Geriatric Genital Dermatology.

As women age, hormonal changes set the stage for a variety of vulvovaginal pathologies. Health care providers in long-term care facilities should be able to recognize and treat these conditions, especially because residents may be unable to communicate their discomfort. The objective of this article is to highlight the major vulvovaginal conditions affecting older women and provide up-to-date information on treatment for providers in long-term care facilities.

Geriatric Psychiatric Emergencies.

Geriatric patients, those 65 years of age and older, often experience psychiatric symptoms or changes in mentation as a manifestation of an organic illness. It is crucial to recognize and treat delirium in these patients as it is often under-recognized and associated with significant morbidity. Iatrogenic causes of altered mentation or delirium due to medication adverse reactions are common. Treatment of the underlying cause, creating an environment conducive to orientation, and minimizing agitation and discomfort are first-line interventions. Antipsychotics are first-line pharmacologic interv

Suicidality in the geriatric population.

Suicide in older adults is a major global concern in both public and mental health. With an ageing population on the rise, a surge in suicidal deaths is predicted in the coming years. The objectives of this paper are to review the risk factors, protective factors, assessment rating scales and current prevention strategies in the geriatric population. The identification of modifiable risk factors and strengthening of protective factors as well as staging according to suicidal ideation, behaviors and/or attempt(s) are necessary to devise appropriate personalized interventions in vulnerable older

Geriatric sudden deafness.

PURPOSE: Sudden sensorineural hearing loss, briefly sudden deafness (SD), in the elderly remains less investigated despite rapidly aging population in most countries around the world. This study investigated whether aging process affects the treatment outcome of SD in the elderly. PATIENTS AND METHODS: Eighty patients with SD, comprising 40 geriatric patients aged >65 years and 40 non-geriatric patients aged 55-64 years, were enrolled in this study. All patients underwent an inner ear test battery including audiometry, and cervical vestibular-evoked myogenic potential (cVEMP), ocular VEMP (oV

Geriatric Pharmacology: An Update.

An aging worldwide population demands that anesthesiologists consider geriatrics a unique subset of patients requiring customization of practice. This article reviews the current literature investigating physiologic changes of the elderly that affect pharmacokinetics and pharmacodynamics. Changes in drug absorption, distribution, metabolism, and excretion are discussed as well as the ultimate effects of medications. Implications for practice regarding specific anesthetic and analgesic drugs are addressed. Despite the immense body of research that contributes to understanding of geriatric pharm

Driving in the Geriatric Population.

Driving helps older adults stay mobile and independent. The risk of being injured or killed in a motor vehicle crash increases with age. This trend has been attributed more to an increased susceptibility to injury and medical complications among older drivers rather than an increased risk of crash involvement. Older adults tend to take steps to stay safe on the road. They tend to have a high incidence of seat belt use and a lower incidence of impaired driving. Furthermore, older drivers self-select safer driving conditions by avoiding night, bad weather, and high-speed roads compared with youn

Geriatric Anorexia Nervosa.

Eating disorders are not commonly diagnosed in individuals aged >50 years, yet they are associated with significant psychiatric comorbidities and overall morbidity. Anorexia nervosa is the most common eating disorder among this age group, and women are affected most often. We present the fatal case of a 66-year old woman with severe malnutrition and newly diagnosed anorexia nervosa. Inpatient refeeding was unsuccessful, and she succumbed to multisystem organ failure. The timely recognition of eating disorders among older people is important for family physicians who care for patients across th

[Dizziness in geriatric patients].

Dizziness is a common complaint in geriatric patients. Age-related changes in organs of balance control make the elderly more susceptible to diseases affecting the same system causing symptoms as dizziness, balance disturbance, fall and syncope. Work-up should be multifactorial and is feasible in geriatric outpatient clinics. Evidence-based interventions are available. New studies have found high frequency of vestibular dysfunction among old fall patients and suggest an association between vestibular dysfunction and orthostatic hypotension. Further research in this area is needed.

Incremental geriatric assessment.

