General practitioners' knowledge of and attitudes to inflammatory bowel disease.

BACKGROUND: Inflammatory bowel disease (IBD) is a chronic disease requiring long-term management. General practitioners (GPs) are often the first point of contact for initial symptoms and flares. Thus we assessed GPs' attitudes to and knowledge of IBD. METHODS: A state-wide postal survey of GPs was performed collecting demographic details, practice and attitudes in IBD-specific management and knowledge. RESULTS: Of 1800 GPs surveyed in South Australia, 409 responded; 58% were male, 80% Australian trained and 73% practised in metropolitan areas. Most GPs (92%) reported seeing zero to five IBD

Knowledge of Iranian general practitioners for acetaminophen dosing in children.

OBJECTIVE: This study aims to explore the knowledge of a randomly selected cohort of Iranian general practitioners (GPs) on the topic of acetaminophen dosing for fever in children. METHODS: In this cross-sectional study, which was conducted in March 2011 in Isfahan (Iran) 515 GPs who were practicing general family medicine were sent a self-administered 10-item questionnaire about the usage of acetaminophen for fever in pediatric patients. Questions were designed to evaluate the knowledge of GPs on acetaminophen dosing amount and interval for fever in children and were formatted as multiple ch

Carpal tunnel syndrome diagnosed by general practitioners: an observational study.

The aim of this study was to investigate the results of both clinical testing and standardised nerve conduction studies performed on patients with Carpal tunnel syndrome (CTS) complaints, who had been referred to the neurologist by their general practitioners. Analysis of the data of neurological examination and electrodiagnostic tests (EDX) were performed on patients that had been referred by general practitioners. A total of 232 patients with clinically defined CTS, who had been referred by general practitioners, were seen by a neurologist and subsequently underwent electrodiagnostic testing

[The Dutch College of General Practitioners' practice guideline on arthritis].

The Dutch College of General Practitioners' new practice guideline on arthritis combines arthritis and gout. One of the important improvements in this guideline is that it recommends that patients be referred to the rheumatologist as soon as 4 weeks after being seen, or earlier, if rheumatoid arthritis is likely or suspected. Obviously, these early referrals are related to the large increase in effective treatment options in rheumatoid arthritis, particularly after early treatment (window of opportunity). The new guideline relating to gout advocates the use of oral prednisone as the second cho

[Inquiry among general practitioners' knowledge regarding clean intermittent self-catheterization].

INTRODUCTION: Clean intermittent self-catheterization is the recommended mode of bladder voiding in patients with urinary retention. Hence, this procedure should be well known and understood by the general practitioners (GP) who are responsible for monitoring such patients. OBJECTIVE: To assess GP' state of knowledge regarding clean intermittent self-catheterization and to make recommendations for dealing with problems that occur during patient follow-up. METHOD: A questionnaire focusing on the practice of clean intermittent catheterization was sent to 910 family GP in the Franche-Comté regi

Value of histopathologic analysis of subcutis excisions by general practitioners.

BACKGROUND: Only around 60% of skin lesions excised by GPs are referred to a pathologist. Clinical diagnoses of skin excisions by GPs may not be very accurate. Subcutis excisions are rarely done by GPs, and there is hence little information in the literature on the histopathological yield of subcutis excisions by GPs with regard to malignancies. The aim of this study was to evaluate the yield of histopathological investigation of a relatively large group of subcutis excisions by GPs, with special emphasis on discrepancies between clinical and histopathological diagnoses of malignancy. METHODS

General practitioners' referral pattern for children with acute facial paralysis.

INTRODUCTION: The relative incidence of paediatric Bell's palsy is two to four times less than the condition in adults. The number encountered in our otolaryngology department falls short of the above prediction. This could either reflect the general practitioners' (GPs) patterns of referral or a lower incidence in our locality. METHODS: Postal questionnaires were sent to our local GPs to determine their referral practice when managing children presenting with acute facial paralysis. RESULTS: Of 233 questionnaires, 172 (74 per cent) were returned and analysed. Fifty-four per cent of GPs refe

Joint and soft tissue injections: a survey of general practitioners.

