeal reflux (LPR) is an extraesophageal variant of gastroesophageal reflux disease that is associated with chronic cough, hoarseness, dysphonia, recurrent throat clearing, and globus pharyngeus. Due to nonspecific symptoms, laryngoscopy is often performed to rule out malignancy, and the diagnosis of LPR is considered with any signs of laryngeal inflammation. However, laryngoscopic findings have high interobserver variability, and, thus, most patients are tried on an empiric course of acid-suppressive therapy to see whether symptoms resolve. In this article, which focuses on the pe

[Tonsillar actinomycosis as a rare cause of oral malodor. Diagnosis beyond a gastroenterologist's nose].

BACKGROUND: Up to 30% of patients seen by dentists suffer from oral malodor. Part of them report serious distress and a sometimes paradoxical discrepancy in subjective and objective perception of symptoms. Less often, patients with oral malodor are primarily seen by general practitioners and specialists like gastroenterologists or ear, nose and throat (ENT) doctors. Correct characterization of the underlying disease and an adequate diagnosis are made most successfully through interdisciplinary cooperation.