Older adults value (1) independence and the ability to make their own decisions, (2) mobility (the ability to travel outside or simply inside the home), (3) family and friends and the time spent with those persons who are important to them, (4) ethnicity, religion, and spirituality, and (5) home, wherever that might be. The importance of recognizing each person's individuality cannot be overemphasized. The method of incremental assessment presented in this article and summarized in Box 9 is intended to provide the office-based clinician with sufficient information to make decisions regarding t

Vasculitis in the geriatric population.

Vasculitis represents an uncommon but important group of disease entities that may affect older patients. The most common systemic vasculitic disease in humans is giant cell arteritis, a disease process seen almost exclusively in patients over the age of 50. Vasculitic disease in geriatric patients presents unique challenges with regard to diagnosis and treatment. A thorough understanding of the vasculitic disease entities that may affect older patients as well as their diagnosis and management is essential in minimizing disease and treatment related morbidity and mortality.

Geriatric failure to thrive.

In elderly patients, failure to thrive describes a state of decline that is multifactorial and may be caused by chronic concurrent diseases and functional impairments. Manifestations of this condition include weight loss, decreased appetite, poor nutrition, and inactivity. Four syndromes are prevalent and predictive of adverse outcomes in patients with failure to thrive: impaired physical function, malnutrition, depression, and cognitive impairment. Initial assessments should include information on physical and psychologic health, functional ability, socioenvironmental factors, and nutrition.

[Geriatric eye diseases].

Ageing of the eye affects all ocular structures. Presbyopia is an early manifestation of physiological ageing resulting in the impossibility to read without additional spectacle correction. Some systemic disorders may interfere with the physiological ageing of the eye and cause blindness. In the western countries blindness is caused by age-related macular degeneration (26%), glaucoma (20.5%), cataract (11.2%), diabetic retinopathy (8.9%) and ischemic optic neuropathy (4 %).

Pediatric and Geriatric Tinnitus.

The subject of tinnitus in the population extremes-children and the elderly-is ignored by the literature, probably because children do not complain of tinnitus spontaneously, whereas it is only one challenge among other major health problems in the elderly. A short review of the literature on this subject is presented. Presbytinnitus, defined as tinnitus that accompanies the progressive hearing loss of presbycusis is classified as: type 1 (normal aging affecting the cochlea), and type II (preexistent sensorineural hearing loss accompanied by multiple systemic complaints, especially of sensory

The geriatric acute abdomen.

An acute abdomen requires prompt diagnosis and therapy in all age groups. In the elderly population, rapid recognition is particularly important, because the patient may have delayed seeking medical care and may have uncommon illnesses complicating the ultimate course. The causes of an acute abdomen in this population are similar to those found in younger patients with some differences in frequency. In each case, a sense of urgency to make the proper diagnosis and then institute appropriate therapy is critical.

Geriatric urinary incontinence.

Urinary incontinence (UI) is now recognized as a prevalent, physically and emotionally disruptive, and costly health problem in the geriatric population. Because incontinence may be a manifestation of a subacute or reversible process within or outside of the lower urinary tract, and because effective treatment is available, it is important for primary care physicians to identify and appropriately assess incontinence in their geriatric patients. The initial evaluation of an incontinent geriatric patients. The initial evaluation of an incontinent geriatric patient includes a targeted history and

The geriatric dental patient.

The life expectancy of our citizens has increased due to better living conditions and medical care. This poses new challenges to dental surgeons because geriatric dental patients have different needs brought on by ageing. Tooth loss, periodontal disease, alveolar bone resorption, mucosal changes, edentulism are just some of the problems faced by the elderly. Recognition of the dental needs of this special category of the population compels us to bear the responsibility of treating them now and in the future. We require a gerodontology programme that emphasizes prevention, maintenance of the or

Violence in geriatric patients.

Two hundred twenty-two patients admitted to a geriatric psychiatry unit were surveyed for the commission of acts dangerous to others. Eighteen of these patients had used either guns or knives in acts of violence. Organic illness accounted for a minority of such acts, the majority having been perpetrated by patients with functional diagnoses of late paraphrenia, schizophrenia, or mania. The most dangerous patients were those who, in a clear sensorium, experienced paranoid delusions, hallucinations, or both, and believed that they were in danger of being attacked. One hundred twenty-one aggressi