OBJECTIVES: To determine the type of joint and soft tissue injections carried out by general practitioners (GPs) in the Bath area and factors affecting activity. METHODS: A questionnaire was sent to 360 GPs requesting information on injections carried out during the previous 12 months, referral pathways for injection, barriers to injecting and training. RESULTS: We received 251 replies. The commonest injections were for tennis elbow, glenohumeral joint, knee, supraspinatus tendonitis and carpal tunnel. The majority of GPs (66.4%) carry out most injections themselves, 26.3% refer to a colleag

Normalisation of unexplained symptoms by general practitioners: a functional typology.

BACKGROUND: Patients often present in primary care with physical symptoms that doctors cannot readily explain. The process of reassuring these patients is challenging, complex and poorly understood. AIM: To construct a typology of general practitioners' (GPs') normalising explanations, based on their effect on the process and outcome of consultations involving patients with medically unexplained symptoms. DESIGN OF STUDY: Qualitative analysis of audiotaped consultations between patients and GPs. SETTING: Seven general practices in Merseyside, United Kingdom. METHODS: Transcripts of audiota

Defining patients as palliative: hospital doctors' versus general practitioners' perceptions.

There appears to be a lack of consensus on the classification of individual patients as 'for palliative care', although the extent of this is unknown. General practitioners (GPs) of 213 patients with a palliative diagnosis of lung or colo-rectal cancer were sent a one-page questionnaire to assess information sent by hospital doctors, and to establish the GPs' perception of patients' palliative status. A total of 185 questionnaires were returned (87% response rate). Of those GPs receiving information from the hospital, one in four rated the adequacy as less than positive; 26% reportedly receive

[Summary of the Dutch College of General Practitioners' "Gout" Standard].

The typical form of acute gout can be clinically diagnosed. The term 'complicated gout' is used if there are more than three acute attacks of gout per year, tophi or urate stones in the urinary tracts. In the case of recurrent probable acute gout, a diagnostic fine needle aspirate from the joint during an attack is indicated. First choice treatment of acute gout consists of NSAIDs. Colchicine is the second choice treatment and the third choice treatment consists of corticosteroids. Excessive alcohol use should be limited. Treatment of chronic gout depends on the uric acid excretion in the 24-h

Variant Creutzfeldt-Jakob disease--a problem for general dental practitioners?

Over a hundred deaths from variant Creutzfeldt-Jakob disease (vCJD) have now been recorded. The incubation period for vCJD may be up to 40 years and the number of asymptomatic carriers in the population could be as many as 100,000. Confirmed iatrogenic transmission of other human transmissible spongiform encephalopathies raises the possibility of cross-infection from apparently healthy persons who are incubating vCJD. Decontamination techniques routinely used in general dental practice are incapable of inactivating the infective protein responsible for transmitting the disease. So far, no evid

Cystic fibrosis carrier testing in early pregnancy by general practitioners.

OBJECTIVE: To assess the feasibility of genetic counselling in general practice by using cystic fibrosis carrier screening at the booking appointment as an integral part of routine antenatal care and as a paradigm for the wider participation of general practitioners in medical genetics. DESIGN: Maternal testing (male partner tested only if woman screens positive) and couple testing for cystic fibrosis carrier status in the antenatal population attending one general practice and, later, in a further six (outreach) practices also. SETTING: Two partner urban training practice (pilot practice) i

General practitioners' experiences of patients with drug and alcohol problems.

Convergent focus groups, a qualitative research methodology, were used to determine what general practitioners considered to be the main obstacles and disincentives to the effective management of persons with drug and alcohol problems. Seven groups were conducted, involving 44 general practitioners whose practices were based in representative parts of metropolitan Sydney and a rural area of New South Wales. The study revealed that most doctors are not as threatened, resistant or pessimistic about drug and alcohol intervention as much of the previous work in the area would suggest. However, the

Huntington's chorea: general practitioners' knowledge about individuals' at-risk status.

We assessed general practitioners' knowledge about the at-risk status of offspring of persons suffering from Huntington's chorea. Of 97 practitioners who were questioned about only one at-risk individual, 16 were aware of the person's at-risk status, while nine of 26 practitioners who were questioned about more than one at-risk person were aware of it. The results indicate that prophylactic action can be achieved more efficiently by registration of families in which Huntington's chorea occurs, followed by genetic counselling.
